Outcomes in checkpoint inhibitor induced neuromuscular immune related adverse events: A case series.

Immune-checkpoint inhibitors, a mainstay of treatment of solid tumors, rarely lead to activation of the host immune system against another target, causing autoimmune inflammation of other end organs known as immune-related adverse events (ir-AEs). When ir-AEs are directed at the neuromuscular system, clinical consequences include myositis and myasthenic syndromes. In this article, we describe three cases presenting with neuromuscular ir-AEs. Two patients had lung adenocarcinoma and were treated with pembrolizumab. One patient had gastroesophageal junction adenocarcinoma treated with nivolumab.

Host cell targets in HCV therapy: novel strategy or proven practice?

The development of novel antiviral drugs against hepatitis C is a challenging and competitive area of research. Progress of this research has been hampered due to the quasispecies nature of the hepatitis C virus, the absence of cellular infection models and the lack of easily accessible and highly representative animal models. The current combination therapy consisting of interferon-alpha and ribavirin mainly acts by supporting host cell defence. These therapeutics are the prototypic representatives of indirect antiviral agents as they act on cellular targets. However, the therapy is not a cur

Nanoarmored Probiotic for Inflammatory Bowel Disease Bacteriotherapy via a Dual-Targeting Host-Microbiota Reprogramming.

Inflammatory bowel disease (IBD) involves oxidative stress, chronic inflammation, and gut microbiota dysbiosis, complicating therapeutic intervention. Herein, we engineer a robust living therapeutic platform via a biomimetic nanoarmoring strategy. A uniform FDA-approved calcium carbonate (CaCO3) nanocoating is in situ mineralized on the surface of the probiotic Escherichia coli Nissle 1917 (EcN) preassembled with the flavonoid luteolin (Lut), creating the nanocomposite EcN-Lut/CaCO3. This nanoarmoring strategy enhances bacterial viability under simulated gastrointestinal conditions (a 100-fold

Autophagy-ferroptosis crosstalk in sepsis: metabolic pathways, redox injury, and host-directed antioxidant nanomedicine.

Sepsis is a dynamic syndrome of infection-driven metabolic and immune dysregulation in which oxidative stress can escalate into an "oxidative storm," promoting organ dysfunction and maladaptive host responses. Within this context, ferroptosis represents a metabolically constrained form of regulated necrotic cell death driven by iron-dependent lipid peroxidation, linking redox collapse to tissue injury in sepsis. Emerging evidence suggests that autophagy critically shapes ferroptosis susceptibility by regulating intracellular iron mobilization, membrane lipid substrate availability, mitochondri

Transcriptome signature for host directed antiviral reprogramming by Nimba & Triphala in macrophage Dengue virus infection models.

Dengue virus (DENV) is responsible for hundreds of millions of infections per year, but no clinically approved antiviral therapy exists, and direct-acting candidates are limited by toxicity and viral genetic diversity. Traditional medicinal preparations such as Nimba/Neem (Azadirachta indica) and polyherbal Ayurvedic preparation Triphala (a combination of Terminalia chebula, Terminalia bellirica and Phyllanthus emblica) contain multiple metabolites with antiviral and antioxidant properties but their mechanisms of action are not fully characterised. Here, we combined cell-based antiviral assays

Methyl jasmonate promotes host defense by activating autophagy during Mycobacterium tuberculosis infection.

BACKGROUND: Jasmonates are a class of plant hormones that play multiple roles in plant growth, development, stress responses, and antimicrobial activity. However, their role in antimicrobial responses in mammalian cells and in vivo, particularly against Mycobacterium tuberculosis (Mtb), remains unclear. Tuberculosis remains a global challenge in human health, particularly with the rise of multidrug-resistant strains. Thus, exploring novel therapeutic agents is urgently warranted. In this study, we investigated the antimicrobial effects of methyl jasmonate (MeJA) against Mtb to evaluate its pot

Resveratrol combats Salmonella infection via liberating vacuole-enclosed bacteria for enhanced autophagic clearance and blocking HMGA2-mediated G2/M arrest.

The escalating crisis of antimicrobial resistance necessitates a paradigm shift toward host-directed therapies (HDTs) that circumvent pathogen evasion strategies. Here we show that the natural polyphenol resveratrol clears intracellular Salmonella typhimurium (S. typhimurium) by orchestrating a multipronged strategy involving Salmonella-containing vacuole (SCV) disruption via ERK1/2 inhibition to liberate bacteria for enhanced autophagic clearance, concurrently reversing pathogen-induced G2/M cell cycle arrest. We identify High Mobility Group AT-Hook 2 (HMGA2) as a critical, previously unrecog

Gut microbiota homeostasis alleviates mycobacterial granuloma pathology in zebrafish.

Growing evidence links the gut microbiota to host immune regulation and tuberculosis (TB) progression; however, its specific impact on the formation and necrosis of TB granulomas remains poorly defined. In this study, we established an adult zebrafish model of antibiotic-induced gut microbiota dysbiosis followed by Mycobacterium marinum (M.m) infection to investigate how gut microbiota perturbation influences host resistance to mycobacterial infection. Our results demonstrated that antibiotic-induced gut microbiota dysbiosis significantly increased the mycobacterial burden in zebrafish, thereb

HCV 5-Methylcytosine Enhances Viral RNA Replication through Interaction with m5C Reader YBX1.

Hepatitis C virus (HCV) is a positive-stranded RNA virus that mainly causes chronic hepatitis, cirrhosis and hepatocellular carcinoma. Recently we confirmed m5C modifications within NS5A gene of HCV RNA genome. However, the roles of the m5C modification and its interaction with host proteins in regulating HCV's life cycle, remain unexplored. Here, we demonstrate that HCV infection enhances the expression of the host m5C reader YBX1 through the transcription factor MAX. YBX1 acts as an m5C reader, recognizing the m5C-modified NS5A C7525 site in the HCV RNA genome and significantly enhancing HCV

TRIM21 blocks DDX3X-driven stress granule formation during Mycobacterium tuberculosis infection via a non-canonical E3 ligase mechanism.

Tripartite motif-containing 21 (TRIM21) is an E3 ubiquitin ligase that binds viral and host proteins to mediate antiviral defense. Its role in Mycobacterium tuberculosis (Mtb) infection, however, is unclear. Here, our analysis of human TB transcriptomic datasets showed that TRIM21 expression levels correlate with TB progression in human patients. We further demonstrated that TRIM21 deficiency suppresses Mtb growth in both macrophages and a murine model of tuberculosis (TB). Mechanistically, TRIM21 limits host resistance to Mtb through a non-canonical pathway distinct with its E3 ligase activit

Principles of Immunotherapeutic Drug Discovery for Severe Infections Using Extreme Phenotypes.

Therapeutically modifying host responses in severe infection is an unmet need in critical care, either to augment pathogen clearance or limit immunopathology. Drug discovery in this space is impeded by the complexity of immune responses during infection. Identifying and studying extreme infection phenotypes is an emerging approach to negotiate this problem: by amplifying biological signal over noise, this can expedite the discovery of host determinants of severe disease or protection. Examples discussed in this article include critical Covid-19, HIV elite controllers, inborn errors of interfer

The antimycobacterial and healing effect of sorafenib through pro-apoptotic and immunomodulatory activities.

Tuberculosis is caused by the bacterium Mycobacterium tuberculosis (Mtb). Emergence of drug resistance in Mtb requires continuous enrichment of anti-tubercular medication. Inclusion of host-directed therapies holds considerable promise in this context. Sorafenib (SRB) is a multi-kinase inhibitor targeting VEGF receptor kinase, Raf, MEK, and extracellular signal-regulated kinase (ERK) signaling cascade to treat several types of cancer, including hepatocellular carcinoma. We have previously established that SRB allosterically inhibits ornithine acetyltransferase (MtArgJ), an essential enzyme in

The value of the monocyte-to-lymphocyte ratio and osteopontin (SPP1) in tuberculosis treatment response monitoring.

There is an urgent need to rapidly diagnose tuberculosis (TB) disease and effectively monitor anti-TB treatment responses. Host-directed therapy (HDT) is a promising platform to mitigate challenges in TB diagnosis and anti-TB treatment response monitoring. Identifying changes in systemic proteins and immune cell distributions during the disease is an integral aspect of developing targeted therapies. Here, samples were collected from healthy individuals (CTRL) [n = 32 plasma, n = 9 bronchoalveolar lavage (BAL)] and newly diagnosed TB patients (TB treatment group) [n = 82 plasma, n = 28 BAL] to

Caffeine-mediated CD39+ Treg activation via the CD39-adenosine receptor pathway is a novel risk factor for pulmonary tuberculosis.

BACKGROUND: The global impact of pulmonary tuberculosis (PTB) is compounded by a limited understanding of modifiable risk factors. While caffeine is the most consumed psychoactive substance, its causal relationship with PTB and the underlying immunological mechanisms remain unknown. METHODS: A three-tiered approach was used: 1) two-sample Mendelian randomization (TSMR) was used to analyze 486 metabolites and 731 immune cells for PTB causality (inverse variance weighting was the primary method with reverse MR and Bonferroni correction), 2) single-cell RNA sequencing (scRNA-seq) and bulk RNA-se

RACK1 in host immune response to infections: molecular mechanisms and therapeutic potentials.

The global healthcare system faces increasing threats from emerging and re-emerging pathogens. Current understanding of host-pathogen interactions and underlying immune mechanisms remains incomplete, which hinders the development of effective diagnostic tools and therapeutic strategies. The receptor for activated C kinase 1 (RACK1) is a multifunctional scaffolding protein that integrates diverse signaling pathways, modulates translation, and regulates key cellular processes. Despite accumulating evidence implicating RACK1 in host immune response to infections, its multifaceted roles and mechan

Epigenetic code during mycobacterial infections: therapeutic implications for tuberculosis.

Epigenetics involves changing the gene function without any change in the sequence of the genes. In the case of tuberculosis (TB) infections, the bacilli, Mycobacterium tuberculosis (M.tb), uses epigenetics as a tool to protect itself from the host immune system. TB is a deadly disease-causing maximum death per year due to a single infectious agent. In the case of TB, there is an urgent need for novel host-directed therapies which can effectively target the survival and long-term persistence of the bacteria without developing drug resistance in the bacterial strains while also reducing the dur

Structure-function analysis of enterovirus protease 2A in complex with its essential host factor SETD3.

Enteroviruses cause a number of medically relevant and widespread human diseases with no approved antiviral therapies currently available. Host-directed therapies present an enticing option for this diverse genus of viruses. We have previously identified the actin histidine methyltransferase SETD3 as a critical host factor physically interacting with the viral protease 2A. Here, we report the 3.5 Å cryo-EM structure of SETD3 interacting with coxsackievirus B3 2A at two distinct interfaces, including the substrate-binding surface within the SET domain. Structure-function analysis revealed that
