Recent research on HIV prevention, regardless of the population, has increasingly recognized the relevance of contextual factors in determining HIV risk. Investigating such factors among female sex workers (FSW) is especially relevant in the South Indian state of Andhra Pradesh, where HIV rates are among the highest across Indian states and where HIV has largely affected FSW. Stable housing is a particular contextual challenge experienced by female sex workers in this region (as well as elsewhere); however, local studies have not examined the impact of this issue on HIV risk. In this paper, we

BACKGROUND: Interventions are needed to improve well-being and promote community reintegration among Veterans with housing insecurity. The objective was to conduct a developmental formative evaluation of a participatory music program.
METHODS: This single-site, pilot study implemented a participatory music program at a U.S. Department of Veterans Affairs (VA) Homeless Domiciliary that included one-hour sessions (group music instruction and ensemble playing), 3 times per week for 3 months. Intervention development was guided by the Model of Human Occupation (MOHO). Evaluation was guided by the

BACKGROUND & AIMS: Social risks are individual-level factors associated with adverse health outcomes. We determined the prevalence and impact of social risks on healthcare use among patients with inflammatory bowel diseases (IBDs) in the United States.
METHODS: In the U.S. National Health Interview Survey 2023, we estimated social risks across 6 domains (food insecurity, financial hardship, housing instability, transportation needs, education and employment, discrimination) in adults with IBD. We evaluated the association between burden of social risk (0 of 6 domains [none], 1 of 6 domains [m…

Social vulnerability is frequently overlooked in emergency departments, where acute medical priorities may overshadow patients' social needs. We evaluated the AP-HP Early Social Frailty Identification Questionnaire in 343 consecutive adult emergency department patients. Fifty patients (15%) met criteria for hospital social worker intervention, compared with only two referrals through routine clinical practice. Housing instability, social isolation, and financial insecurity were the most frequent needs identified. Male sex and higher triage acuity were independently associated with referral. Th…

This study evaluated a cumulative and syndromic relationship among commonly co-occurring vulnerabilites (homelessness, incarceration, low-income, residential transition) in association with HIV-related risk behaviors among 635 low-income women in Baltimore. Analysis included descriptive statistics, logistic regression, latent class analysis and latent class regression. Both methods of assessing multidimensional instability showed significant associations with risk indicators. Risk of multiple partners, sex exchange, and drug use decreased significantly with each additional domain. Higher stabi

BACKGROUND: Housing insecurity is a fundamental social determinant of migrant health. While the relationship between housing and health outcomes is well-documented, there is a paucity of longitudinal data examining this association among humanitarian migrants. Accordingly, we aimed to assess i) longitudinal trends in housing insecurity, psychological distress, post-traumatic stress disorder (PTSD), and self-rated health and ii) the longitudinal associations between housing insecurity and mental and physical health outcomes among humanitarian migrants over the period 2013/14 to 2023.

People living with HIV who are seeking jobs experience unique barriers to obtaining employment at the individual, group, and community levels. Traditional employment assistance programs can provide support but may not be tailored to some people living with HIV who often experience barriers to work related to their social needs (such as housing instability) or their lack of consistent engagement in the workforce. To understand how people living with HIV return to work, in-depth interviews were conducted with 43 participants enrolled in interventions coordinating HIV care with housing and employ…

BACKGROUND: Worries about societal issues such as climate change, pandemics, geopolitical tensions, and economic instability may negatively impact mental health. However, few studies have assessed a broad range of worries within a single study, examining both individual and cumulative associations with mental disorders.
METHODS: In this cross-sectional analysis of wave 2 of NEMESIS-3, a population-based study conducted in 2023-2024 (n = 4,688; age 21-78) worries about climate, COVID-19, international tensions, energy supply/prices, daily living costs, and the housing market were each assessed

OBJECTIVES: Limited evidence describes how geographic context and the built environment relate to non-fatal overdose. We examined demographi … sportation corridors and gas stations and are more commonly observed among individuals experiencing housing instability. Spatial surveillance may inform targeted overdose intervention, outreach, and linkage to treatment.

INTRODUCTION: Whole Health programs aim to integrate mental, physical, and social health to improve veteran well-being. Homeless-experienced veterans may face barriers to engagement owing to housing instability and comorbidities. This study evaluated Whole Health participation among homeless-experienced veterans at the VA Greater Los Angeles Care, Treatment, and Rehabilitation Service site compared with that among the broader VA Greater Los Angeles Healthcare System veteran population.
METHODS: A retrospective quality improvement study analyzed Whole Health participation among 619 homeless-ex…

Transgender persons have high rates of alcohol and other drug use disorders (AUD and DUD, respectively) and commonly experience social and economic stressors that may compound risk for adverse substance-related outcomes. National VA electronic health record data were extracted for all outpatients in each facility with documented alcohol screening 10/1/09-7/31/17. We describe the prevalence of eight individual-level social and economic stressors (barriers to accessing care, economic hardship, housing instability, homelessness, social and family problems, legal problems, military sexual trauma, …

BACKGROUND: Substance use disorders and unmet social needs (like housing instability and social isolation) are major contributors to U.S. morbidity and mortality. We, patients, staff and researchers, established a research advisory committee (RAC) in an addictions treatment setting to guide research and interventions based on lived-experience.
METHODS: We employed community engaged outreach methods to recruit, train, and establish the RAC, and the 10-Step Framework for Continuous Patient-Engagement to advise research activities, and to initiate our own research.
RESULTS: The RAC contributed …

People with intellectual and developmental disabilities (IDD) experience elevated risk for poor health and social outcomes in adulthood and are at risk for experiencing homelessness and housing instability. Although the exact prevalence of IDD among homeless populations is unknown, a small body of literature related to the intersection of IDD and homelessness suggests differential health needs and service use patterns, with a need for targeted health and social services. In this study, we explore the perceptions and experiences of 18 homeless or disability service providers about (a) their cli…

Suicide prevention is a top priority for the US Department of Veterans Affairs (VA), and suicide is often associated with adverse social factors (e.g., financial, legal, and housing problems). The VA provides social services integrated with healthcare services, which may increase the opportunities to detect and document suicide attempt in EHR records. Using VA administrative data, we examined three cohorts of all patients from 2014 to 2018 who had housing instability (n = 659,987), justice involvement (n = 200,487), and unemployment (n = 346,556). Administrative records were used to determine …

BACKGROUND: Given the housing instability and frequent residential relocation (both volitional and hegemonic) of people who inject drugs, we sought to determine whether residential relocation (defined as sleeping in a different place in the past 30 days) is associated with health outcomes in a sample of people who inject drugs (PWID).
METHODS: We recruited 601 PWID using targeted sampling and interviewed them between 2016 and 2018 in San Francisco and Los Angeles, CA about housing, drug use practices, and service utilization. We then developed multivariable regression models to investigate ho…

INTRODUCTION: Compared to their peers, youth who leave the foster care system without permanency experience greater risks for adverse young adult outcomes, including homelessness, incarceration, substance abuse, and early child birth. Extant literature focuses on individual-level factors related to adversity. In this study, we estimated the impact of state and individual-level risk and protective factors on adverse 19-year-old outcomes among a cohort of U.S. transition age youth.
METHODS: We used multilevel modeling to analyze prospective, longitudinal data from two waves of the National Yout
