A 35-year-old female patient was hospitalized for a headache. She was referred to our department for the evaluation of right hydronephrosis noted on the excretory urogram which was performed as part of a hypertensive diagnostic study. Endocrine examination revealed renal hypertension. Excretory urogram and antegrade pyelography showed obstruction of the lower part of the right ureter. A 26-year-old man had asymptomatic macroscopic hematuria. A left hydronephrotic sac outlined with a thin curvilinear shadow of calcium deposition was found in a plain film and stenosis at the ureteropelvic junction (UPJ) in a retrograde pyelogram. Left nephrectomy was performed and the fluid content was 1330 ml. Histologic examination showed subepithelial calcification of the renal pelvis and fibrous proliferation at the UPJ.

This report covers 30 cases of prenatal diagnosis of uni- or bilateral hydronephrosis not associated with an overdistended bladder. Oligohydramnios was observed only in four cases while polyhydramnios occurred in three affected fetuses. In no case was fetal urine aspirated or drained prenatally. Early delivery was performed in four cases with oligohydramnios. A case of hypertension secondary to unilateral hydronephrosis due to congenital obstruction of ureteropelvic junction in a 6-year-old boy is reported. Preoperatively, the plasma renin activity (PRA) in the peripheral vein and in the affected kidney was elevated. Blood pressure decreased significantly following administration of converting enzyme inhibitor. The value of ultrasound and 131-Hippuran renography for diagnosing hydronephrosis during pregnancy was investigated. In a control series of 31 symptom-free pregnant women the ultrasonographically measured renal pelvic diameters in the three trimesters were 5, 10 and 12 mm on the right side and 3, 4 and 5 on the left, respectively. 34 children with congenital hydronephrosis, operated upon for erroneosly diagnosed acute appendicitis (17), malignant tumor (11), and ileus (8), were under observation. To prevent from unwarranted surgical interventions in dubious cases excretory urography and other x-ray and instrumental methods of examination must be used widely. After the removal of an unchanged vermiform process in children, suffering from abdominal pain, the examination of the urinary tract is indicated. Such patients should be under dispensary observation of a children's surgeon.

The first reported urological complication of Dwyer Instrumentation for scoliosis is presented. This patient demonstrated ureteral obstruction secondary to retroperitoneal fibrosis temporally associated with this procedure. Routine IVP following surgery is recommended for early detection of urinary complication.
