The problem of recurrent thyroglossal duct remnants.

The removal of a thyroglossal duct or sinus is a common pediatric surgical operation. It has been taught that if the duct remnant is removed down to and including the middle third of the hyoid bone, the "correct" operation has been done. The three authors have an experience totaling over 60 years in pediatric surgery during which time they operated on 270 thyroglossal duct cysts and sinuses. In this group were 27 recurrences of which three belonged to the authors. Most recurrent thyroglossal duct remnants were found to have the middle third of the hyoid bone still in place, and with its remova

Evaluating the effect of trunk angle on the biomechanics of tongue, hyoid, and swallowing-related muscles during normal oropharyngeal swallowing with a non-invasive sensing system.

Body posture plays an important role in the normal oropharyngeal swallowing process. We aimed to clarify the effects of trunk angle on the biomechanics of the tongue, hyoid, and suprahyoid and infrahyoid muscles during normal oropharyngeal swallowing. For this purpose, fifteen healthy male participants (aged 27.7 ± 1.8 years) were recruited. They swallowed 15 ml of water in three random trunk angles (90°, 60°, 30°), and tongue pressure, hyoid bone movement, and the activity of the suprahyoid and infrahyoid muscles were monitored simultaneously by a non-invasive sensing system. Then, the tempor

Hyolaryngeal Kinematics in Primary Muscle Tension Dysphonia Determined by Ultrasound.

OBJECTIVES: The purpose of this study was to determine hyolaryngeal kinematics during voicing in people with primary muscle tension dysphonia (pMTD) compared with healthy speakers, and to investigate the relationships between hyolaryngeal displacement and self-perceived vocal function. METHODS: Twenty-six participants, 13 with pMTD and 13 healthy speakers, were assessed using sonography during sustained vowel phonation and rest. Displacement of the hyoid bone and thyroid cartilage was measured from still frames extracted from ultrasound video recordings, with measures normalized to reflect ch

The relationship of vertical skeletofacial morphology to oropharyngeal airway shape using cone beam computed tomography: possible implications for airway restriction.

OBJECTIVE: To determine if the shape of the oropharyngeal airway is related to the vertical morphology of the skeletofacial complex, including the hyoid bone. MATERIALS AND METHODS: Cone beam computed tomography scans from 50 pretreatment adult orthodontic records were used to obtain skeletal and airway measurements. Linear regression statistics were used to compare soft tissue variables to hard tissue predictor variables. RESULTS: Transverse airway widening was significantly increased when the distance between the hyoid and vertebrae was reduced; when the three-dimensional (3D) facial axis

Case Report: Synchronous lingual osseous choristoma and suprahyoid thyroglossal duct cyst: insights into the embryological thyroglossal-hyoid axis.

Lingual osseous choristoma is an exceedingly rare benign lesion characterized by the presence of mature bone tissue within the soft tissue of the tongue, where osseous structures are not normally found. Thyroglossal duct cysts (TGDCs) are common congenital midline neck lesions resulting from incomplete involution of the thyroglossal duct during embryogenesis. Both entities share a common embryological pathway extending from the tongue base through the hyoid bone to the lower cervical region. However, the synchronous occurrence of a lingual osseous choristoma and a suprahyoid TGDC in a single p

Comparison of upper esophageal sphincter opening in healthy asymptomatic young and elderly volunteers.

Deglutitive upper esophageal sphincter opening (UES) in the elderly has been incompletely studied. Our aim was to determine in the elderly the temporal and dimensional characteristics of deglutitive UES opening; anterior and superior hyoid and laryngeal excursions as measures of distracting forces imparted on the UES; and hypopharyngeal intrabolus pressure (IBP). Fourteen healthy elderly and 14 healthy young volunteers were studied by concurrent videofluoroscopy and hypopharyngeal manometry during swallowing of 5- and 10-mL barium boluses. The anteroposterior UES diameter, as well as the anter

Transoral laser-assisted microscopic excision for lingual thyroglossal duct cyst.

PURPOSE: Thyroglossal duct cyst (TGDC) management, particularly lingual variants, poses challenges in treatment due to its anatomical location and potential for recurrence. This study aims to introduce a novel technique for managing lingual TGDC, combining transoral laser microscopic excision with cauterization of the hyoid mid portion, to improve treatment efficacy and reduce recurrence rates. METHODS: The effectiveness of this approach was evaluated in 11 patients (age range: 2-16 years), including three cases with recurrent TGDC following simple transoral excision at other institutions. Expansion hyoidplasty. 1983 First-Place Resident Research Award: clinical category.

An inspiratory, circumferential, passive collapse of the hypopharyngeal lumen is the mechanism of airway obstruction in some patients with "idiopathic" obstructive sleep apnea syndrome. While permanent tracheotomy has resolved the obstruction and reversed the associated cardiopulmonary sequelae, it is not without complications. The expansion hyoidplasty was conceived as an alternative. The hyoid bone is trisected just medial to each lesser cornu, then held in an expanded position by a permanent brace. The greater cornua with attached middle constrictor and hyoglossus are moved laterally, while

How a personalised transportable folding device for seating impacts dysphagia.

PURPOSE: A personalised transportable folding device for seating (DATP) on a standard seat was developed by an occupational therapist at the Toulouse University Hospital Centre (patent no. WO 2011121249 A1) based on the hypothesis that the use of a seat to assist with better positioning on any chair during meals modifies the sitting posture and has an impact on cervical statics which increases the amplitude of movements of the axial skeleton (larynx and hyoid bone) and benefits swallowing. The aim of this work is to demonstrate that an improvement in sitting posture with the help of the DATP,

Computed tomographic signs of hyoid apparatus disease in 165 horses.

INTRODUCTION: Horses with hyoid bone disease may present with clinical symptoms such as exercise intolerance, resistance to ridden work, anorexia, or headshaking. This study aims to describe the computed tomographic (CT) signs and prevalence of hyoid disease and to evaluate their association with other CT changes of the head. METHODS: In this study, four investigators conducted a retrospective analysis of their findings after reviewing the medical records of 200 horses that had undergone CT scans between 2009 and 2019. A total of 32 parameters were analyzed, and associations were evaluated be

Effect of posture on deglutitive biomechanics in healthy individuals.

The purpose of this study was to assess displacement of oropharyngeal structures, particularly the hyoid bone and velum, and variations in timing of the pharyngeal stage of swallowing in the upright versus the supine position. Twelve Caucasian adult subjects between 19 and 27 years of age participated. Subjects were recorded swallowing 7 cc of liquid barium in the upright and supine positions. The hyoid bone had a significantly greater amount of anterior displacement while in the supine position compared to that of the upright position (p < 0.01). While in the upright position, the velum comes

Useful Genioplasty for Repeated Recurrent Sleep Apnea of Congenital Anomalies and Its Evaluation.

UNLABELLED: Congenital facial anomalies with hypoplasia of the midface or lower face are associated with obstructive apnea syndrome. Although such patients underwent bone advancement surgery and their sleep apnea improved in the short term, it often recurred several years after surgery. It is difficult to perform another major osteotomy because of impairment of the facial contour or prior orthodontic treatment. Genioplasty was performed for genioglossus muscle advancement in patients with congenital anomalies and repeated sleep apnea. In this study, we evaluated the usefulness of this procedur

Metastatic neck disease beyond the limits of a neck dissection: attention to the 'para-hyoid' area in T1/2 oral tongue cancer.

OBJECTIVE: We evaluated patients with small oral tongue cancer suffering from recurrence, which develops in the intervening area between the primary site and the neck. Lesions in the area around the cornu of the hyoid bone ('para-hyoid' area) often involve the hypoglossal nerve and the root of the lingual artery, resulting in treatment failure and death. METHODS: A 10-year retrospective chart review was conducted of 248 oral tongue cancer patients with small primary tumors (T1/2). No patients who underwent postoperative radiotherapy (PORT) were included. RESULTS: After excluding those who ha
