An Imitation Too Perfect: Isolated Neurosarcoidosis Mimicking Spheno-Orbital Meningioma With Hyperostosis.

We report a rare case of isolated neurosarcoidosis presenting as a spheno-orbital mass with hyperostosis of the sphenoid wing, initially misdiagnosed as a meningioma. Neurosarcoidosis may manifest as dural-based lesions that mimic tumors radiologically; however, hyperostosis, a hallmark of meningiomas, is usually absent. This unusual presentation causes diagnostic confusion. This case highlights the diagnostic challenges when sarcoidosis mimics meningiomas, particularly in the presence of hyperostosis. Awareness of such atypical presentations is crucial for accurate diagnosis and appropriate m…

A Multicenter Allelic Analysis of Diffuse Idiopathic Skeletal Hyperostosis: Nature Versus Nurture?

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) is an incompletely defined disease process with no known unifying pathophysiological mechanism. OBJECTIVE: To our knowledge, no genetic studies have been performed in a North American population. To summarize genetic findings from previous studies and to comprehensively test for these associations in a novel and diverse, multi-institutional population. METHODS: Cross-sectional, single nucleotide polymorphism (SNP) analysis was performed in 55 of 121 enrolled patients with DISH. Baseline demographic data were available on 100 patient…

Diffuse Idiopathic Skeletal Hyperostosis Causing Progressive Dysphagia: A Case Report and Review.

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) is a rare noninflammatory disorder impacting spinal longitudinal ligament and enthesis. The majority of DISH cases are asymptomatic or have few manifestations. Manifestations include neck pain and stiffness, stridor, breathing disturbances, and dysphagia. Case Presentation. A mid-aged man with progressive dysphagia to solid food was admitted to Loghman Hakim Hospital. In cervical X-ray, a huge ossification in the anterior longitudinal ligament was evident. Eventually, he was diagnosed with DISH. Because of coronary artery disease, con…

Hypertrophic Pachymeningitis Related Brain Infarction in Synovitis-Acne-Pustulosis-Hyperostosis Osteomyelitis Syndrome.

A 50-year-old woman with a history of synovitis-acne-pustulosis-hyperostosis osteomyelitis (SAPHO) syndrome was admitted for left unilateral neglect, dysarthria, and left hemiparesis. Brain MRI showed multiple infarctions in the territory of the right middle cerebral artery and gadolinium enhancement of the thickened frontotemporal dura mater on the right side. MR angiography showed significant narrowing of the cavernous segment of the right internal carotid artery. The right internal carotid artery stenosis was thought to originate from hypertrophic pachymeningitis associated with SAPHO syndr…

DIFFUSE IDIOPATHIC SKELETAL HYPEROSTOSIS IN CAPTIVE GORILLAS (GORILLA SPP.): APPEARANCES AND DIAGNOSIS.

Diffuse idiopathic skeletal hyperostosis (DISH) is a disorder of unknown cause, in which new bone forms in soft tissues attached to the skeleton. Originally described in humans, in whom it is quite common, it is usually asymptomatic. New bone may completely bridge across joints, especially in the spine. However, it can be difficult to distinguish from diseases such as spondyloarthritis and spondylosis. With safer and increased use of radiography in diagnosis, the unfamiliar skeletal changes of asymptomatic DISH may now be coincidentally revealed during investigation of other disorders and resu…

Frontal Sinus Osteoma Accompanied by Intracranial Mucocele and Local Hyperostosis Frontalis Interna.

Frontal sinus osteoma accompanied by intracranial mucocele and local hyperostosis frontalis interna has never been reported. A 47-year-old woman presented with a 3-month history of intermittent headache. Physical examination revealed no neurologic abnormality. Contrasted magnetic resonance imaging showed a frontal heterogeneously enhanced lesion with adjacent nonenhanced cyst. Computed tomography showed a bone density mass, which was accompanied by local hyperostosis frontalis interna, which filled the left frontal sinus and extended intracranially. The patient underwent a left frontobasal cra…

Diffuse idiopathic skeletal hyperostosis is associated with lumbar spinal stenosis requiring surgery.

Factors related to the onset and progression of lumbar spinal stenosis (LSS) have not yet been identified. Diffuse idiopathic skeletal hyperostosis (DISH) increases mechanical loading on the non-fused lumbar levels and may therefore lead to LSS. This cross-sectional study aimed to identify associations between LSS and DISH. This study included 2363 consecutive patients undergoing surgery for LSS and 787 general inhabitants without symptoms of LSS as participants of the population-based cohort study, Research on Osteoarthritis/Osteoporosis Against Disability. Standing whole-spine radiographs we…

Bilateral vocal fold paralysis and dysphagia secondary to diffuse idiopathic skeletal hyperostosis.

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) is an idiopathic spinal disease common in the elderly and characterized by flowing ossification and osteophyte formation along the spinal column. Cervical hyperostosis is capable of producing dysphagia, stridor, and airway obstruction; however, there are no extant reports of true paralysis of bilateral vocal folds in patients fulfilling the criteria for DISH. METHODS AND RESULTS: We report a case of a 61-year-old man presenting with dysphagia and dyspnea. Flexible laryngoscopy revealed bilateral true vocal fold paralysis. Cervical ra…

Dorsal vertebral column abnormalities in dogs with disseminated idiopathic skeletal hyperostosis (DISH).

Although disseminated idiopathic skeletal hyperostosis (DISH) most often affects the ventral aspect of the vertebral column, this study evaluated the occurrence, nature and clinical relevance of dorsal vertebral column abnormalities in 10 dogs with DISH for which CT or MRI and a complete neurological examination were available. Dorsal vertebral column abnormalities were present in eight dogs and included articular process hypertrophy (n=7 dogs), periarticular new bone formation (n=1), pseudoarthrosis between spinous processes (n=4) and thickening of the dorsal lamina (n=4). These dorsal verteb…

Bone metabolic activity in hyperostosis cranialis interna measured with 18F-fluoride PET.

PURPOSE: (18)F-Fluoride PET/CT is a relatively undervalued diagnostic test to measure bone metabolism in bone diseases. Hyperostosis cranialis interna (HCI) is a (hereditary) bone disease characterised by endosteal hyperostosis and osteosclerosis of the skull and the skull base. Bone overgrowth causes entrapment and dysfunction of several cranial nerves. The aim of this study is to compare standardised uptake values (SUVs) at different sites in order to quantify bone metabolism in the affected anatomical regions in HCI patients. METHODS: Nine affected family members, seven non-affected family…

Cervical myelopathy caused by soft-tissue mass in diffuse idiopathic skeletal hyperostosis.

A rare case of cervical spinal cord compression in diffuse idiopathic skeletal hyperostosis (DISH or Forestier's Disease) caused by a craniocervical mass of soft-tissue is reported. The objective is to describe an uncommon mechanism of spinal cord compression in DISH. Three weeks after a cardiac infarction a 69-year-old man slowly developed spastic tetraparesis. Magnetic resonance tomography showed a craniocervical tumor compressing the spinal cord and a massive DISH of the cervical spine. An extended mass of yellowish amorphous material was removed from between the dura, the posterior odontoi…

[Dysphagia caused by neurogenic deglutition disorders and diffuse idiopathic skeletal hyperostosis (DISH)].

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) may lead to dysphagia caused by osteophytes of the cervical spine. Osteophytes can be resected transorally or transcervically, but operative ablation should not be indicated generously because of the threat of severe complications. PATIENT: A fifty-year-old man with dysphagia and loss of weight of 15 kg in the last three months is presented. He also suffered from a brain damage during infancy which caused grand-mal-seizures. One seizure lead to cardiac arrest which required cardio-pulmonary resuscitation and subsequent tracheostomy. …

[Infantile cortical hyperostosis (Caffey-Silverman syndrome). Histologic, histochemical and electron microscopic studies].

Infantile cortical hyperostosis, also known as Caffey's disease or Caffey-Silverman syndrome, is an uncommon clinico-pathological lesion of unknown etiology and uncertain histogenesis. One of the most striking features is the early age of patients at the onset of the disease, showing swelling of the soft tissues overlaying bones, hyperirritability, and, subsequently, periosteal new bone production. The natural history of the disease proves to be self-limiting. Multiple areas are involved in the majority of cases. These polyostic forms are easily clinically diagnosed. But in rare monostic prese…

Skeletal hyperostosis and extraosseous calcification in patients receiving long-term etretinate (Tigason).

In an ongoing study of patients on long-term etretinate (Tigason) therapy, 13 patients with a congenital or inherited disorder of keratinization and 10 patients with psoriasis were examined to investigate the incidence of, and the factors associated with, skeletal hyperostosis. Skeletal scintigraphy, plain radiographs, haematological and biochemical analyses were performed. Using all criteria, 7 of 13 patients with a congenital or inherited disorder of keratinization showed evidence of hyperostosis. No single investigation was able to detect all these cases; in particular, skeletal scintigraph…

Assessment of muscle quality and quantity in individuals with diffuse idiopathic skeletal hyperostosis.

OBJECTIVES: Diffuse idiopathic skeletal hyperostosis (DISH) may reduce spinal mobility and affect muscle quantity and quality, increasing sarcopenia risk. However, longitudinal data are limited. We investigated muscle quantity, quality, and their changes in DISH using computed tomography (CT) and bioelectrical impedance analysis (BIA). METHODS: We analysed health screening data, including data of participants who underwent CT and BIA twice over 5 years. We identified 143 DISH patients (58.0 ± 8.4 years, 19 females) and 143 age- and sex-matched controls (58.4 ± 8.8 years, 19 females). At L3, p…

Massive Stylohyoid Hyperostosis With Fusion to the Hyoid: An Extreme Variant of Eagle Syndrome.

Eagle syndrome is classically defined by symptomatic elongation of the styloid process or calcification of the stylohyoid ligament. While elongated styloid processes are relatively common radiographic findings, extreme bony hypertrophy with direct involvement of the hyoid bone is exceedingly rare. We report the case of a 69-year-old male with progressive unilateral odynophagia and neck pain found to have massive hyperostosis of the left styloid process, measuring approximately 9 cm in craniocaudal length, with abutment, erosion, and fibrous union to the lateral body and greater horn of the hyo…

An Investigation of Hyperostosis Frontalis Interna in a Modern Anatomical Body Donor Population.

This research sought to examine the prevalence and severity of hyperostosis frontalis interna (HFI) in the Chicagoland anatomical body donor population. The study further aimed to elucidate potential demographic risk factors for HFI, including sex, age at death, and structural vulnerability index (SVI), as well as any common comorbidities, as gleaned from death certificates. HFI is an irregular bony overgrowth of the endocranial surface of the frontal bone. It is most often observed in postmenopausal women or in individuals with growth hormone disorders. This work investigated the distribution…

A rare presentation of diffuse idiopathic skeletal hyperostosis with progressive dysphagia and choking.

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) is a rheumatological disorder characterized by the formation of spinal osteophytes due to the ossification of paravertebral ligaments and muscles, particularly involving the anterior longitudinal ligament. The disease typically manifests after the fifth decade of life and is most commonly seen in the thoracic spine. Here, we present the case of an 85-year-old female with cervical DISH who experienced dysphagia, choking, and even airway obstruction. Following anterior resection of the osteophytes, her symptoms largely resolved. CASE D…
