A 62-year-old woman without evidence of immunocompromise was evaluated for uveitis and a subretinal lesion in the right eye. Laboratory evaluation, including cerebrospinal fluid analysis, revealed no apparent cause. The diagnosis of subretinal cryptococcosis was established by transscleral needle biopsy of the subretinal mass. Treatment with intravenous amphotericin B and oral 5-flucytosine brought recovery of visual acuity to 20/30-1 and resolution of the inflammation. Forty nine patients (19 adults and 30 children) with oocysts of Cryptosporidium in their faeces had repeated stool specimens taken until oocysts could no longer be identified. They were found in the stools up to 35 days after the onset of symptoms in one patient, but most had stopped shedding them by 20 days. In 25 of the 49 patients in whom symptoms could be compared with the shedding of oocysts, 19 (76%) had symptoms corresponding to the shedding period while symptoms persisted in four (16%) after shedding had stopped.

A study was made of the Harderian and lachrymal glands of fowls, pheasants, turkeys and pigeons. Accumulation of heterophils and plasma cells commenced soon after hatching, was well advanced by four weeks of age and was always greater in the Harderian than in the lachrymal glands. In patients exposed to inhaled noxious substances, nocardia should also be considered in cases of unclear pulmonary infections. Disseminated nocardiosis is often difficult to diagnose. The risk of complications is increased due to the delay in diagnosis and the long and complicated antibiotic therapy. In the case report presented here, the connection between occupationally induced pulmonary predisposition and suspected inhalation of dust must be emphasized as particularly important. Interdisciplinary diagnostics and subsequent infectious disease-supported prolonged therapy are necessary.
