Implementation of optimized kangaroo care for infection prevention and control in neonatal intensive care units (NeoIPC): Protocol for the implementation elements of a multicenter parallel cluster randomized hybrid type 2 implementation-effectiveness study.

BACKGROUND: Hospital-acquired neonatal severe infections and resistant bacterial colonization in neonatal care are a world-wide challenge. Preventing the spread of resistant bacteria in neonatal intensive care units (NICUs) is specifically challenging, for example, due to multi-patient rooms, spaces being crowded with equipment, and high antibiotic use in these settings. Kangaroo care (KC), a practice that involves skin-to-skin contact between newborn infants and caregivers, is a promising, low-cost intervention that has been associated with reduced morbidity and mortality in low-birthweight i

Contributions of Qualitative Methods to Real-Time Implementation Strategy Design: A Case Study Linking Justice Involved Individuals at Risk for HIV to Pre-exposure Prophylaxis.

Pre-exposure prophylaxis (PrEP) exemplifies a medical advance that is highly efficacious in reducing an individual's risk of acquiring HIV. However, the use of PrEP remains considerably suboptimal in communities that could benefit from it most, especially in the southern United States where HIV infection rates are highest. We conducted a study using both qualitative methods and implementation science to develop and deploy a multicomponent implementation strategy to link individuals being released from jail to PrEP services in a southern US city. Implementation science supports the translation …

Evaluating the tailored implementation of a multisite care navigation service for mental health in rural and remote Australia (The Bridging Study): protocol for a community-engaged hybrid effectiveness-implementation study.

BACKGROUND: A dramatic decline in mental health of people worldwide in the early COVID-19 pandemic years has not recovered. In rural and remote Australia, access to appropriate and timely mental health services has been identified as a major barrier to people seeking help for mental ill-health. From 2020 to 2021 a care navigation model, Navicare, was co-designed with rural and remote communities in the Greater Whitsunday Region of Central Queensland in Australia. The Exploration, Preparation, Implementation and Sustainment (EPIS) framework was used to design and guide multiple aspects of a mul

Adapting and testing measures of organizational context in primary care clinics in KwaZulu-Natal, South Africa.

BACKGROUND: Implementation science frameworks situate intervention implementation and sustainment within the context of the implementing organization and system. Aspects of organizational context such as leadership have been defined and measured largely within US health care settings characterized by decentralization and individual autonomy. The relevance of these constructs in other settings may be limited by differences like collectivist orientation, resource constraints, and hierarchical power structures. We aimed to adapt measures of organizational context in South African primary care cli…

Household Disinfection Interventions to Prevent Cholera Transmission: Facilitators, Barriers, Training, and Evidence Needs.

There are two common household disinfection interventions to prevent interhousehold transmission of cholera: household spraying, whereby a team disinfects cholera patients' households, and household disinfection kits (HDKs), whereby cleaning materials are provided to cholera patients' family members. Currently, both interventions lack evidence, and international agencies recommend HDK distribution; however, household spraying remains widely implemented. To understand this disconnect, we conducted 14 key informant interviews with international and national responders and a study in Haiti assess

The influence of formal and informal policies and practices on health care innovation implementation: A mixed-methods analysis.

BACKGROUND: The implementation science literature has contributed important insights regarding the influence of formal policies and practices on health care innovation implementation, whereas informal implementation policies and practices have garnered little attention. The broader literature suggests that informal implementation policies and practices could also influence innovation use. PURPOSE: We used the Organizational Theory of Innovation Implementation to further understand the role of formal and informal implementation policies and practices as determinants of implementation effective…

Ready to screen implementation trial protocol: a cluster randomized implementation trial comparing a bundled implementation strategy versus a simple implementation strategy to improve intention to screen in the Australian National Lung Cancer Screening Program.

BACKGROUND: Uptake of lung cancer screening (LCS) in high-risk populations remains suboptimal internationally. Primary care practitioners play a critical role in identifying eligible patients and initiating referrals for LCS. Targeted implementation strategies are needed to address health care barriers to the uptake of LCS such as limited awareness, eligibility assessment, low engagement, and poor health system preparedness. Implementation trials are needed to determine optimal decision-making and participant knowledge gains to ultimately increase screening uptake. DESIGN: The Ready to Screen

Translational and Implementation Barrier in the Management of Biliary Tract Cancer: Diagnostic, Molecular, Therapeutic, and Patient-Centered Challenges.

Biliary tract cancer (BTC) is a rare but highly lethal form of cancer and is frequently diagnosed at a late stage, thus limiting treatment options and prognosis. Although there have been great strides in the field of oncology, there are still a number of obstacles that stand in the way of improving patient care. Unfortunately, early diagnosis is still challenging due to the absence of sensitive biomarkers and effective screening strategies. Limited therapeutic options are also available, in which a small number of patients with molecular changes (IDH1/2, FGFR2, EGFR, HER2) benefit from FDA-app

Mixed-Methods Evaluation to Identify Factors Influencing High-Value Surgeon Decisions in Orthopedics: An Example in Anterior Cruciate Ligament Reconstruction.

RATIONALE: Many advances have been made to identify novel, effective orthopedic care practices. For all that is known about the comparative effectiveness of various clinical decisions in anterior cruciate ligament (ACL) injury care, surprisingly little is known about how surgeons approach decision-making in the face of this evidence. Implementation science models such as Capability, Opportunity, Motivation - Behavior (COM-B) offer a way to organize and comprehensively understand how clinicians make decisions. AIMS AND OBJECTIVES: This evaluation aimed to understand and quantify the factors th…

Implementation research: including breast examinations in a cervical cancer screening programme, Rwanda.

OBJECTIVE: To evaluate whether integrating breast and cervical cancer screening in Rwanda's Women's Cancer Early Detection Program led to early breast cancer diagnoses in asymptomatic women. METHODS: Launched in three districts in 2018-2019, the early detection programme offered clinical breast examination screening for all women receiving cervical cancer screening, and diagnostic breast examination for women with breast cancer symptoms. Women with abnormal breast examinations were referred to district hospitals and then to referral hospitals if needed. We examined how often clinics were held

A Phased Approach: The General Surgery Experience Adopting Entrustable Professional Activities in the United States.

Entrustable professional activities (EPAs) have been increasingly used as an assessment framework to formally capture the myriad ad hoc entrustment decisions that occur on a daily basis in clinical settings with learners present. Following the definition of Core EPAs for Entering Residency by the Association of American Medical Colleges in 2014, the American Board of Surgery (ABS) began to explore the utility of EPAs as a framework to support competency-based resident education within general surgery in 2016. As the complement of EPAs drafted for a specialty serve to define the core tasks of a

Implementing group visits for opioid use disorder: A case series.

Background: Group-based models of Office-Based Opioid Treatment with buprenorphine-naloxone (B/N) are increasingly being implemented in clinical practice to increase access to care and provide additional therapeutic benefits. While previous studies reported these Group-Based Opioid Treatment (GBOT) models are feasible for providers and acceptable to patients, there has been no literature to help providers with the more practical aspects of how to create and maintain GBOT in different outpatient settings. Case series: We present 4 cases of GBOT implementation across a large academic health care

Optimization of care and recovery in older colorectal cancer surgery patients: a hybrid effectiveness-implementation pilot study protocol.

BACKGROUND: The exponential rise in colorectal cancer surgeries among older adults combined with 30-day readmission rates as high as 25% highlights the crucial need to optimize post-discharge care. While geriatrics surgery programs such as the Optimization of Senior Care and Recovery (OSCAR) model improve outcomes, they do not address the post-discharge transition. This study will address this gap by systematically integrating the OSCAR program with an evidence-based Care Transitions Intervention (CTI), using implementation science frameworks to guide adaptation. The resulting integrated model…

Improving science will require more than just training.

Van Til et al. (2025) makes many implicit and explicit assumptions about those goals, practices, and implementation strategies that require empirical evidence, or which are conditioned or unsubstantiated by existing evidence. Open science has a set of goals that should be carefully defined and measured. Open science practices (e.g., study registrations before data collection or secondary analyses, writing registered reports, data sharing) are interventions designed to achieve those goals. Graduate coursework in open science is an example of an implementation strategy, a method used to promote
