Rapid deterioration of primary fourth ventricular outlet obstruction resulting in syndrome of inappropriate antidiuretic hormone secretion.

Fourth ventricular outlet obstruction (FVOO) is a rare cause of obstructive hydrocephalus. Although FVOO accompanied by malformative syndrome and secondary causes of obstruction are common, there are few reports of primary FVOO (PFVOO). The syndrome of inappropriate antidiuretic hormone secretion (SIADH) is a rare presenting feature of hydrocephalus. A 1-year, 8-month-old boy with a normal head circumference developed SIADH accompanied by rapid deterioration of symptoms of intracranial hypertension. PFVOO was diagnosed because magnetic resonance imaging revealed an enlarged ventricular system

Syndrome of inappropriate secretion of antidiuretic hormone associated with angiotensin-converting enzyme inhibitor administration.

Angiotensin-converting enzyme inhibitors (ACEI's) are an important medication in the treatment of congestive heart failure. However, ACEIs may cause harmful side effects, such as the syndrome of inappropriate secretion of antidiuretic hormone (SIADH), which is a rare but important side effect. We describe here a case of SIADH associated with ACEI administration in a 6-year-old boy with restrictive cardiomyopathy. After recovery from acute exacerbation of congestive heart failure by tolvaptan administration, an ACEI (cilazapril) was started to decrease the production of angiotensin II, which up

Syndrome of inappropriate antidiuretic hormone in association with amiodarone therapy: a case report and review of literature.

BACKGROUND: Amiodarone is a class III antiarrhythmic agent that is widely used in the treatment of a variety of arrhythmias. Several different systemic side effects are reported after use of this medication. In this article, we report a case that had developed syndrome of inappropriate antidiuretic hormone (SIADH) after starting treatment with this agent. CASE REPORT: The patient is a 66-year-old male with past medical history of hypertension, hyperlipidemia, coronary artery disease, and class III New York Heart Association congestive heart failure who presented with monomorphic nonsustained

[A case of mild pulmonary tuberculosis complicated with the syndrome of inappropriate antidiuretic hormone secretion which caused impaired consciousness].

An 81-year-old woman was admitted to our hospital for hyponatremia and impaired consciousness after unsuccessful antibiotic treatment for pneumonia-like symptoms by a previous doctor. A chest X-ray film revealed unilateral infiltration. Mycobacterium tuberculosis was detected on a sputum smear and pulmonary tuberculosis was diagnosed. Based on the diagnostic criteria, we believed that her hyponatremia a consequence of syndrome of inappropriate antidiuretic hormone secretion (SIADH) as a complication of pulmonary tuberculosis. Sodium loading and water restriction quickly improved her serum sodi

Nephrogenic syndrome of inappropriate antidiuresis.

The nephrogenic syndrome of inappropriate antidiuresis (NSIAD) is a rare, recently recognized disorder in water balance affecting not only infants but also adults. A new molecular mechanism responsible for NSIAD has recently been identified: a gain of function of the arginine vasopressin (AVP) receptor type 2 (V2R), causing the constitutive activation of the receptor. Clinical presentation and laboratory findings of NSIAD resemble those of the syndrome of inappropriate secretion of antidiuretic hormone and consist of hyponatraemia, seizures and the lack of urinary dilution; however, the AVP le

Hyponatremia during therapy with amiodarone.

We describe an elderly woman who presented with significant hyponatremia caused by the syndrome of inappropriate secretion of antidiuretic hormone (SIADH) during therapy with amiodarone. The hyponatremia resolved after discontinuation of the drug and did not recur during a follow-up period of 6 months. Such an association has previously been reported only once in the medical literature. Since severe hyponatremia can produce significant neurologic damage and is associated with a high mortality rate, clinicians should be aware that this complication may occur during amiodarone therapy.

[Polydipsia: review of the literature].

Polydipsia, unrelated to organic polyuria is present in 20% of the patients hospitalised in psychiatry on a long term basis. It seems that about a third of them have presented at some point an episode of water intoxication. Most patients with psychogenic polydipsia excrete all the water they absorb. In a subgroup of them, at risk for water intoxication, the capacity to excrete water is deficient. This is usually due to an inappropriate secretion of antidiuretic hormone. The first two steps in the treatment of patients with psychogenic polydipsia consist in making the diagnosis and in evaluatin

[Acute intermittent porphyria associated with hyperaldosteronism and inappropriate antidiuretic hormone secretion syndrome].

Acute intermittent porphyria, the most commun acute porphyria in France, is an autosomal dominant disorder of heme biosynthesis. The basic biochemical defect is reduced activity of the enzyme porphobilinogen deaminase. Clinical evolution is characterized by acute attacks, with a severe prognosis due to acute abdominal pain and risk of neurological complications, induced by drug intake, infection, alcohol intake or unknown factors. We report the case of a patient with an inappropriate antidiuretic secretion syndrome and secondary hyperaldosteronism associated with acute intermittent porphyria a

[Congenital multiple anomaly syndrome with recurrent vomiting accompanied with latent SIADH; a case report].

A 13-year-old girl with multiple minor anomalies and severe mental retardation had recurrent episodes of severe vomiting. At each episode, marked elevations of plasma ADH, ACTH, cortisol and salivary type amylase were found with reduction of serum Na level and osmolarity. This case is similar to that with periodic ACTH-ADH discharge syndrome (Sato). However, she had underlying disease, and neither hypertension nor depressive state was observed. Latent SIADH was detected by water loading test. After DZP administration, ADH secretion was suppressed in this test, and actually the duration of each

[Valproate related syndrome of inappropriate secretion of antidiuretic hormone (SIADH)--a case report].

We report an 82-year-old retired man who developed syndrome of inappropriate secretion of ADH (SIADH) caused by sodium valproate (VPA). He had been taking VPA for treatment of symptomatic epilepsy due to cardioembolic stroke. Although he was clinically asymptomatic, he was found to have decreased level of serum sodium concentration (128 mEq/l). Association of hyponatremia, normal urinary sodium concentration, high urine osmolality and increased concentration of serum ADH strongly suggested the presence of SIADH. There were no underlying disorders which can cause SIADH, such as malignant neopla

Inappropriate antidiuretic hormone secretion after cataract extraction under local anesthesia.

We report a case of a patient who underwent bilateral cataract extractions under local anesthesia. In both operations, the patient developed signs, symptoms, and laboratory data consistent with the syndrome of inappropriate secretion of antidiuretic hormone. In this era of outpatient cataract surgery, we believe this diagnosis should be entertained postoperatively in the proper clinical setting.

[Electrolyte abnormalities associated with cancer: a review].

There are a variety of water and electrolyte disorders in patients with cancer. These disorders occur during the growth of tumors, generally as a consequence of inadequate intake and absorption of electrolytes, renal failure secondary to tumor or rapid tumor destruction and production of metabolically active substances by the tumor. In this paper, the electrolyte abnormalities associated with cancer were reviewed. Hyponatremia is one of the most common clinical electrolyte abnormalities in advanced cancer. Some patients may have hyponatremia, in spite of increased total body sodium and absence
