Managing Gout in the Patient with Renal Impairment.

Hyperuricaemia is an independent risk factor for renal function decline. Evidence is emerging that urate-lowering therapy might be beneficial in subjects with renal impairment. We  review the association between renal impairment and gout, some of the related pathogenic processes and the possible impact of gout treatment on the progression of renal impairment. Nevertheless, the management of gout is more complex in the presence of chronic kidney disease. The main aim of gout therapy is to fully dissolve the urate crystals, thus curing the disease. Avoidance of attacks-prophylaxis-and their prom

Psychometric properties of the Liebowitz Social Anxiety Scale (LSAS) in a longitudinal study of African Americans with anxiety disorders.

The Liebowitz Social Anxiety Scale (LSAS) is a widely used measure of social anxiety. However, no study has examined the psychometric properties of the LSAS in an African American sample. The current study examined the LSAS characteristics in 97 African Americans diagnosed with an anxiety disorder. Overall, the original LSAS subscales showed excellent internal consistency and temporal stability. Similar to previous reports, fear and avoidance subscales were so highly correlated that they yielded redundant information. Confirmatory factor analyses for three previously proposed models failed to

[Exercise therapy in the treatment of chronic back pain. An integral part of interdisciplinary therapy].

Chronic low back pain requires interdisciplinary and biopsychosocial treatment. Apart from the medical, psychological, and physiotherapeutic aspects of therapy, exercise therapy is an important component. Together with"work hardening," it represents the elements for reconditioning. The isolated effect is scientifically difficult to specify with the available data, but in most analytical studies, exercise therapy is an important component in an interdisciplinary setting. A nonspecific, diversified training program is superior to exercise solely of the trunk muscles. The primary tasks are to rec

Threat processing in women with bulimia.

Cognitive-behavioral treatments are the most commonly used approaches for bulimia nervosa. However, these treatments tend to produce only moderate remission rates. Therefore, it is useful to consider how experimental cognitive approaches can help to revise current cognitive-behavioral models of bulimia. Information processing models highlight the role of attentional biases and schema avoidance in the etiology and maintenance of pathology. This review considers how these processes relate to bulimia. In particular, a growing body of evidence suggests that bulimic behaviors may function to "block

Gout: can management be improved?

Ongoing reviews of Cochrane collaboration show that there is still very little reliable information based on randomized controlled trials on which to base treatment decisions in acute and chronic gout. Recent studies have stressed that avoidance of factors contributing to development of gouty attacks such as diuretic therapy, weight gain, and alcohol consumption may lead to a decrease in gouty arthritis. Attention to minidose aspirin and its effect on serum uric acid levels was addressed. A low carbohydrate, high protein and unsaturated fat diet was recommended for gouty patients since they al

Sun awareness in school teachers.

School children are important targets for sun awareness education, but the knowledge, attitudes and behaviour of school teachers with respect to sun awareness are poorly understood. A questionnaire-based survey of 76 school teachers was undertaken. Twenty-four per cent of teachers indicated previous experience in teaching sun awareness, but 93% of teachers had no classroom resources for teaching sun awareness. Sun awareness was perceived by respondents as unimportant relative to other health education issues. There was considerable scope for improvement in attitudes and behaviour of teachers w

Posttraumatic stress disorder after a bus accident.

There is insufficient information on the prevalence of posttraumatic stress disorder (PTSD) and other psychological reactions caused by surviving serious road accidents. This paper presents the assessments of 29 people who narrowly escaped death in a coach accident that killed eleven people. Their high vulnerability is clearly established, with 41% having PTSD, and 52% severe intrusion or avoidance phenomena, which included 31% who had both. The occurrence of psychological sequelae was associated with being currently distressed by another event, but not with the survivors' age, gender or acqua

Information Processing and Decision-Making in Pathological Worriers and their Potential Role in Mechanisms of Generalized Anxiety Disorder.

Systematic information processing and decision-making under uncertainty are key constructs of new conceptions explaining the severity of pathological worry. The current study attempted to analyze their usefulness in subclinical and clinical groups. In the first phase of the study (N = 251) participants were examined with the Penn State Worry Questionnaire (PSWQ), a GP consultationrelated survey, and a screening survey for generalized anxiety disorder (GAD). In the second phase (N = 220), the State-Trait Anxiety Inventory, the PSWQ, and tasks measuring systematic information processing (SIP) ve

Inhibition in developmental disorders: A comparison of inhibition profiles between children with autism spectrum disorder, attention-deficit/hyperactivity disorder, and comorbid symptom presentation.

Many children with autism spectrum disorder (ASD) also have symptoms of attention-deficit hyperactivity disorder (ADHD). Children with ASD and ADHD often experience difficulties with inhibition. This study had the goal of understanding inhibition in children with ASD, ADHD, ASD + ADHD, and children who are typically developing (TD) using tasks that measured several aspects of inhibition. Results indicate that children with ASD + ADHD had greater difficulty inhibiting behavioral responses than TD children. Children with ASD + ADHD also differed from children with ASD and with ADHD in their inhi

Interpretation of the adult polysomnogram.

Polysomnography provides information on the physiological changes occurring in many different organ systems in relation to sleep stages and wakefulness. It allows qualitative and quantitative documentation of abnormalities of sleep and wakefulness, of sleep-wake transition, and of physiological function of other organ systems that are influenced by sleep. Polysomnography is considered to be the "gold standard" for diagnosing sleep disordered breathing (SDB) and other sleep disorders; however, as with most other diagnostic tests, polysomnography is not ideal, but is rather the best available me

[Detecting phobic vertigo!].

The syndrome of phobic postural vertigo is characterized by a combination of non-rotational dizziness, subjective disturbance in the upright stance and gait, despite normal clinical balance tests. Phobic postural vertigo most frequently occurs in form of attacks. It may be accompanied by anxiety and panic symptoms. It is almost invariably associated with particular constellations of perceptual stimuli or social situations. There is a tendency for rapid conditioning, generalization, and avoidance behavior to develop. Frequently, onset of condition follows periods of particular psychosocial stre

Hypersensitivity pneumonitis in a raptor handler and a wild bird fancier.

BACKGROUND: Hypersensitivity pneumonitis has been associated with a variety of antigens in various settings. We have recently encountered one case of hypersensitivity pneumonitis from owl droppings and another case from a wild Moluccan cockatoo. OBJECTIVE: It is important to alert physicians to the possibility of hypersensitivity pneumonitis when dealing with wildlife workers and wild bird fanciers. METHOD: A report of two cases with progress and response to therapy. RESULTS: Clinical and serologic information are of value in the diagnosis of hypersensitivity pneumonitis due to birds and av

Online oral health misinformation and information quality: a scoping review.

AIM: Online oral-health misinformation and low-quality information are increasingly studied, but the dental evidence base remains fragmented across platforms, topics and measurement approaches. This scoping review mapped how online oral-health misinformation and information quality have been defined, measured and reported, and identified the extent to which patient-level outcomes have been directly examined. METHODS: A scoping review was conducted using the Arksey and O'Malley framework and reported in accordance with PRISMA-ScR. Searches of Scopus, Web of Science and Ovid Medline identified

The diverse relationship between self-prioritization effect and emotional valence: Self-positivity bias and self-negative repulsion.

Although the self-positivity bias (SPB) suggests a preferential association with positive valence, emerging evidence indicates a more robust self-interaction with negativity. We propose self-negativity repulsion (SNR)-an active avoidance mechanism-as a distinct process complementing SPB. Using a novel nonsemantic paradigm, we examined the bidirectional influence between self-relevance (self, stranger, none) and emotional valence (positive, negative, neutral) across two tasks: one assessing emotional valence's impact on self-processing (Social Task), the other vice versa (Emotional Task). FilterformerPose: Satellite Pose Estimation Using Filterformer.

Satellite pose estimation plays a crucial role within the aerospace field, impacting satellite positioning, navigation, control, orbit design, on-orbit maintenance (OOM), and collision avoidance. However, the accuracy of vision-based pose estimation is severely constrained by the complex spatial environment, including variable solar illumination and the diffuse reflection of the Earth's background. To overcome these problems, we introduce a novel satellite pose estimation network, FilterformerPose, which uses a convolutional neural network (CNN) backbone for feature learning and extracts featu

"Association splitting" versus cognitive remediation in obsessive-compulsive disorder: A randomized controlled trial.

Studies have confirmed the efficacy of the cognitive intervention Association Splitting (AS) in obsessive-compulsive disorder (OCD) when applied as a self-help technique. AS aims to alter symptom-provoking automated cognitive networks of OC-related stimuli by building new or strengthening established but weak neutral associations. The aim of this study was to investigate the acceptance and benefits of therapist-assisted AS as an add-on to cognitive behavioral therapy (CBT). One hundred and nine patients with OCD who were undergoing CBT were randomly assigned to either AS or cognitive remediati

HIV testing experiences of Aboriginal youth in Canada: service implications.

The objective of this study was to explore HIV testing experiences and service views of Canadian Aboriginal youth in order to provide information for HIV testing services. An exploratory, mixed-method, community-based research design was used for this study. Findings reported here are from 210 survey participants who had experienced an HIV test. Youth were recruited through 11 Aboriginal organizations across Canada, including AIDS service organizations, health centers, community organizations, and friendship centers. Youth who had tested for HIV ranged in age from 15 to 30 years of age (20% we

AI feedback-seeking and learning engagement in art education: psychological antecedents and reflective mechanisms.

INTRODUCTION: Generative artificial intelligence provides students with a feedback source that differs from traditional teacher and peer interaction, yet limited research has explained why students actively seek feedback from AI or how such feedback-seeking is associated with subsequent learning engagement. Drawing on transactional stress and coping theory and social cognitive theory, this study examined the psychological antecedents and reflective mechanisms of AI feedback-seeking in art education. METHODS: Two complementary studies were conducted. Study 1 utilized survey data from 315 highe
