State-of-the-evidence reviews: advantages and challenges of including grey literature.

BACKGROUND: Increasingly, health policy decision-makers and professionals are turning to research-based evidence to support decisions about policy and practice. Systematic reviews are useful for gathering, summarizing, and synthesizing published and unpublished research about clearly defined interventions. State-of-the-evidence reviews are broader than traditional systematic reviews and may include not only published and unpublished research, but also published and unpublished non-research literature. Decisions about whether to include this "grey literature" in a review are challenging and lea

Predicting the likelihood of falls among the elderly using likelihood basis pursuit technique.

OBJECTIVES: Epidemiological studies have shown that low levels of dehydroepiandrosterone might increase the risk of developing metabolic syndrome. The aim of this study was to evaluate whether dehydroepiandrosterone supplementation in schizophrenic patients treated with olanzapine would influence the anthropometric and biochemical parameters of metabolic syndrome. METHODS: Male schizophrenic patients (no=55) participated in a twelve-week, randomized, double blind, placebo controlled study. They received 100 mg dehydroepiandrosterone (DHEA) or placebo as an augmentation of olanzapine treatment

Systematic review of sexual problems: epidemiology and methodology.

We conducted a comprehensive systematic literature review of five sexual problems in order to estimate their population prevalence and to investigate issues arising from the reviewing of epidemiological literature. Two independent reviewers scrutinized abstracts, extracted data, and assessed methodological quality. Twenty-eight relevant studies were identified. Prevalence figures varied considerably, and meta-analysis was not possible because of heterogeneity in study quality and design. Prevalence depends on case definition, characteristics of the study population, and time frame of the preva

CeRTS: certainty retrieval token search in large language model clinical information extraction.

OBJECTIVE: Large language models (LLMs) must effectively communicate their uncertainty to be viable in clinical settings. As such, the need for reliable uncertainty estimation grows increasingly urgent with the expanding use of LLMs for information extraction from electronic health records. Previous token-level uncertainty estimators have only used token probabilities within a single output sequence. Here, by leveraging the constraints of JSON output structure, we instead consider all likely sequences and their respective probabilities to obtain a more robust measure of model confidence. We de

A comparative analysis of large language models versus traditional information extraction methods for real-world evidence of patient symptomatology in acute and post-acute sequelae of SARS-CoV-2.

BACKGROUND: Patient symptoms, crucial for disease progression and diagnosis, are often captured in unstructured clinical notes. Large language models (LLMs) offer potential advantages in extracting patient symptoms compared to traditional rule-based information extraction (IE) systems. METHODS: This study compared fine-tuned LLMs (LLaMA2-13B and LLaMA3-8B) against BioMedICUS, a rule-based IE system, for extracting symptoms related to acute and post-acute sequelae of SARS-CoV-2 from clinical notes. The study utilized three corpora: UMN-COVID, UMN-PASC, and N3C-COVID. Prevalence, keyword and fa

Implications of Data Extraction and Processing of Electronic Health Records for Epidemiological Research: Observational Study.

BACKGROUND: The use of routinely recorded electronic health record (EHR) data is increasingly common, especially in epidemiological research. However, data must be processed and prepared for secondary use, and decisions made during this process could significantly impact research outcomes. A demonstration of the extent of these consequences is necessary. OBJECTIVE: The aim of this study was to investigate the influence of data processing steps on research outcomes derived from the secondary use of EHR data. METHODS: EHR data from 8 Dutch general practices from 2019 were used. These practices

The AI and Automation in Anaesthesia (TRIPLE-A) Perioperative Database in Eastern Denmark 2017-: A Platform for Epidemiology, AI-Driven Prediction, Quality Control and Automated Data Retrieval.

INTRODUCTION: Electronic health records can be used to create high-quality databases if data are structured and well-registered, which is the case for most perioperative data in the Capital and Zealand Regions of Denmark. We present the purpose and development of the AI and Automation in Anaesthesia (TRIPLE-A) database-a platform designed for epidemiology, prediction, quality control, and automated research data collection. METHODS: Data collection from the electronic medical record (EPIC Systems Corporation, WI, USA) was approved by the Capital Region, Denmark, and ethical approval was waive

Triangulating data sources for further learning from and about the MDSR in Ethiopia: a cross-sectional review of facility based maternal death data from EmONC assessment and MDSR system.

BACKGROUND: Triangulating findings from MDSR with other sources can better inform maternal health programs. A national Emergency Obstetric and Newborn Care (EmONC) assessment and the Maternal Death Surveillance and Response (MDSR) system provided data to determine the coverage of MDSR implementation in health facilities, the leading causes and contributing factors to death, and the extent to which life-saving interventions were provided to deceased women. METHODS: This paper is based on triangulation of findings from a descriptive analysis of secondary data extracted from the 2016 EmONC asses

COVID-19 Case Detection: Cuba's Active Screening Approach.

Meningitis, neuropathy, HIV, dengue-since the 1960s, Cuba has faced its share of epidemics. More recently, Cuban health pro-fessionals tackled domestic outbreaks of H1N1 (2009) and Zika (2016), and worked alongside colleagues from around the world to stem Ebola in West Africa; all three were categorized by WHO as public health emergencies of international concern. In December 2019, China reported its fi rst cluster of pneumo-nia cases, later identifi ed as the novel coronavirus disease COVID-19. In January 2020, Cuban authorities convened a multi-sector working group coordinated by the Ministr

Professional collaboration in searching the evidence for an ill-defined concept.

This paper outlines the inter-professional collaboration of the authors, a PhD student, his supervisor and an information professional, to systematically search the literature for an ill-defined concept. The research question posed for the scoping literature review indicated that the topic, the subjective socio-economic status and health of adults with intellectual disabilities, was rare. The need for a methodological search process was therefore identified and successfully carried out. The paper presents an analysis of the processes and the collaboration involved in developing a successful se

Nora: A Vocabulary Discovery Tool for Concept Extraction.

Coverage of terms in domain-specific terminologies and ontologies is often limited in controlled medical vocabularies. Creating and augmenting such terminologies is resource intensive. We developed Nora as an interactive tool to discover terminology from text corpora; the output can then be employed to refine and enhance natural language processing-based concept extraction tasks. Nora provides a visualization of chains of words foraged from word frequency indexes from a text corpus. Domain experts direct and curate chains that contain relevant terms, which are further curated to identify lexic

[Extent and use of administrative hospital data in the German DRG system].

In 2003 the mandatory introduction of diagnosis related groups as an overall payment system started for all hospitals in Germany, except for departments of psychiatry and psychotherapy as well as psychosomatics and psychotherapy. All hospitals are legally obliged to deliver extensive data on hospital treatment, including demographic data, diagnoses, comorbidities, complications, and procedures, to the "Institute for the Hospital Remuneration System" which uses the data for yearly adaptation of the German DRG system (G-DRG). This article describes legal issues of data collection, parameters and

Estimating the Horizon of articles to decide when to stop searching in systematic reviews: an example using a systematic review of RCTs evaluating osteoporosis clinical decision support tools.

Researchers conducting systematic reviews need to search multiple bibliographic databases such as MEDLINE and EMBASE. However, researchers have no rational search stopping rule when looking for potentially-relevant articles. We empirically tested a stopping rule based on the concept of capture-mark-recapture (CMR), which was first pioneered in ecology. The principles of CMR can be adapted to systematic reviews and meta-analyses to estimate the Horizon of articles in the literature with its confidence interval. We retrospectively tested this Horizon Estimation using a systematic review of rando

Treatment of missing values with imputation for the analysis of otologic data.

Usefulness of imputation in the treatment of missing values in an otologic database was studied. Missing values were filled in with means (ME), regression (LR) and Expectation-Maximization (EM) imputation methods. A random imputation method (RA) provided baseline results. ME, LR and EM methods agreed on 41-42% of the imputed missing values. The level of agreement between these and RA method was 20-22%. Despite the moderate agreement, discriminant functions were similar and accurate (prediction accuracy 83-99%) for each diagnosis. A lot of data were missing in otoneurotologic tests which have l

A framework for genetic service provision for haemophilia and other inherited bleeding disorders.

This framework document offers guidance to patients, doctors, nurses, laboratory scientists, funders and hospitals on the provision of clinical and laboratory genetic services for haemophilia. With recent advances in molecular laboratory techniques it is now possible to give the vast majority of individual patients and family members very reliable genetic information. To enable these genetic data to be used for both the optimal treatment of patients with inherited bleeding disorders and for appropriate reproductive decisions in carriers, there needs to be a clear and robust framework for syste
