Does psychological distress predict risk of orthopaedic surgery and postoperative opioid prescribing in patients with hip pain? A retrospective study.

BACKGROUND: Clinicians and public health professionals have allocated resources to curb opioid over-prescription and address psychological needs among patients with musculoskeletal pain. However, associations between psychological distress, risk of surgery, and opioid prescribing among those with hip pathologies remain unclear. METHODS: Using a retrospective cohort study design, we identified patients that were evaluated for hip pain from January 13, 2020 to October 27, 2021. Patients' surgical histories and postoperative opioid prescriptions were extracted via chart review. Risk of hip surge

Neonatal circumcision.

Because of an economically motivated study, there has been an attempt to shift insurance dollars from neonatal circumcision to neonatal intensive care. Negative comments regarding the advisability of early circumcision have created confusion for both physicians and parents. Neonatal circumcision is the most common surgical operation in the United States, with probably fewer complications than any other. I believe this valid prophylactic procedure is falling victim to an unwarranted bias.

Hematopoietic Cell Transplantation Outcomes among Medicaid and Privately Insured Patients with Sickle Cell Disease.

Compared with privately insured patients, recipients of Medicaid have been reported to have worse outcomes in several clinical conditions and following various surgical and medical procedures. However, the relationship between health insurance status and allogeneic hematopoietic cell transplantation (alloHCT) outcomes among patients with sickle cell disease (SCD) is not well described. We sought to compare alloHCT outcomes between patients with SCD who underwent alloHCT while enrolled on Medicaid versus those who underwent alloHCT while covered by private health insurance. We conducted a retro

[Practical Approaches for Post-Operative and Post-Traumatic Lymphoedemas].

Swelling of the extremities is frequently observed after traumatic lesions and poses differential diagnostic challenges. Causes include seromas, haematomas and thromboses, as well as lymphostatic oedemas, which may become chronic if left untreated. These are characterized by a high percentage of protein and, over the long term, they can lead to considerable impairment and complications such as recurrent erysipelas as a result of fibrotic tissue rearrangement. Usually a clinical diagnosis is made, which presupposes that the symptoms are known. The most important treatment option is complete dec

Mandible Fracture Fixation Adverse Outcomes Higher For Patients With Human Immunodeficiency Virus (HIV), Even if on Antiretroviral Therapy.

BACKGROUND: There is limited information on how human immunodeficiency virus (HIV) status correlates with outcomes following craniofacial procedures. The current study aimed to evaluate the relationship between HIV status and outcomes following mandibular fracture open reduction and internal fixation (ORIF) with and without antiretroviral therapy (ART) using a large, national cohort. METHODS: Adults (18 y or older) with closed mandible fractures who underwent ORIF within 14 days of diagnosis were identified in the 2010 to 2023 PearlDiver M170 database. HIV status, ART prescriptions, and patie

Social Determinants of Health and End-of-Life Circumstances in a Quaternary Children's Hospital.

CONTEXT: Inequitable access to quality care based on social determinants of health (SDoH) adversely impacts outcomes. Prior studies suggest that disparities extend to end-of-life (EOL) and subspecialty pediatric palliative care (SPPC) utilization, although pediatric data remain limited. OBJECTIVES: To examine relationships between SDoH, SPPC, and EOL circumstances among decedents in a quaternary children's hospital. METHODS: We conducted a retrospective cohort study of all inpatient deaths at a quaternary pediatric hospital (1.1.2018-12.31.2023). Clinical data, SDoH (race/ethnicity, interpre

Low Back Pain: Utilization of Urgent Cares Relative to Emergency Departments.

STUDY DESIGN/SETTING: Retrospective study. OBJECTIVE: To understand why patients utilize emergency departments (EDs) versus urgent care centers for low back pain (LBP). SUMMARY OF BACKGROUND DATA: LBP is a common reason for ED visits. In the setting of trauma or recent surgery, the resources of EDs may be needed. However, urgent care centers may be appropriate for other cases. MATERIALS AND METHODS: Adult patients below 65 years of age presenting to the ED or urgent care on the day of diagnosis of LBP were identified from the 2019 PearlDiver M151 administrative database. Exclusion criteria

New persistent opioid use after bariatric surgery: a systematic review and pooled proportion meta-analysis.

INTRODUCTION: Increasing evidence suggests surgical patients are at risk for developing new, persistent opioid use (NPOU) following surgery. This risk may be heightened for patients undergoing bariatric surgery. Few studies have evaluated this important long-term outcome and little is known about the rate of NPOU, or factors associated with NPOU for bariatric surgery patients. METHODS AND PROCEDURE: We conducted a systematic review of MEDLINE, Embase, Scopus, Web of Science, and Cochrane databases in August 2021. Studies were reviewed and data extracted independently by two reviewers followin

The James A. Rand Young Investigator's Award: Administrative Claims vs Surgical Registry: Capturing Outcomes in Total Joint Arthroplasty.

BACKGROUND: Administrative claims in total joint arthroplasty are used for observational studies and payment adjustments under the Comprehensive Care for Joint Replacement (CJR) legislation. Claims data have not been validated against prospective surgical outcome registries for primary total hip (THA) or knee arthroplasty (TKA). We hypothesized that significant differences in reported comorbidity and adverse event measures exist between administrative claims and prospective registry data relevant to payment adjudication under the CJR reimbursement model. METHODS: Comorbidities and outcomes in

The cancer survivor.

Patients who have completed therapy for malignant disease face many problems, including the possibility and persistent fear of recurrent disease or a new malignancy. The side effects of therapy, including surgical scars, radiation effects or damage to physiological function may strongly influence patients' complete recovery. Depletion of financial resources or a lengthy interruption of employment may present serious difficulties, particularly if patients cannot function as well as before. They may also find it difficult to buy life insurance, or mistakenly believe that they are not eligible to

Impact of middle meningeal artery embolization on neurological complications and discharge outcomes in chronic subdural hematoma: A machine learning-based comparative study.

INTRODUCTION: Chronic subdural hematoma (cSDH) predominantly affects older adults, often those with prior head trauma, anticoagulation therapy, or chronic comorbidities. Traditional management involves surgical evacuation; however, middle meningeal artery (MMA) embolization has emerged as a less invasive alternative with the potential for fewer complications. This study compares outcomes of surgical evacuation, MMA embolization, and combination therapy using machine learning to identify predictors of neurological complications and discharge disposition. METHODS: Patients with cSDH from the 20

Sick leave and disability pension in a cohort of TMD-patients - The Swedish National Registry Studies for Surgically Treated TMD (SWEREG-TMD).

BACKGROUND: Temporomandibular disorders (TMD) are common and affect approximately 10% of the adult population. TMD is usually associated with headache, pain in the masticatory muscles and/or the temporomandibular joint, clicking or crepitations during mandibular movement as well as painful and/or reduced mouth opening. This study aimed to investigate the level TMD-patients use social insurance benefits before and after their first time of diagnosis or first surgical event, compared to the general population. Furthermore, the aim was to investigate the differences in the use of social insurance
