Overview of the 2012 Food and Drug Administration circulatory system devices panel meeting on the reclassification of external counterpulsation, intra-aortic balloon pump, and non-roller-type cardiopulmonary bypass blood pump devices.

The Food and Drug Administration held a Circulatory System Devices Advisory Panel meeting, December 5 and 6, 2012, to review the classification or potential reclassification of the following device types: external counterpulsation, intra-aortic balloon pump (IABP), and non-roller-type cardiopulmonary bypass blood pumps. These 3 devices are preamendment (Medical Device Amendments of 1976) class III devices. The advisory panel discussed the data and provided recommendations for reclassification of these devices. The panel recommended reclassification of ECP to class II for stable angina pectoris

Intra-aortic balloon pump support and early mortality after surgical repair of post-infarction ventricular septal rupture: A 24-year real-world cohort.

BACKGROUND: Post-infarction ventricular septal rupture (VSR) remains a rare but highly lethal complication of acute myocardial infarction, with persistently high early mortality. Although surgical repair is the definitive treatment, the optimal perioperative management strategy remains uncertain, particularly regarding the role of intra-aortic balloon pump (IABP) support. METHODS: We conducted a retrospective cohort study including consecutive patients with post-infarction VSR who underwent surgical repair at a tertiary cardiovascular centre over a 24-year period. Clinical characteristics, ti

Concurrent intra-aortic balloon pump and veno-arterial extracorporeal membrane oxygenation for acute coronary syndrome-related cardiogenic shock: A meta-analysis of multivariate studies.

Concurrent intra-aortic balloon pump (IABP) use has been suggested to reduce mortality in patients with acute coronary syndrome (ACS)-related cardiogenic shock (CS) on veno-arterial extracorporeal membrane oxygenation (ECMO). However, this observation is primarily based on small-scale univariate studies. The aim of this meta-analysis was to evaluate whether concurrent IABP and ECMO were independently associated with reduced mortality in patients with ACS-related CS. We searched Medline, Web of Science, and Embase for studies published up to May 28, 2024. The inclusion criteria were longitudina

Dynamic Arterial Lactate Values Are Associated with 30-Day Mortality in Patients with Acute Myocardial Infarction and Cardiogenic Shock on Intra-Aortic Balloon Pump Circulatory Support.

INTRODUCTION: In patients with acute myocardial infarction and cardiogenic shock (AMICS), the intra-aortic balloon pump (IABP) remains the most commonly used form of mechanical circulatory support. However, information on the characteristics of nonresponders is limited. This study evaluated the risk factors associated with 30-day mortality in a cohort of patients with AMICS, on IABP support. METHODS: The medical records of patients admitted for AMICS, who underwent IABP insertion over a period of 5 years, were extracted from the electronic database of a tertiary cardiovascular disease center.

Intra-aortic balloon pump implantation in early-stage cardiogenic shock reduces 30-day mortality in patients with ventricular septal defect following myocardial infarction.

BACKGROUND: Ventricular septal defect (VSD) is a serious mechanical complication of acute myocardial infarction, with an extremely high mortality. Intra-aortic balloon pump (IABP) has become a bridge to surgical repair. However, its effect on mortality varies partly because of shock severity. AIMS: This study aims to evaluate the association between IABP implantation in early-stage cardiogenic shock and the 30-day all-cause mortality in patients with VSD after myocardial infarction (MI-VSD). METHODS: The retrospective data were collected based on MI-VSD patients who were admitted to Qilu Hos

In-hospital outcomes of cardiogenic shock patients: A propensity score-matched nationwide comparative analysis between intra-aortic balloon pump and percutaneous ventricular assist devices.

BACKGROUND: Percutaneous ventricular assist devices (pVAD) and intra-aortic balloon pumps (IABP) are mechanical circulatory support options for patients with cardiogenic shock (CS). While pVADs provide greater hemodynamic support, their impact on mortality and hospital outcomes compared to IABP remains unclear. METHODS: We conducted a propensity score-matched analysis of 65,858 CS admissions from the national inpatient sample (2016-2020), evenly divided between IABP and pVAD groups. Admissions, where ECMO or both IABP and pVAD were used during the same admission, were excluded. The primary ou

Intra-aortic balloon pump still has a role in late-onset myocardial infarction complicated by ventricular septal rupture with intractable heart failure: a case report.

BACKGROUND: The current guidelines have discouraged the routine use of intra-aortic balloon pump (IABP) in cardiogenic shock complicating acute coronary syndrome (ACS). Since then, the trend of IABP utilization in ACS has been declining. Nevertheless, the guidelines still preserve the recommendation of IABP use in hemodynamic instability or cardiogenic shock caused by post myocardial infarction (MI) ventricular septal rupture (VSR). CASE PRESENTATION: A 46-years-old diabetic Southeast Asian female was referred from a peripheral facility with intractable heart failure despite treatment with va

Efficacy of Intra-aortic Balloon Pump before versus after Primary Percutaneous Coronary Intervention in Patients with Cardiogenic Shock from ST-elevation Myocardial Infarction.

BACKGROUND: Previous studies showed that patients with cardiogenic shock (CS) from ST-elevation acute myocardial infarction (STEMI) supported by intra-aortic balloon pump (IABP) before primary percutaneous coronary intervention (PCI) decreased the risk of in-hospital mortality than patients who received IABP after PCI. However, little evidence is available on the optimal order of IABP insertion and primary PCI. The aim of this study was to investigate the impact of the sequence of IABP support and PCI and its association with major adverse cardiac and cerebrovascular events (MACCEs). METHODS:

Intra-Aortic Balloon Pump Catheter Insertion Using a Novel Left External Iliac Artery Approach in A Case of Chronic Heart Failure Due to Dilated Cardiomyopathy.

BACKGROUND The use of an intra-aortic balloon pump (IABP) in patients with advanced heart failure can provide interim mechanical support as a bridge to further treatment, including cardiac transplantation. The femoral artery, axillary artery, and subclavian artery are the main approaches to IABP catheter placement. A case is reported of the use of a left external iliac artery approach to IABP catheter placement using a subcutaneous channel in a patient with chronic heart failure. CASE REPORT A 67-year-old woman presented with a history of heart failure. She had New York Heart Association (NYHA

Percutaneous left ventricular assist device vs. intra-aortic balloon pump in patients with severe left ventricular dysfunction undergoing cardiovascular intervention: A meta-analysis.

OBJECTIVE: Although controversial, the intra-aortic balloon pump (IABP) and percutaneous left ventricular assist device (PLVAD) are widely used for initial hemodynamic stabilization. We performed a meta-analysis to compare the clinical outcomes of these two devices in patients with severe left ventricular (LV) dysfunction undergoing percutaneous coronary intervention (PCI) or ventricular tachycardia (VT) ablation. METHODS: MEDLINE, EMBASE, the Cochrane Registry of Controlled Trials, and reference lists of relevant articles were searched. We included randomized controlled trials (RCTs) and pro

Comparative Outcomes of Direct Impella Use vs. Intra-Aortic Balloon Pump (IABP)-to- Impella Escalation Strategy in Cardiogenic Shock: Insights from the National Inpatient Sample.

BACKGROUND: Impella and IABP are widely used temporary mechanical circulatory support devices in cardiogenic shock (CS). While the Impella is used as an initial support or after IABP as escalation strategy, comparative data on these two strategies remains limited. METHODS: Using ICD-10 codes in the 2016-2022National Inpatient Sample (NIS) database, we identified patients with primary diagnosis of CS who received Impella support and divided in two groups: direct Impella use and escalation from IABP to Impella. Propensity score matching was performed to adjust for confounding. RESULTS: Among 1

Comparative Outcomes of Intra-Aortic Balloon Pump Versus Percutaneous Left Ventricular Assist Device in High-Risk Percutaneous Coronary Intervention: A Systematic Review and Meta-Analysis.

Background/Objectives: High-risk percutaneous coronary interventions (HR-PCIs) often require mechanical circulatory support (MCS) to maintain hemodynamic stability. Intra-aortic balloon pump (IABP) and percutaneous left ventricular assist device (PLVAD) are two commonly used MCS devices that differ in their mechanisms. We aimed to evaluate and compare the clinical outcomes associated with IABP and PLVAD use in HR-PCIs without cardiogenic shock. Methods: We conducted a search of PubMed, Scopus, Cochrane, Mendeley, Web of Science, and Embase to identify relevant randomized controlled trials and

Percutaneous coronary intervention assisted by invasive mechanical ventilation and intra-aortic balloon pump for acute myocardial infarction with cardiogenic shock: Retrospective cohort study and meta-analyses.

There is little evidence to recommend the optimal invasive mechanical ventilation (IMV) modes and ideal positive end-expiratory pressure stress levels for acute myocardial infarction-cardiogenic shock (AMI-CS) patients. The aim of this study was to compare the mortality outcome in patients with AMI-CS who were treated with percutaneous coronary intervention (PCI) assisted by intra-aortic balloon pump (IABP) + IMV with historical controls. From January 1, 2016 to June 1, 2017, 60 patients were retrospectively enrolled at Tianjin Chest Hospital. Out of these, 88.3% of patients achieved thromboly
