[Intensive care and perioperative monitoring in liver transplantation].

Perioperative intensive care has several objectives in the context of liver transplantation. Prior to the operation, it is essential for the management of fulminant hepatic failure in order to limit the consequences of cerebral oedema. During the operation, intensive care consists of monitoring and correction of the occasionally severe circulatory and metabolic disorders induced by vascular exclusion of the liver and revascularization of the cold, ischaemic, orthotopic transplant. The possibility of major haemorrhage requiring massive transfusion with stored blood aggravates these disorders. C

Do Patients Want to Be Involved in Their Carpal Tunnel Surgery Decisions? A Multicenter Study.

PURPOSE: Carpal tunnel syndrome requires multiple decisions during its management, including regarding preoperative studies, surgical technique, and postoperative wound management. Whether patients have varying preferences for the degree to which they share in decisions during different phases of care has not been explored. The goal of our study was to evaluate the degree to which patients want to be involved along the care pathway in the management of carpal tunnel syndrome. METHODS: We performed a prospective, multicenter study of patients undergoing carpal tunnel surgery at 5 academic medi

Selective intraoperative cholangiography: A retrospective single-centre analysis compared with national data and implications for surgical training.

PURPOSE: Intraoperative cholangiography (IOC) can assist in visualising the biliary anatomy during laparoscopic cholecystectomy, particularly in cases with unclear anatomical conditions, and may thereby help to prevent complications such as bile duct injury. Despite its potential relevance, the method is infrequently used in many hospitals and is often not included in surgical training curricula. The aim of this study was to quantify the selective, on-demand use of IOC in a specialised visceral surgery centre, compare local data with national statistics, and interpret the findings in the conte

Brain tumors in eloquent areas: A European multicenter survey of intraoperative mapping techniques, intraoperative seizures occurrence, and antiepileptic drug prophylaxis.

Intraoperative mapping and monitoring techniques for eloquent area tumors are routinely used world wide. Very few data are available regarding mapping and monitoring methods and preferences, intraoperative seizures occurrence and perioperative antiepileptic drug management. A questionnaire was sent to 20 European centers with experience in intraoperative mapping or neurophysiological monitoring for the treatment of eloquent area tumors. Fifteen centers returned the completed questionnaires. Data was available on 2098 patients. 863 patients (41.1%) were operated on through awake surgery and int

Does routine intra-operative cholangiography reduce the risk of biliary injury during laparoscopic cholecystectomy? An evidence-based approach.

The role of routine intra-operative cholangiography (IOC) during cholecystectomy remains controversial. The purpose of this evidence-based review was to analyze the role of IOC in the prevention and detection of biliary ductal injury during cholecystectomy. The relative rarity of biliary complications means that the randomized trials and meta-analyses that include fewer than 12,000 patients cannot answer the question. Therefore, only six comparative studies were included in this review. The conclusions of these studies were conflicting, half showing a protective effect of routine IOC and the o

[Anesthetic management of undiagnosed pheochromocytoma: a case report].

A 55-year-old man was scheduled for resection of an adrenal tumor under the diagnosis of a non-functioning adrenal tumor. He was admitted for unstable angina 58 days preoperatively, and on the second hospital day, he had an episode of ventricular tachycardia for 20 seconds following epigastric pain. Abdominal computed tomography scans revealed a retroperitoneal abscess, which was subsequently drained effectively, and a right adrenal tumor. Because of the almost normal plasma catecholamine concentration, the tumor was considered a nonfunctioning tumor. However, when the surgeon manipulated the

[Cardiac arrest during bilateral inguinal herniorrhaphy under epidural anesthesia].

A 73-year-old man with hyper gamma-globulinemia was scheduled for bilateral herniorrhaphy and hemorroidectomy. In preoperative examination, the serum concentrations of gamma-globulin and total protein were 7.31 g x dl(-1) and 11.9 g x dl(-1). Although platelet count was 6.5 x 10(4) x microl(-1), epidural anesthesia was selected to minimize hemodynamic changes which could increase a risk of thrombosis. Epidural catheters were inserted at T 12-L 1 and L 4-5. Analgesia level checked by a pin prick was T 4 after administering 10 ml of 2% lidocaine, 7 ml into T 12-L 1 and 3 ml into L 4-5. Bilateral

[Anesthetic management of a patient with Bartter's syndrome].

A 48-year-old woman with Bartter's syndrome underwent right mastectomy under general anesthesia. Her operative course was uneventful. She was preoperatively complicated with severe hypokalemia but had no signs and symptoms of hypokalemia. In anesthetic care of patients with Bartter's syndrome, even when they have no symptoms of hypokalemia, the meticulous intravenous administration of potassium chloride is required in order to maintain the preoperative level of the serum potassium during anesthesia. In addition, attention should be paid to factors causing an additional reduction in the serum p

[Anesthesia experience of three patients with partial intramural coronary artery].

We experienced three patients with partial intramural coronary artery diagnosed preoperatively. We gave balanced anesthesia using narcotics and non-narcotic analgesics for non-cardiac surgery, administering phenylephrine against hypotension. The anesthetic course was relatively stable. According to the special pathophysiology of this disease, tachycardia and hypotension associated with anesthesia should be avoided, and special caution is needed when nitroglycerine is used due to its adverse effect on systolic narrowing of the coronary artery.

Management of segmental intercostal and lumbar arteries during descending and thoracoabdominal aneurysm repairs.

The etiology of spinal cord injury after descending or thoracoabdominal operation is multifactorial. There are three major events that may cause the injury. This review addresses the second, namely the operative management of the segmental intercostal and lumbar arteries. However, failure to address any of the etiological events may result in injury, and thus a multimodal approach is required to prevent spinal cord injury.

Clinical evaluation of the safety of repetitive intraoperative defibrillation threshold testing.

One goal of the initial implantation procedure for a cardioverter defibrillator is determination of the configuration and patch location with the lowest defibrillation threshold (DFT). To determine the safety of multiple defibrillation tests, an analysis of the intraoperative defibrillation threshold tests (DFTT) in our patients was performed. In 84 patients, the mean number of DFT trials was 5.27; the mean number of joules received was 275.0. The maximum number of shocks in one implant procedure was 50 for a total of 4,895 joules without complications. Four patients received 30 or more DFT sh

How to prepare the patient for robotic surgery: before and during the operation.

Robotic surgery in the treatment of gynecologic diseases continues to evolve and has become accepted over the last decade. The advantages of robotic-assisted laparoscopic surgery over conventional laparoscopy are three-dimensional camera vision, superior precision and dexterity with EndoWristed instruments, elimination of operator tremor, and decreased surgeon fatigue. The drawbacks of the technology are bulkiness and lack of tactile feedback. As with other surgical platforms, the limitations of robotic surgery must be understood. Patient selection and the types of surgical procedures that can

Frozen section: guiding the hands of surgeons?

Frozen section (FS) analysis is a powerful tool that can provide a rapid diagnosis, directing operative management. However, FSs can also be misused. We consider an FS to be "inappropriate" when it does not influence operative management or immediate patient care. Not only can inappropriate FSs compromise diagnostic material, they can impact turnaround time of other FSs. We evaluated the utilization of FSs at our institution and assessed influence on intraoperative management. Frozen sections performed at the University of Alabama at Birmingham Hospital in 2013 were stratified by surgical subs

Therapeutic hypothermia and traumatic brain injury.

PURPOSE OF REVIEW: Therapeutic hypothermia after traumatic brain injury (TBI)? For the last 10 years, no topic has been more popular and more controversial among neurointensivists. This article reviews the most current findings (experimental, clinical, adult and pediatric TBI), as well as the clinical management of therapeutic hypothermia. RECENT FINDINGS: Despite ample experimental evidence, the clinical utility of therapeutic hypothermia has still to be conclusively demonstrated in terms of reduced mortality or improved functional recovery after TBI (even in pediatric TBI). Current findings

[Surgical treatment of tubal pregnancy using a laparoscopic approach].

Laparoscopy was employed to treat extrauterine pregnancy in 16 patients aged 19 to 38 years. An operation was the continuum of diagnostic laparoscopy in 3 patients, but it was emergency in 13 after the diagnosis was established by other methods. Tubectomy was performed in 15 patients, conservative removal of the fetal ovum from the tubal lumen was made in 1 patient. No complications were observed. The operation duration was 12 to 20 minutes. The postoperative period ran significantly easier than that after tubectomies conducted by laparotomy.
