Enteral supplementation of phosphate does not prevent hypophosphatemia during refeeding of cachectic patients.

Hypophosphatemia due to parenteral nutrition has been described frequently. It was attributed to the lack of phosphorus content in parenteral nutrition solutions. With modern parenteral nutrition regimens containing phosphorus, this problem has been virtually eliminated. Enteral nutrition solutions contain adequate phosphate for patients with normal phosphate stores. Hypophosphatemia has therefore rarely been reported in enteral nutrition. We describe two patients with protein-energy malnutrition who developed severe hypophosphatemia during tube feeding with phosphorus-containing formula diets

Post-Endoscopic Retrograde Cholangiopancreatography Pancreatitis Prophylaxis: Does the Choice of Crystalloid Fluid Matter? A Systematic Review and Meta-analysis of Randomized Controlled Trials.

OBJECTIVES: The optimal crystalloid choice for post-ERCP pancreatitis (PEP) prophylaxis is not clear. To evaluate the published literature further, we conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) comparing intravenous (IV) hydration with lactated Ringer (LR) and normal saline (NS) for PEP prevention. MATERIALS AND METHODS: A random-effects model was used. The primary outcome was the risk of PEP. Effect estimates were expressed as risk difference (RD) and 95% CI. RESULTS: Pooling analysis of published RCTs with 594 patients (41.1% males, mean age: 57.

Hemodynamic support of the trauma patient.

PURPOSE OF REVIEW: The aim of this review is to address and summarize some key issues and recent insights into the hemodynamic support of the trauma patient related to fluid administration. RECENT FINDINGS: Colloids are not superior to crystalloids in treating hypovolemia in the trauma patient and show no survival benefit. Furthermore, several adverse effects (renal failure, bleeding complications and anaphylaxis) have been reported with the use of artificial colloids. Hypertonic saline is effective and well tolerated in the treatment of hypovolemic shock and traumatic brain injury. Potential

Do colloids in comparison to crystalloids for fluid resuscitation improve mortality?

Permanent neurological impairment or death arising from hospital-acquired hyponatremia in both children and adults is well documented. The choice of intravenous fluids for fluid resuscitation in critically ill patients is a top priority in evidence-based medicine. The question of whether colloids in comparison to crystalloids can improve mortality in such cases remains to be answered. Well powered, randomized clinical trials addressing the comparative efficacy of different types of intravenous fluids is a high priority as is the ethical justification for such trials. The understanding of the p

Hypertonicity, but not hypothermia, elicits substance P release from rat C-fiber neurons in primary culture.

Isocapnic dry gas hyperventilation provokes hyperpnea-induced bronchoconstriction in guinea pigs by releasing tachykinins from airway sensory C-fiber neurons. It is unknown whether dry gas hyperpnea directly stimulates C-fibers to release tachykinins, or whether this physical stimulus initiates a mediator cascade that indirectly stimulates C-fiber tachykinin release. We tested the hypotheses that mucosal hypothermia and/or hyperosmolarity--physical consequences of airway heat and water loss imposed by dry gas hyperpnea--can directly stimulate C-fiber tachykinin release. Neurons isolated from n

Relative contributions of the fraction of unfrozen water and of salt concentration to the survival of slowly frozen human erythrocytes.

As suspensions of cells freeze, the electrolytes and other solutes in the external solution concentrate progressively, and the cells undergo osmotic dehydration if cooling is slow. The progressive concentration of solute comes about as increasing amounts of pure ice precipitate out of solution and cause the liquid-filled channels in which the cells are sequestered to dwindle in size. The consensus has been that slow freezing injury is related to the composition of the solution in these channels and not to the amount of residual liquid. The purpose of the research reported here was to test this

Dipsogenic effect of urea in rats.

Anephric rats injected subcutaneously with urea in isotonic saline drank much more than anephric control animals receiving isotonic saline alone. The pattern of water intake and urine output of normal rats, repeatedly injected with urea solution, was similar to that of rats injected with dipsogenic hypertonic NaCl and quite different from that of rats receiving furosemide. A well-evident dipsogenic effect (unrelated to the urine output) was observed in normal rats repeatedly injected with urea solution and having a rapid rise of serum urea concentrations. On the contrary, in rats receiving a s

Thrombocytopenia in dogs with anticoagulant rodenticide-induced hemorrhage: eight cases (1990-1995).

Thrombocytopenia was documented in eight of 11 dogs with anticoagulant rodenticide-induced hemorrhage. Thrombocytopenia was transient and generally mild-to-moderate, but it became marked (i.e., less than 30,000 platelets/microl) in two cases. Petechial hemorrhages were not noted in any case. There was no relationship between hematocrit and platelet count. Platelet count changes in response to treatment with fresh-frozen plasma and isotonic electrolyte solutions were variable. Anticoagulant rodenticide toxicity should be included as a differential diagnosis for dogs with hemorrhage accompanied

pH effects on red cell deformability.

The effect of pH on erythrocyte deformability was determined in the ektacytometer. In isotonic solutions, cell deformability was essentially unaltered at pH values from 6.44 to 7.7, despite the known cell volume changes that occur over this pH range. Analysis by osmotic scan ektacytometry showed that erythrocyte deformability is homeostatically maintained constant during acidosis and alkalosis because of a balance between the countervailing effects of changes in intracellular viscosity and in the surface to volume ratio. This balance is perturbed in the sickle erythrocyte, so that acidosis imp…

Transthoracic electrical impendance in anaesthesia and intensive care.

Pulmonary fluid accumulation plays an important role in the development of post-traumatic pulmonary insufficiency. Yet initially diagnosis may be difficult. The measurement of transthoracic electrical impedance was used to detect early pulmonary fluid overload. In healthy test persons infusions of isotonic electrolyte solutions and diuresis were accompanied by changes of impedance dependent on the amount of infused or withdrawn fluid. In patients with pulmonary insufficiency a relatively low mean body impedance was recorded. Enforced diuresis resulted in a rise and in diminution of the alveola

Hypotonic and isotonic fluid overload as a complication of hysteroscopic procedures: two case reports.

Hysteroscopy is used extensively for both the diagnosis and treatment of intrauterine pathology. Although considered a safe procedure, complications such as cervical laceration, uterine perforation, absorption of irrigation solutions and, rarely, gas or air embolism may occur. The authors present two cases with successful outcomes, whereby complications following hysteroscopy occurred due to excessive fluid absorption. Hypotonic as well as isotonic distention media can cause serious complications. The authors focus on factors that may increase the risk of fluid overload, which should be known

Effects of volume expansion with Fluosol-DA on renal function of the nonhuman primate.

The renal responses to an intravenous infusion of a high 02 affinity fluorocarbon equal to 15% of the estimated blood volume was determined in 5 monkeys and 1 baboon. In response to the infusion, blood pressure, renal plasma flow and glomerular filtration rate did not change significantly while heart rate and central venous pressure increased transiently. Significant increases in sodium and potassium excretion and osmolal and free-water clearances occurred. The renal responses to Fluosol-DA (20%) mimic in general those observed when blood volume is expanded with isotonic isooncotic dextran sol

Measurement of small intestinal transit time in children.

The lactulose hydrogen breath-test provides a non-invasive measure of mouth to caecum transit-time. Its use, particularly in children has not been thoroughly investigated. We have studied the effects on transit time of altering the lactulose concentration of the test solution, and of administering it in a liquid nutrient preparation. Concentration markedly affected transit-time; studies with an isotonic solution may be least affected by delayed gastric emptying, and so may reflect small bowel transit-time. Day-to-day variation was large, and was not reduced by altering lactulose concentration,

Mechanisms underlying the antidiarrheal, antispasmodic and bronchodilator activities of Fumaria parviflora and involvement of tissue and species specificity.

ETHNOPHARMACOLOGICAL RELEVANCE: In the Greco-Arab (Unani) traditional medicine, Fumaria parviflora Linn. is widely used in hypreractive gut and respiratory disorders including diarrhea, abdominal cramps, indigestion and asthma but scientific studies to provide rational for these medicinal uses are sparse. This study was therefore undertaken to provide ethnopharmacological basis for its medicinal use in diarrhea, abdominal cramps and asthma. MATERIALS AND METHODS: The in vivo studies were conducted in mice and rats while isolated gut and tracheal preparations of rat, guinea-pig and rabbit were
