Feeding catheter gastrostomy with the round ligament of the liver prevents mechanical bowel obstruction after esophagectomy.

Jejunostomy, which requires the fixation of the jejunum to the abdominal wall, is commonly used as an enteral feeding access after esophagectomy. However, this procedure sometimes causes severe complications, such as mechanical bowel obstruction. In 2009, we developed a modified approach to insert an enteral feeding tube through the reconstructed gastric tube using the round ligament of the liver. The aim of this study is to investigate the usefulness of this approach as compared to the approach through jejunostomy. Between January 2005 and March 2015, 420 patients with thoracic esophageal can…

Biliary atresia--a 25-year survey.

In the past 25 years, from 1963 to 1988, 90 children from the Department of Pediatric Surgery of the Pediatric Hospital in Cologne, Germany were treated for biliary atresia. Of these, 47 had purely extrahepatic bile duct lesions, 21 had purely intrahepatic bile duct lesions, and 22 had both extrahepatic and intrahepatic bile duct lesions. Forty-five of the children underwent a drainage operation, whereas the remaining 45 children underwent no surgery at all or simply a diagnostic laparotomy. Until 1966 hepato-jejunostomy with implantation of artificial bile ducts was conducted in 12 cases. Lat…

[Percutaneous endoscopic gastrojejunostomy in the Guillain-Barré syndrome].

The nutritional support through an enteral route in patients diagnosed with Guillain-Barré syndrome, may be affected by the digestive alterations derived from the non-autonomic affliction. Even though the percutaneous endoscopic gastro-jejunostomy is still an infrequent technique in intensive-care units, its use in certain patients may ease the application of enteral nutrition. We present a case of Guillain-Barré syndrome, treated with enteral nutrition in an ICU during 100 days, 85 of which were by means of a gastro-jejunostomy tube, with good results.

Pancreas divisum: a teenager with calculi in the duct of Santorini. Report of a case treated by double drainage procedure.

Since the age of sixteen, a teenager suffers from acute recurrent pancreatitis with a weight loss of 17 kg, without evident etiologic factor. At laparotomy the duct of Wirsung is absent; calculi are found in a grossly dilated duct of Santorini and the minor papilla is stenosed with periductal fibrosis and a large retention pseudocyst. Recurrent pancreatitis is cured by sphincteroplasty of the minor papilla and side-to-side pancreatico-jejunostomy. After a follow-up of 17 months the recurrent pancreatitis seems cured by sphincteroplasty of the minor papilla and side-to-side pancreatico-jejunost…

[Cholestasis caused by chronic pancreatitis in childhood. Surgical treatment and genetic analysis].

Authors report two cases of childhood chronic pancreatitis, causing severe symptoms and common bile duct stenosis with cholestasis. Both patients had to be operated on. Chronic pancreatitis with calcification led to significant common bile duct stenosis in a 13 years old girl. After ERCP a double bypass procedure was performed (Wirsungo-jejunostomy and hepatico-jejunostomy). During 42 months follow-up the patient remained pain- and symptom-free gaining 16 kilograms. In a 9 years old girl severe stenosis of the intrapancreatic common bile duct and a small duct type chronic pancreatitis with ext…

[Recurrent neoplasm in the gastric stump, penetrating the pancreas. Total gastrectomy, total duodenopancreatectomy. Problems of surgical technic].

The authors performed a new surgical intervention in a patient with cancer of the gastric stump recidivating after a resection performed two years previously. Due to the extension of the tumour, and to involvement of the pancreas it was necessary to perform total gastrectomy, associated with splenectomy and total pancreatectomy. The refection of the continuity of the digestive-biliary pathways was achieved by eso-jejunostomy of the termino-terminal type associated with choledocho-jejunostomy on omega loop excluded from the digestive pathway by way of the Braun-type fistula. A knowledge of the …

General and Surgical Considerations for Enteral Access.

Durable enteral access with feeding tube placement is critical for patients expected to require enteral nutrition for an extended period. There are different options for enteral access, including gastrostomy tube and jejunostomy tube. Gastrostomy tubes are often preferred for feeding access when possible, as they are generally more physiologic and because the stomach is distensible and can accommodate a large volume, making intermittent bolus feeding possible, which is generally much easier for patient and provider management. Gastrostomy tubes may not be preferred in patients at risk of aspir…

Ileosigmoid knot complicated by gastric and splenic mucormycosis: a lethal combination.

Two very rare conditions, ileosigmoid knot and intra-abdominal mucormycosis, occurred sequentially in a middle-aged HIV positive male.At laparotomy for peritonitis, a gangrenous sigmoid volvulus with ileosigmoid knot was resected; 2 days later, colo-rectal continuity was restored, the distal ileum stapled and an end jejunostomy fashioned. At re-laparotomy on day 15, necrotic stomach and spleen required a total gastrectomy and splenectomy. At reoperation 2 days later, a bile leak at the oesophago-jejunostomy site was repaired. Despite this intervention he continued to deteriorate and died of mul…

Gastrostomy feeding versus oral feeding alone for children with cerebral palsy.

BACKGROUND: Children with cerebral palsy can be significantly disabled in terms of their ability to suck, chew and swallow. This can lead to significant impairment in feeding and, eventually, to undernutrition. It can also result in aspiration of food into the lungs. Length of feeding time may be considerably increased and, instead of being an enjoyable experience, mealtimes may be distressing for both child and carer. For children unable to maintain a normal nutritional state feeding by mouth, gastrostomy or jejunostomy tubes are increasingly being used to provide the digestive system with nu…

Intrajejunal feeding: development and current status.

Techniques of jejunostomy were established in surgical practice by the turn of the century. They were mainly used to administer normal food for the palliation of advanced gastric cancer. Standard postoperative intravenous fluid therapy did not begin in earnest until the late 1930's and did not become routine until the late 1940's because of pyrogens, fear of fluid overload, and commercial nonavailability. For most gastric procedures performed from 1900 until 1940, postoperative treatment consisted of nutrient and saline enemas and subcutaneous infusion of fluid. Jejunal feedings had their grea…

Pneumocephalus in a patient with a ventriculoperitoneal shunt after percutaneous gastrojejunostomy catheter placement: case report.

BACKGROUND: Percutaneous gastrostomy and/or jejunostomy associated with ventriculoperitoneal (VP) shunting in critically ill neurosurgical patients is not an uncommon combination. Massive intraventricular pneumocephalus has not been previously reported as a complication of percutaneous gastrostomy and/or jejunostomy placement in a patient with a VP shunt. A case is presented here where we believe such a complication occurred. CASE DESCRIPTION: Our patient is a 68-year-old woman who experienced a subarachnoid hemorrhage from a right anterior choroidal aneurysm rupture. The patient underwent en…

Evolving trends in enteral alimentation.

Nutrition has become a cornerstone of surgical patient care. With better understanding of metabolic and compositional requirements, great advances have been made in the area of total parenteral nutrition. Recent attention to full utilization of alimentary tract function has prompted a resurgence of interest in dietary formulas and methods of delivery. Three new approaches to the alimentary tract provide better access for feeding. Needle catheter jejunostomy allows early alimentary tract utilization following operations, while percutaneous endoscopic gastrostomy and jejunostomy provide long-ter…

[Digestive reconstruction using mechanical staplers after total gastrectomy].

From 1980 up to nowadays, after total gastrectomy for gastric cancer, the intestinal continuity was assured by oesophago-jejunostomy and oesophago-duodenoplasty using stapling devices. Two patients died for causes not related to the technique, and other two patients developed a stenosis of the oesophago-jejunostomy, which was easily managed by endoscopic dilatation. In one patient a partial dehiscence of the oesophago-jejuno-anastomosis was treated by total parenteral nutrition. The Authors have systematically adopted mechanical sutures after total gastrectomy. However, possible intraoperative…

Achieving Digestive Autonomy and Gastrointestinal Continuity in a Patient with Short Bowel Syndrome Secondary to Concomitant Jejunal Atresia and Small Intestinal Hirschsprung's Disease.

Concomitant presentation of jejunal atresia and Hirschsprung's disease is rare and places children at high risk for developing short bowel syndrome and parenteral nutrition dependence, which can affect the feasibility/timing of pull-through. A patient was born with jejunal atresia with a delayed diagnosis of Hirschsprung's disease. After several procedures and bowel resections, the patient was ultimately left with an end jejunostomy and long Hartman's pouch with short bowel syndrome, dependent on parenteral nutrition. The patient initially presented to our institution at age 2 with failure to …
