Current surgical options for keratoconus.

The treatment of keratoconus is partly dependent on the severity of the disease process. When contact lenses fail because the patient is intolerant or they do not provide sufficient acuity to meet the patient's needs, penetrating keratoplasty is the usual surgical modality for adequate visual rehabilitation. Keratoconus is considered a contraindication to incisional refractive surgery such as radial keratotomy and laser in situ keratomileusis because of the corneal instability conferred by these procedures. When the cornea is transparent, intrastromal corneal ring implantation is a refractive …

[Bilateral keratoconus: a case report].

Among corneal dystrophies, the keratoconus is one of the most frequently observed among young adults. A clinico pathological case is reported in a 13-year-old-girl of African origin. The diagnosis of bilateral keratoconus was established based on the obvious changes of the corneal curvature and thickness. After an unsuccessful attempt to improve vision with contact lenses, a keratoplasty was finally performed on one side to remove the pathological cornea. Its histopathological study found the characteristic changes of keratoconus: breaks of Bowman's layer and corneal thinning.

Keratoconus and mitral valve prolapse.

PURPOSE: To investigate the prevalence of keratoconus in patients with mitral valve prolapse (MVP). METHODS: Videokeratography was performed in 36 patients (72 eyes) with an echocardiographic diagnosis of MVP, and were compared with 25 controls. All participants underwent slit-lamp examination and completed a medical questionnaire about joint hypermobility, arthralgia, asthma, and allergic reactions. RESULTS: Keratoconus was found in eight eyes (11. 1%) (P =.056) of eight patients (22.2%) (P =.049) in the MVP group and in one eye (2%) of one patient (4%) in the control group. The keratoconus…

Keratocyte apoptosis associated with keratoconus.

Keratoconus is an ectatic corneal dystrophy associated with stromal thinning and disruption of Bowman's layer. The purpose of this study was to explore a possible association between keratocyte apoptosis and keratoconus. Keratocyte apoptosis was evaluated in corneas of patients with keratoconus, corneas of patients with stromal dystrophies, and normal donor corneas using the transferase-mediated dUTP-digoxigenin nick and labeling (TUNEL) assay. Keratocyte apoptosis was also studied in keratoconus and normal corneas using transmission electron microscopy. TUNEL-stained keratocytes were detected…

Circumscribed posterior keratoconus: case report.

BACKGROUND: Posterior keratoconus has only a few scattered case reports in the literature. Posterior keratoconus is characterized by a posterior stroma thinning and a depression of the posterior corneal surface. The effect on acuity is variable and may be related to other ocular and systemic conditions. CASE REPORT: An African-American woman came to us with posterior concavity (corneal thinning), with stromal scarring in both eyes and an epithelial iron ring present in the left eye. The endothelia layer appeared intact in both eyes. Corneal topography of the right eye demonstrated a central f…

RGP fitting philosophies for keratoconus.

Rigid gas permeable (RGP) lenses are the choice of vision correction for most keratoconic patients. Various fitting philosophies and different lens designs are available and the question of which technique is the most appropriate is a controversial issue in the field of contact lens practice. This article reviews the different philosophies with respect to their rationale, optical performance and physiological influence on the cornea. The 'three-point-touch' approach is now the most widely-accepted corneal lens fitting philosophy in clinical practice. It provides a well-distributed pressure bet

Excimer laser surgery for keratoconus.

PURPOSE: To determine whether there is increased risk associated with excimer laser surgery of primary keratoconus. SETTING: Department of Ophthalmology, County Council Hospital, Ryhov, Jönköping, Sweden. METHODS: Twenty-four eyes in 23 patients with keratoconus had photorefractive keratectomy (PRK) to reduce the steepness of the cone. A VISX Twenty-Twenty B laser (193 nm) was used for the treatments. Spherical ablations, cylindrical ablations, or both were used. Patients were followed for a mean of 22 months (range 6 to 46 months). RESULTS: Fourteen patients (58%) had improved visual acuit…

Clinical detection of unilateral keratoconus.

PURPOSE: To determine the incidence of true unilateral keratoconus on the basis of computerised corneal topography in a group with clinically diagnosed unilateral keratoconus. METHODS: Retrospective review of 295 patients with keratoconus identified 51 patients with a provisional diagnosis of unilateral keratoconus. Thirty-one of these patients were re-examined clinically and 21 patients met the criteria for a clinical diagnosis of unilateral keratoconus. Computerised corneal topographic analysis (Tomey TMS) was then performed. RESULTS: In a group of 21 patients with clinically diagnosed uni…

Ring corneal clouding in keratoconus.

Rings of corneal edema were noted in one eye of each of three keratoconic patients, all wearing McGuire design rigid contact lenses of limited oxygen transmissibility (Dk/La). This finding appears to be analogous to the previously reported "central circular clouding" (CCC) with this specific appearance associated with the particulars of the keratoconic cornea-contact lens relation. Estimation of local oxygen transmissibilities suggests that a Dk/L < 3 x 10(-9) may produce this gross clinical sign underneath the lens midperiphery, whereas central Dk/L > or = 6 x 10(-9) may be sufficient to prev

Acute corneal hydrops in keratoconus.

PURPOSE: To determine the clinical factors associated with the development of acute corneal hydrops and its subsequent outcome. METHODS: The authors identified 147 eyes (124 patients) with acute hydrops from a database of 5242 eyes (2723 patients) with keratoconus. They compared the clinical features of patients in whom hydrops developed with unaffected patients and assessed the progression to penetrating keratoplasty by actuarial methods. RESULTS: Patients in whom acute hydrops developed tended to be younger males who had advanced corneal ectasia and a poor corrected Snellen visual acuity a…

Bilateral keratoconus in Crouzon's syndrome.

Crouzon's syndrome is an autosomal dominant disorder characterized by premature craniosynostosis leading to multiple ocular abnormalities and vision loss. Strabismus, optic atrophy, exposure keratitis, hypertelorism, nystagmus, and unexplained visual loss have been reported. Keratoconus has been described only once previously in a patient with other ocular abnormalities. This article reports bilateral keratoconus in a patient with Crouzon's syndrome without other ocular pathology. We emphasize the importance of early recognition and therapy of this treatable cause of decreased vision in Crouzo…

Corneal topography of posterior keratoconus.

Posterior keratoconus is an unusual abnormality of the cornea generally classified as one of the anterior chamber cleavage anomalies. It is characterized clinically by the presence of a circumscribed or generalized corneal thinning with posterior depression of the cornea and is considered distinct from keratoconus. Although patients with posterior keratoconus may have visual complaints clearly related to their abnormal corneas, the surface topography of these corneas has not been studied in detail. Keratometry and photokeratoscopy provide an incomplete picture of the surface geometry of poster…
