Newborn screening for G6PD deficiency--why is it important for India?

Neonatal screening, the most important preventive public health programme of the 21st century, is implemented in majority of the developed countries. The Asia-Pacific region has a long history-the late Emeritus Professor Wong Hock Boon in 1965 initiated cord blood G6PD screening in Singapore, which virtually eliminated kernicterus. In India currently there is no government funded neonatal screening programme for the masses, but most private hospitals have started screening for disorders which the pediatrician there thinks is relevant in that part of the country. Neonatal jaundice in Asia.

Neonatal jaundice is a major clinical problem globally, especially in the Asian and south-east Asian regions. There is no universal definition of hyperbilirubinaemia, and comparisons of management and control of hyperbilirubinaemia in infants at different centres are difficult. G6PD deficiency, ABO incompatibility, low birth weight and sepsis are the common causes of neonatal jaundice, but there is a group of babies whose cause of neonatal jaundice has yet to be found. Genetic factors may be responsible for ethnic differences in the ability to eliminate bilirubin, while unidentified environmen

Does hyperbilirubinemia damage the brain of healthy full-term infants?

In the 1950s, exchange transfusion to keep the total serum bilirubin below 20 mg/dl was shown to be an effective way of preventing kernicterus in babies with erythroblastosis fetalis. For the last 15 to 20 years this level has also been used to determine the need for intervention in healthy full-term infants who do not have hemolytic disease. A critical review of all the available data including six studies from the collaborative perinatal project (more than 30,000 infants) and several smaller studies of term infants without hemolysis reveals essentially no evidence of adverse effects of bilir

Hematin and bilirubin binding to human serum albumin and newborn serum.

In jaundiced newborn infants, hemolytic disease is considered a risk factor for kernicterus due to the suspected competition between bilirubin and other hemoglobin breakdown products for albumin binding. We have studied the effect of hematin on bilirubin-albumin binding using the peroxidase assay and a light-scattering technique for measuring unbound bilirubin. Our results show that hematin does not affect bilirubin-albumin binding. To determine if other albumin binding functions are affected by hematin, we used a microdialysis rate technique employing two ligands, diazepam and monoacetyldiami

[Hemolytic disease of the newborn due to ABO incompatibility (author's transl)].

Up to now, the exact diagnosis of ABO hemolytic disease of the newborn cannot be made either clinically or serologically. Affected babies are almost all either group A or group B from mothers of group O. In contrast to Rh hemolytic disease the immunological findings do not correlate well with the severity of the clinical course. Sometimes, it is impossible to differentiate between ABO hemolytic disease and non-antibody mediated hyperbilirubinemia. Pathogenetic aspects are discussed which may explain the differences between ABO and Rh hemolytic disease of the newborn. Aids to immunological and

Reserve albumin binding capacity, salicylate saturation index, and red cell binding of bilirubin in neonatal jaundice.

105 blood samples from 72 infants, mostly with jaundice due to haemolytic disease, were analysed for reserve albumin binding capacity (HBABA method), salicylate saturation index (SI), and red cell binding of bilirubin. 2 infants with clinical symptoms of bilirubin encephalopathy had abnormally large amounts of red cell bound bilirubin, though the HBABA binding capacity and salicylate saturation index did not suggest a risk of bilirubin encephalopathy. On the other hand, 48 of the other samples showed `risk values' for saturation index and 2 of the other samples showed such values as judged by

Hemolytic disease of the fetus and newborn due to minor blood group alloimmunization in a mother of sickle cell disease with multiple alloantibodies.

Hemolytic disease of the fetus and newborn is due to maternal IgG antibodies that transport through the placenta and destroy neonatal red cells. A mismatch of antigens between mother and fetus causes isoimmunization resulting in mild anemia, which may progress to fetal hydrops in the intrauterine period and severe hyperbilirubinemia to kernicterus in neonates. The isoimmunization is mainly caused by Rh-D and ABO antibodies. In this case report, we found neonatal hyperbilirubinemia due to the presence of anti-c alloantibody previously developed in a sickle cell disease (SCD) pregnant female. It

Crigler-Najjar type II in pregnancy: A case report.

Crigler-Najjar is a rare, autosomal recessive disorder that results in mutations causing a complete absence (type I) or deficiency (type II) of the hepatic uridine diphospho-glucuronosyl transferase (UDPGT) enzyme. Both forms, however, result in unconjugated hyperbilirubinaemia which can lead to kernicterus and potentially death. Phenobarbitone can be used as an enzyme inducer in Type II to facilitate a reduction in total serum bilirubin. We report two consecutive pregnancies in a 29-year-old woman with Crigler-Najjar Type II syndrome. Phenobarbitone therapy was commenced in the first pregnanc

Management strategies of neonatal jaundice during the coronavirus disease 2019 outbreak.

The outbreak of coronavirus disease 2019 (COVID-19; formally known as 2019-nCoV) has become a most challenging health emergency. Owing to rigorous quarantine and control measures taken in China, routine neonatal health surveillance and follow-up have become challenging. Without follow-up surveillance, some rapid and progressive newborn diseases, such as bilirubin encephalopathy, may be ignored. The characteristics of onset age of kernicterus suggest that monitoring of bilirubin level at home provides a useful way to alert hospital visits and to prevent the development of extremely hyperbilirub

Rhesus disease: a global prevention strategy.

After nearly five decades of effective prophylaxis in high-income countries, the incidence of rhesus haemolytic disease (also known as haemolytic disease of the fetus and newborn) has substantially decreased, and as a result, clinical experience of the disease among health-care providers is insufficient. By contrast, a worldwide study found that rhesus haemolytic disease continues to be a public health problem in low-income and middle-income countries, affecting annually in more than 150 000 children, and causing thousands of stillbirths, neonatal deaths, and cases of hyperbilirubinaemia with

Immunizations, neonatal jaundice, and animal-induced injuries.

This article reviews recent advances and recommendation changes in the areas of immunization, neonatal hyperbilirubinemia, and animal-induced injury. Physician education of families and care of patients in these three topic areas are important public health measures. First, pediatricians are always actively promoting public health through immunization, so the 2003 vaccination recommendations are critiqued along with perceived barriers to proper immunization. New information about smallpox vaccine is included in this year's review because of increased concerns about the use of smallpox as a wea

Neuropathology of the dentate nucleus in developmental disorders.

The dentate nucleus was examined histologically and immunohistochemically in 47 cases of nonprogressive developmental disorders. Neuronal loss and/or atrophy was observed in 13 cases, while mild neuronal lesions, characterized by dendritic swelling and/or the appearance of eosinophilic materials around the neurons, were exhibited in 20 cases. The former change was accompanied by diffuse central nervous system involvement, and the etiology was perinatal hypoxic ischemic encephalopathy, acute encephalopathy, and meningoencephalitis in most cases. On the other hand, most of the patients with kern

Acute bilirubin encephalopathy: Some lessons learned.

Acute bilirubin encephalopathy (ABE) is still an insufficiently addressed cause of mortality and long-term morbidity in low- and middle-income countries (LMICs). This article highlights that delayed or incorrect medical advice, inaccurate bilirubin measurements as well as ineffective phototherapy are some of the relevant causes predisposing jaundiced newborns to develop extreme hyperbilirubinemia [EHB, total serum/plasma bilirubin (TB) ≥ 25 mg/dL (428 µmol/L)] and subsequent ABE. Obstacles preventing state of the art management of such infants are also discussed. Prevention of ABE cannot occur

Management of hyperbilirubinemia in preterm infants in Turkey.

BACKGROUND/AIM: Prematurity is a significant risk factor for developing unconjugated hyperbilirubinemia. This study investigated the current approach to managing hyperbilirubinemia in preterm newborns in Turkey. MATERIALS AND METHODS: A study-specific questionnaire on the management of jaundice in preterm infants was sent to 100 level III neonatal intensive care units in Turkey. RESULTS: Responses were obtained from 84 centers from all regions of Turkey. Of the centers, 75.3% used the Turkish Neonatology Society guidelines for deciding to start phototherapy, and 24.7% used different guidelin

[Not Available].

This is the clinical history of a term baby born at home who presents a severe hyperbilirubinémia. The medical monitoring was assessed by a private midwife according to parental choice. On the third day of life, the newborn presented an icterus and was exposed to natural daylight in the familial greenhouse under the midwife recommandations. On that day, no laboratory test precised the bilirubin level. On the fifth day, a blood sampling revealed a very high blood bilirubinémia (31 mg/dl or 527 mmol/L), the baby is refered to our NICU and underwent an exchange transfusion. The radiological asses

Impact of Rhesus disease on the global problem of bilirubin-induced neurologic dysfunction.

Clinical experience with Rhesus (Rh) disease and its post-icteric sequelae is limited among high-income countries because of nearly over four decades of effective prevention care. We hypothesized that Rh disease is prevalent in other regions of the world because it is likely that protection is limited or non-existent. Following a worldwide study, it has been concluded that Rh hemolytic disease is a significant public health problem resulting in stillbirths and neonatal deaths, and is a major cause of severe hyperbilirubinemia with its sequelae, kernicterus and bilirubin-induced neurologic dysf

Toward understanding the connections between infant jaundice and infant feeding.

Parents face a paradox when they are told: Breast is best; bottle-feeding is hazardous to health. But breast-fed babies are more likely to become severely jaundiced than bottle-fed babies, and severe jaundice can lead to brain damage. This article will explore the natural physiology of jaundice with a focus on breast-feeding-associated jaundice, primary prevention of hyperbilirubinemia, and current evidence-based recommendations about feeding jaundice breast-fed infants.

The effect of instituting a prehospital-discharge newborn bilirubin screening program in an 18-hospital health system.

OBJECTIVE: Kernicterus is a rare but devastating condition. The prevention of bilirubin-induced brain injury is based on the detection of infants at risk for developing severe hyperbilirubinemia. In an 18-hospital health system, Intermountain Health Care (IHC), we initiated a program of predischarge bilirubin screening of all neonates and coupled this with a results assessment using a percentile-based nomogram. Data during 2 periods of time, before versus after initiating the program, were compared to assess the effect of the program on significant hyperbilirubinemia and rehospitalization. ME
