A Biomechanical Comparison of the Arciero and LaPrade Reconstruction for Posterolateral Corner Knee Injuries.

BACKGROUND: Injury to the posterolateral corner (PLC) of the knee requires reconstruction to restore coronal and rotary stability. Two commonly used procedures are the Arciero reconstruction technique (ART) and the LaPrade reconstruction technique (LRT). To the authors' knowledge, these techniques have not been biomechanically compared against one another. PURPOSE: To identify if one of these reconstruction techniques better restores stability to a PLC-deficient knee and if concomitant injury to the proximal tibiofibular joint or anterior cruciate ligament affects these results. STUDY DESIGN

Acute patellar dislocation with multiple ligament injuries after knee dislocation and single session reconstruction.

Knee dislocation is a relatively rare condition of all orthopaedic injuries. Accompanying multiple ligament injuries are common after knee dislocations. A 41-year-old male presented to the emergency department suffering from right knee dislocation in June 2013. The patient had anterior cruciate ligament, medial collateral ligament (MCL), medial patellofemoral ligament (MPFL) rupture, and lateral meniscal tear. A single-bundle anatomic reconstruction, medial collateral ligament reconstruction, medial patellofemoral ligament reconstruction and meniscus repair were performed in single session. At

High-volume image-guided injection for recalcitrant medial collateral ligament injuries of the knee.

AIM: To evaluate the effectiveness of a novel injection technique in the management of recalcitrant medial collateral ligament (MCL) injuries of the knee. MATERIALS AND METHODS: The injection, comprising 10 ml local anaesthetic with 25-50 mg hydrocortisone, is directed beneath the periosteal attachment of the MCL. Twenty-eight patients who received the intervention were asked to complete a questionnaire, a visual analogue scale (VAS) and the International Knee Documentation Committee (IKDC) subjective knee form to quantify symptoms pre-injection and at follow-up. Data were assessed using desc

How severe are initially undetected injuries to the knee accompanying a femoral shaft fracture?

BACKGROUND: Fractures of the femur are severe injuries that quickly attract the physician's attention. Previous reports have shown that injuries to the ipsilateral knee can occur. In most cases, such injuries were diagnosed on delay. Excluding cases in which a knee injury was apparent already at admission, we sought to investigate the number and severity of initially undetected lesions to the knee concomitant with a femoral shaft fracture and give an overview of the literature referring to these combined injuries. METHODS: Charts and X-rays of patients treated for a femoral shaft fracture fro

Magnetic resonance imaging for the evaluation of acute posterolateral complex injuries of the knee.

Clinical evaluation of posterolateral complex injuries of the knee can be difficult. To determine if magnetic resonance imaging can assist in decision-making in the treatment of posterolateral complex injuries, six consecutive patients with acute posterolateral knee trauma were imaged preoperatively with standard magnetic resonance imaging sequences. Magnetic resonance imaging findings were then correlated with results of examination under anesthesia or open lateral reconstruction. There were five complete lateral complex injuries (grade III) and one partial tear. Magnetic resonance imaging wa

The role of arteriography in assessing vascular injuries associated with dislocations of the knee.

All patients with a diagnosis of knee dislocation during a 7 1/2-year period (1984-1991) were reviewed retrospectively. There were 37 knee dislocations (KDs) in 35 patients. The mechanism of injury was predominantly motor vehicle or pedestrian crashes. Popliteal artery injury was present in 6 of 37 (16%). There was no significant vascular injury in 31 of 37 knee dislocations as evidenced by normal arteriograms or absence of vascular complications on follow-up. Of those with no significant vascular injury, 3 of 31 had limb ischemia that improved with reduction of the KD, had normal arteriograms

Factors predisposing Army conscripts to knee exertion injuries incurred in a physical training program.

Factors causing predisposition to knee exertion injuries were sought by means of a questionnaire and detailed quantitative physical measurements in 62 healthy Finnish Army conscripts. The subjects consisted of two groups of 18- to 26-year-old conscripts; Group A comprised 34 subjects without knee exertion injuries and Group B, 28 subjects who experienced knee exertion injuries during the first eight weeks of military service. In Group B the extensor mechanism was the most common source of symptoms. The factors found in Group B that differed significantly from those in Group A included increase

Time from Injury Is the Key Predictor of Meniscal Injury in ACL-Deficient Knees.

The meniscus is one of the most important structures of the knee that needs to be saved if possible. Previous studies showed that increasing time from an anterior cruciate ligament (ACL) injury (TFI) could lead to a meniscal tear, especially medial meniscus (MM). We developed the present study to see if the TFI alone is a predictor of meniscal injury in ACL-deficient knees. We included 111 patients who had a reconstructed ACL injury at our institution from March 2015 to March 2016 in this retrospective cohort study. All demographic data, including age, gender, and body mass index (BMI), were c

Nerve injury complicating multiligament knee injury: current concepts and treatment algorithm.

Multiligament knee injuries account for <0.02% of all orthopaedic injuries, and 16% to 40% of these patients suffer associated injury to the common peroneal nerve (CPN). The proximity of the CPN to the proximal fibula predisposes the nerve to injury during local trauma and dislocation; the nerve is highly vulnerable to stretch injury during varus stress, particularly in posterolateral corner injuries. CPN injuries have a poor prognosis compared with that of other peripheral nerve injuries. Management is determined based on the severity and location of nerve injury, timing of presentation, asso…

Hoffa's Fracture with Associated Injuries Around the Knee Joint: An Approach to a Rare Injury.

Fractures of the distal femur typically occur in the axial and sagittal planes. A Hoffa fracture is a coronal plane fracture of the femoral condyle, which accounts for 8.7% to 13% of distal femoral fractures. It is usually associated with other injuries around the knee joint and hence is often missed. We conducted a comprehensive systematic review of papers published in the English language using PubMed, Web of Science, Scopus, and the Cochrane Database, which reported Hoffa's fracture associated with other injuries around the knee joint. We selected 11 eligible papers for final analysis and r

Injuries to the posterolateral aspect of the knee. Association of anatomic injury patterns with clinical instability.

Seventy-one consecutive patients with posterolateral knee injuries had clinical stability testing abnormalities documented prospectively. We compared these findings with the incidence and patterns of their injuries documented at surgery. An abnormal reverse pivot shift test was associated with injury to the fibular collateral ligament (P = 0.01), popliteal components (P = 0.01), and midthird lateral capsular ligament (P = 0.02). An abnormal posterolateral external-rotation test at 30 degrees of flexion was associated with injury to the fibular collateral ligament (P = 0.0001) and lateral gastr…

Knee Kinematics During Landing: Is It Really a Predictor of Acute Noncontact Knee Injuries in Athletes? A Systematic Review and Meta-analysis.

BACKGROUND: Although knee kinematics during landing tasks has traditionally been considered to predict noncontact knee injuries, the predictive association between noncontact knee injuries and kinematic and kinetic variables remains unclear. PURPOSE: To systematically review the association between kinematic and kinetic variables from biomechanical evaluation during landing tasks and subsequent acute noncontact knee injuries in athletes. STUDY DESIGN: Systematic review; Level of evidence, 2. METHODS: Databases used for searches were MEDLINE, LILACS, IBECS, CINAHL, SPORTDiscus, SCIELO, IME, …

Heterotopic ossification around the knee after tibial nailing and ipsilateral antegrade and retrograde femoral nailing in the treatment of floating knee injuries.

PURPOSE: Floating knee injuries are relatively uncommon injuries. We report the prevalence, location, and severity of heterotopic ossification (HO) around the knee in patients treated with antegrade tibial intramedullary nailing and ipsilateral antegrade versus retrograde femoral intramedullary nailing as well as how the severity of HO around the knee affects knee range of motion (ROM). METHODS: From 2004 to 2014, 26 floating knee injuries were included. Radiographs were reviewed to determine presence, location, and severity of HO. Post-operative knee ROM was determined. RESULTS: A significa…
