Effects of an intervention program for reducing severe perineal trauma during the second stage of labor.

BACKGROUND: Obstetric anal sphincter injuries lead frequently to short- and long-term consequences for the mother, including perineal pain, genital prolapse, and sexual problems. The aim of the study was to evaluate whether the implementation of an intervention program in the second stage of labor involving altered perineal support techniques reduced severe perineal trauma. METHODS: All women reaching the second stage of labor and giving birth vaginally to singleton babies at Landspítali University Hospital (comprising 76% of births in Iceland in 2013) were enrolled in a cohort study. Data we

Birth experiences in adult women with a history of childhood sexual abuse.

OBJECTIVE: Although childhood sexual abuse (CSA) may seriously impair childbirth experiences, few systematic evaluations on associations, mediating influences, risk and protective factors are available. As such information is mandatory to improve obstetric care, the present study aimed to provide such data. METHODS: The study compared childbirth experiences from 85 women after CSA and at least one pregnancy resulting in a life birth with those from 170 control women matched for nationality, personal age and children's age. Trained specialists from support centers investigated CSA. Obstetrical

Amniotic fluid embolism: prophylaxis with heparin and aspirin.

The thromboplastic activity of amniotic fluid (AF) from women in labor may be a significant factor in amniotic fluid embolism (AFE) and obstetric disseminated intravascular coagulation (DIC). Infusion of AF from women in labor into laboratory animals was found to depress the platelet count, as seen in DIC. In this study we also investigated the ability of heparin and aspirin to block the thromboplastic activity and thrombocytopenia subsequent to AF infusion. In animals, heparin prophylaxis before infusion of AF prevented thrombocytopenia, whereas aspirin prophylaxis did not. Heparin prophylaxi

Pregnancy outcome following non-obstetric surgical intervention.

OBJECTIVE: To evaluate the effects of non-obstetric surgical procedures on maternal and fetal outcome. METHODS: A systematic review of all English language literature. RESULTS: Fifty-four papers met the inclusion criteria. The overall number of patients reported was 12,452. Reported maternal death was rare at .006%. The miscarriage rate was 5.8%; however, this number is difficult to interpret since matched controls were not available. The rate of elective termination of pregnancy following non-obstetric surgery was 1.3%. The rate of premature labor induced by non-obstetric surgical intervent

Monocytes at the crossroads of inflammation and labor: biomarker potential in prolonged deliveries - a narrative review.

Prolonged labor is a significant obstetric complication, often leading to adverse maternal and neonatal outcomes. Recent research has focused on the role of the immune system, particularly monocytes, in the pathophysiology of prolonged labor. Monocytes contribute to the inflammatory processes that govern cervical ripening, uterine contractility, and labor progression. However, dysregulation in monocyte activation and cytokine release can impair these processes, leading to labor complications. This review examines the role of monocytes as biomarkers in prolonged labor, emphasizing their involve

Trends in pain relief in labour: implications for obstetric analgesia service in Nigeria.

Labour and delivery result in severe pain for most women. Attention to comfort and analgesia for women in labour is important for physiological reasons and out of compassion. A review of common methods of pain relief of labour was done. Inhalation method as well as intravenous administration of opioids for pain relief in labour is fast giving way to lumbar epidural analgesia. The use of local anaesthetic in labour offers superior pain relief, is effective and safe. The inhalation and parenteral routes seem reserved for patients with contraindication to insertion of epidural. The administration

The effect of antenatal hypnosis training on pharmacological analgesia use during labour and birth: A systematic review and meta-analysis.

BACKGROUND: The use of hypnosis as a means of pain management during labour is becoming increasingly popular. While recent reviews have reported on pain perception, relaxation and other psychological benefits the impact of hypnosis on the use of pharmacological analgesia use has not been specifically examined. QUESTION: For women in labour at term, does antenatal hypnosis instruction compared to no instruction result in decreased use of pharmacological analgesia and influence maternal and infant birth outcomes. METHODS: Databases such as PubMed, CINAHL, Cochrane Central Register of Controlle

Epidural-related maternal fever: incidence, pathophysiology, outcomes, and management.

Epidural-related maternal fever affects 15% to 25% of patients who receive a labor epidural. Two meta-analyses demonstrated that epidural-related maternal fever is a clinical phenomenon, which is unlikely to be caused by selection bias. All commonly used neuraxial techniques, local anesthetics with or without opioids, and maintenance regimens are associated with epidural-related maternal fever, however, the impact of each component is unknown. Two major theories surrounding epidural-related maternal fever development have been proposed. First, labor epidural analgesia may lead to the developme

Analgesic considerations for induction of labor.

Induction of labor may be indicated to minimize maternal and fetal risks. The rate of induction is likely to increase as recent evidence supports elective induction at 39 weeks gestation. We review methods of induction and then analgesic options as they relate to indications and methods to induce labor. We specifically focus on parturients at high risk for anesthetic complications including those requiring anticoagulation, and those with cardiac disease, obesity, chorioamnionitis, prior spinal instrumentation, elevated intracranial pressure, known or anticipated difficult airway, thrombocytope

Understanding preterm labor.

Increased uterine contractility at term and preterm results from activation and then stimulation of the myometrium. Activation can be provoked by mechanical stretch of the uterus and by an endocrine pathway resulting from increased activity of the fetal hypothalamic-pituitary-adrenal (HPA) axis. In fetal sheep, increased cortisol output during pregnancy regulates prostaglandin H synthase type 2 (PGHS2) expression in the placenta in an estrogen-independent manner, resulting in increased levels of PGE2 in the fetal circulation. Later increases in maternal uterine expresssion of PGHS2 require ele

Anesthetic Challenges of Labor and Delivery in a Patient With Catecholaminergic Polymorphic Ventricular Tachycardia and Left Ventricular Non-Compaction Cardiomyopathy.

Catecholaminergic Polymorphic Ventricular Tachycardia (CPVT) and Left Ventricular Non-Compaction Cardiomyopathy (LVNC) are inherited disorders that pose significant challenges in the obstetric population due to the potential exacerbation of ventricular arrhythmias and potentially lethal cardiac compromise during pregnancy and labor. This case report aims to provide insights into the anesthetic management of CPVT and LVNC in obstetric patients. Goals of management should include careful optimization of anesthesia and analgesia, particularly during the birthing process. The choice of anesthetic

Epidural Anesthesia in a Patient With Turner Syndrome: A Case Report.

Turner syndrome (TS) is associated with short stature, cardiac abnormalities, and musculoskeletal deformity, which pose unique obstetric anesthesia challenges in the small proportion of TS patients who go through spontaneous or assisted pregnancy. There is very limited data available on the anesthetic management of these patients. This case report presents the management of a 32-year-old gravida 2 para 0 (G2P0010) patient with mosaic TS undergoing labor induction at 37 weeks of gestation. An indwelling epidural catheter placed under ultrasound guidance provided adequate labor analgesia with ab
