Microsurgical ear replantation without venous repair: failure of development of venous channels despite patency of arterial anastomosis for 14 days.

The authors describe a case of microvascular ear replantation with repair of the artery only and medicinal leech therapy that survived for 14 days but ultimately failed as a result of the absence of development of venous channels between the replant and the recipient bed. A 35-year-old man presented with complete avulsion of 80% of the right external ear. The auricle was revascularized successfully via transposition of the superficial temporal artery (STA) and end-to-end anastomosis between the STA and an identified arterial branch on the posterior surface of the ear, using the technique of lo

Hirudotherapy for neonatal limb ischemia during ECMO support: A word of caution.

INTRODUCTION: Disordered coagulation, clot formation and distal limb ischemia are complications of extracorporeal membrane oxygenation (ECMO) with significant morbidity and mortality. Medicinal leech therapy (hirudotherapy) has been attempted in plastic and orthopedic surgeries to improve venous congestion and salvage ischemic flaps. To our knowledge this has not been reported in pediatric cardiac surgery or during ECMO support. We present a complex neonate whose ECMO course was complicated by distal limb ischemia for whom leech therapy was attempted. PATIENT AND INTERVENTION: A 2-week-old 2.

Detailed ultrastructure of the Hirudo (Annelida: Hirudinea) salivary gland.

Medical leeches have been widely used in medical applications and treatments for millennia. Studies on the salivary glands of blood-sucking leeches have focused on their bioactive secretions and mechanisms of action, with little attention to ultrastructure. In this study, we examined dissected embryonic and adult Hirudo verbana salivary glands by scanning electron microscopy (SEM). Gland cells of embryos were physically separated while adults displayed highly developed cell bunches in which each cell was connected to others by fine channels. Channels from each bunch combined to form a larger c…

Lichen Planus Due to Hirudotherapy.

Lichen planus is a traumatic (koebner positive), chronic, inflammatory and autoimmune disease affecting the oral and genital mucosa, scalp and nails. The Food and Drug Administration approved the use of medical leeches for therapeutic purposes (hirudotherapy) in 2004 to ensure flap nutrition in plastic surgery. A 34-year-old male patient was admitted to our dermatology outpatient clinic with a swollen, itchy and purple-coloured rash on legs and back for a month, and white and reticulated plaques in the mouth. It was learned that a week earlier, eight leeches was applied to both knees and ankle…

Spatio-temporal variation and the use of host body surface by ectoparasites of the chelonians Phrynops geoffroanus and Mesoclemmys tuberculata in areas of the Caatinga and Atlantic Forest in northeast Brazil.

Ectoparasites such as hematophagous leeches and monogeneans are common in chelonians, occupying different parts of the body. Thus, the present study aimed to identify and describe the fauna of ectoparasites that infest Phrynops geoffroanus and Mesoclemmys tuberculata to evaluate the effect of host conditions and seasonality (dry and rainy season) on the abundance and composition of ectoparasites. We verified the presence of ectoparasites in 73.2% of the examined turtles, with four species of leeches belonging to Glossiphoniidae, Haementeria brasiliensis sensu Cordero, 1937, Helobdella cf. adia…

Nasopharyngeal hirudiniasis, a rare cause of masked hemoptysis in an older patient: A case report.

Leeches commonly inhabit freshwater environments and feed on blood; however, pharyngeal leech infestation in humans remains an unusual cause of hemoptysis that poses significant diagnostic challenges. We report the case of a male patient in his early 60s presenting with a 2-week history of persistent hemoptysis and pharyngeal discomfort. Physical examination revealed a transient, mobile structure behind the uvula that initially mimicked a blood clot. Flexible nasopharyngoscopy subsequently demonstrated a moving foreign body attached to the nasopharyngeal wall. The organism, identified as a 3-c…

Laryngeal hirudiniasis: an unusual cause of airway obstruction and hemoptysis.

Cases of childhood hemoptysis are rare and usually result from foreign body aspiration or congenital heart or lung diseases. However, human hirudiniasis due to the leech still exists, and could involve the upper airways after drinking infested water from quiet streams and pools. We report the case of a 6-year-old child who presented suffocating at the emergency room after having been misdiagnosed and treated for asthma over a 1-month period. His mother reported he had had recurrent hemoptysis, as well. The child inadvertently drank leech-infested water in a rural area of northern Syria. Surgic

A hormone-activated central pattern generator for courtship.

BACKGROUND: Medicinal leeches (Hirudo spp.) are simultaneous hermaphrodites. Mating occurs after a stereotyped twisting and oral exploration that result in the alignment of the male and/or female gonopores of one leech with the complementary gonopores of a partner. The neural basis of this behavior is presently unknown and currently impossible to study directly because electrophysiological recording techniques disrupt the behavior. RESULTS: Here we report that (Arg(8))-conopressin G and two other members of the oxytocin/vasopressin family of peptide hormones induce in Hirudo verbana a sequenc…

Anticoagulant peptide LR from Whitmania pigra ameliorates cerebral ischemia-reperfusion injury.

BACKGROUND: Ischemic stroke (IS) is a leading cause of death and disability worldwide. Hypercoagulability and thrombus formation play critical roles in its pathogenesis. Leech-derived peptides, especially those obtained from Whitmania pigra, have demonstrated potential antithrombotic and neuroprotective effects. This study aimed to investigate the therapeutic efficacy and underlying mechanisms of a leech peptide in a rat model of cerebral ischemia/reperfusion injury. METHODS: Adult male Sprague-Dawley rats were subjected to middle cerebral artery occlusion followed by reperfusion to establish

How parasite exposure and time interact to determine Australapatemon burti (Trematoda: Digenea) infections in second intermediate hosts (Erpobdella microstoma) (Hirudinea: Erpodellidae).

Australapatemon spp. are cosmopolitan trematodes that infect freshwater snails, aquatic leeches, and birds. Despite their broad geographic distribution, relatively little is known about interactions between Australapatemon spp. and their leech hosts, particularly under experimental conditions and in natural settings. We used experimental exposures to determine how Australapatemon burti cercariae dosage (number administered to leech hosts, Erpobdella microstoma) affected infection success (fraction to encyst as metacercariae), infection abundance, host survival, and host size over the 100 days …

Tracheal Hirudiniasis Induced by Ingestion of Unboiled Mountain Spring Water.

Leeches, which are bloodsucking annelids widely distributed in natural environments globally, can cause hirudiniasis when they invade the human body. The most common site of infestation in humans is the nasopharynx, although they can also infiltrate less common sites, such as the trachea, esophagus, and urinary and reproductive tracts. We present the case of a 47-year-old male patient with a persistent cough for 2 months associated with hemoptysis and intermittent hoarseness for 1 month. Nasopharyngoscopy revealed an actively moving worm located above the vocal cords. Subsequently, under intra…

Unusual Cause of Persistent Epistaxis with Severe Anemia in a Child.

Epistaxis in children can be caused by different systemic and local pathologies. Respiratory infections, nasal mucosa dryness, and foreign bodies are some local causes of bleeding from the nose. In developing countries, infestations still contribute a significant proportion of anemia in children. But it is very unusual for leech-causing persistent epistaxis with a consequence of severe anemia. We herein report a rare cause of severe anemia in a 5-year-old child presented to our clinic for persistent epistaxis. A leech was taken out with forceps, and his anemia was treated accordingly.

Adenoid cystic carcinoma of the nasal cavity and paranasal sinuses: a meta-analysis.

Treatment of penile haematomas includes conventional therapy and/or surgical intervention which may not always result in the recovery of all penile functions. For valuable breeding stallions, a safe and effective treatment is necessary and medical leech therapy meets these criteria. Additionally, the proven efficiency and safety of the use of leeches in the resolution of haematomas makes this method applicable to other fields of veterinary medicine. A 14-year-old stallion was presented and assigned to the university hospital with a swollen and prolapsed penis. The owner reported that the stall…

[Hirudin therapy and blood coagulability].

The changes in blood coagulative properties, which occur in the treatment with medical leeches, were studied in 57 patients with coronary heart, rheumatic and chronic pulmonary diseases. Hirudin therapy was established to affect blood coagulation. The leech salina penetrated into the skin wound to block hemostasis. Having entered the circulating blood, some part of the saliva normalized its impaired coagulative properties. The correction of the above abnormalities and related bleeding indicate it beneficial to use hirudin therapy in secondary coagulopathies and circulatory decompensation.

Bitter substances suppress afferent responses to an appetitive mixture: evidence for peripheral integration of chemosensory stimuli.

The processes that lead from detection of chemicals, transduction, and coding with the appropriate message to initiate ingestion of a palatable meal or to reject a potentially noxious substance are poorly understood in vertebrates owing to the complex organization of the taste system. As a first step in elucidating the cellular basis of the behavioral differences elicited by appetitive stimuli and bitter compounds, we recorded from the afferent nerves conveying peripheral chemosensory information to the CNS in the head of the leech, Hirudo medicinalis. Superfusion of the chemosensory region of

Botanical repellents and pesticides traditionally used against hematophagous invertebrates in Lao People's Democratic Republic: a comparative study of plants used in 66 villages.

Hematophagous parasites such as leeches, ticks, mites, lice, bedbugs, mosquitoes, and myiasis-producing fly larvae are common health problems in Lao People's Democratic Republic. Several arthropod-borne infections, e.g., malaria, dengue fever, and Japanese encephalitis, are endemic there. Effective vector control methods including the use of pesticides, insecticide-treated bed nets, and synthetic and plant-based repellents are important means of control against such invertebrates and the pathogens they may transmit or directly cause. In this study, we documented traditional knowledge on plants…
