Factors associated with boarding and length of stay for pediatric mental health emergency visits.

OBJECTIVE: To determine demographic and clinical risk factors associated with boarding (length of stay ≥24 h) for pediatric mental health emergency department (ED) visits. METHODS: This is a retrospective cross-sectional analysis of mental health visits identified by diagnosis codes for children 5-18 years old presenting to a tertiary pediatric ED in 2016. We performed multivariate logistic regression to identify demographic and clinical factors associated with boarding. RESULTS: There were 1746 mental health visits and 386 (22%) visits had length of stay ≥24 h. In the multivariate logistic …

Relationship Between Handgrip Strength and Length of Stay for Left Ventricular Assist Device Implantation.

BACKGROUND: Advanced heart failure is associated with a wide range of physiological derangements, including muscular deterioration, weight loss, and cardiac cachexia. These conditions, as well as poor nutrition status, have been shown to affect the quality of outcomes in patients who undergo left ventricular assist device (LVAD) implantation. Noting that handgrip strength has been shown to be an effective indicator of poor functional status, muscular strength, and nutrition, we investigated its use as a tool for prediction of length of stay (LOS) in patients undergoing LVAD implantation. MATE…

Adult Native Septic Arthritis in an Inner City Hospital: Effects on Length of Stay.

The objective of this retrospective study was to assess what factors affected length of stay (LOS) in 183 adult patients with native septic arthritis. Diagnosis was based on a representative physical examination, fluid cell count/Gram stain, and organisms isolated from joint fluid culture. Data included demographics, comorbidities, laboratory results, treatment, and discharge times. Joint fluid cultures were positive in 55% (100 of 183) of the patients, and these patients were the subjects of this study. Blood cultures were taken for 65 patients and were positive in 54%; when positive, they we…

Psychiatric illness and length of stay in general hospitals: do case finding methods matter?

OBJECTIVE: Several prior studies have investigated whether patients with "non-cognitive" mental disorders (i.e., organic disorders, substance abuse, delirium, and psychotic disorders excluded) have longer Length Of Stay (LOS) than mentally healthy individuals in nonpsychiatric hospital settings. These studies yielded contrasting results. The present paper aims to examine whether methods of psychiatric case finding can explain these differences. METHODS: Using the Clinical Interview Schedule (CIS) and the General Health Questionnaire (GHQ), 462 in-patients of medical, surgical, gynecological, …

Factor affecting length of stay in late preterm infants: an US national database study.

OBJECTIVES: Late preterm infants are the fastest growing segment of the premature infant population in the United States. However, it is not known if demographic and clinical factors can impact the length of hospital stay (LOS) in this population. The objectives of this study are to determine the following: (a) factors associated with a LOS > 3 d and (b) whether there is any difference in risks between infants born at 33-34 versus 35-36 weeks. METHODS: Utilizing the Nationwide Inpatient Sample Database, a de-identified dataset produced by the Healthcare Cost and Utilization Project, analysis

Factors affecting length of stay after elective posterior lumbar spine surgery: a multivariate analysis.

BACKGROUND CONTEXT: Elective posterior lumbar fusion is a common surgical procedure, but reported length of hospital stay is variable (usually 3-7 days). The effect of a limited number of factors on length of stay (LOS) has previously been evaluated. However, multivariate analysis using LOS as a dependent variable to separate potentially confounding variables has not been performed. PURPOSE: To facilitate setting of realistic expectations and considering the significant costs of hospitalization, it would be ideal to have a clear understanding of the variables affecting LOS for this surgery. …

Accounting for variation in length of NICU stay for extremely low birth weight infants.

OBJECTIVE: To develop a length of stay (LOS) model for extremely low birth weight (ELBW) infants. STUDY DESIGN: We included infants from the California Perinatal Quality Care Collaborative with birth weight 401 to 1000 g who were discharged to home. Exclusion criteria were congenital anomalies, surgery and death. LOS was defined as days from admission to discharge. As patients who died or were transferred to lower level of care were excluded, we assessed correlation of hospital mortality rates and transfers to risk-adjusted LOS. RESULTS: There were 2012 infants with median LOS 79 days (range…
