Conversion disorder following termination of pregnancy.

A variety of psychologic disorders may present as physical complaints without organic basis. As characterized by Sydenham over 300 years ago, such disorders "mimic all the physical disease to which man is heir." The case here presents the evolution of a conversion disorder (urinary retention) in response to a decision to terminate pregnancy. The predominant disturbance in a conversion disorder is an involuntary loss or alteration in somatic function that is temporally related with an environmental stressor. Conversion, as a defense mechanism, provides primary gain by keeping the conflict from

Development of adjuvanted HIV vaccines tailored for early life: immunologic rationale, clinical experience, and future directions.

PURPOSE OF REVIEW: This review highlights the rationale for and clinical experience in advancing age-specific adjuvantation systems to develop well tolerated and effective pediatric HIV vaccines. RECENT FINDINGS: Due to both cellular and soluble factors, infant immunity is distinct from that of older children and adults, featuring Th2 bias, tolerogenic APC programming, and reduced Tfh support. Due to distinct functional responses downstream of pattern recognition receptor signaling, adjuvant action is age-specific. Despite a growing pipeline of adjuvants and increasing interest in developing

Wireless bioelectronic control architectures for muscle-powered biohybrid robotic systems.

Wireless bioelectronic interfaces are increasingly used to control tissue-engineered, muscle-powered biohybrid robotic systems. However, a unifying engineering framework linking device design to system-level control remains underdeveloped. Here, we formulate wireless control in biohybrid robotics as a coupled co-design problem integrating signal delivery, spatial selectivity, scalability, sensing and feedback, controller implementation, biological life support, and interface stability. We analyze three representative actuation and control strategies-wireless electrical stimulation, wireless op

Improving bystander CPR and AED training among college students: strategies and barriers.

Out-of-hospital cardiac arrest (OHCA) remains a leading cause of mortality, with survival highly dependent on timely bystander cardiopulmonary resuscitation (CPR) and automated external defibrillator (AED) use. Despite well-established benefits, rates of bystander intervention and training remain suboptimal, particularly within college populations. This review examines current gaps in CPR/AED training, identifies key barriers to implementation, and evaluates emerging strategies to enhance basic life support (BLS) readiness in college settings. Available evidence demonstrates that limited train

Walking and postural balance in adults with severe short stature due to isolated GH deficiency.

OBJECTIVES: Walking and postural balance are extremely important to obtain food and to work. Both are critical for quality of life and ability to survive. While walking reflects musculoskeletal and cardiopulmonary systems, postural balance depends on body size, muscle tone, visual, vestibular and nervous systems. Since GH and IGF-I act on all these systems, we decided to study those parameters in a cohort of individuals with severe short stature due to untreated isolated GH deficiency (IGHD) caused by a mutation in the GHRH receptor gene. These IGHD subjects, despite reduction in muscle mass,

Invited Commentary: Explaining the Relationship Between Attending Worship Services and Mortality-A Brief Excursion Into the Contribution of Social Relationships in Religious Institutions.

In this issue of the Journal, VanderWeele et al. (Am J Epidemiol. 2017;000(00):000-000) report that more frequent attendance at worship services is associated with a lower mortality risk. However, the underlying processes that might explain this relationship were not discussed fully. A problem arises because the potentially beneficial association between attending worship services and mortality risk may be due to a number of different factors. At least part of the explanation may be found by turning to social relationships that arise in religious institutions. Within this context, there are po

Four-year experience with rapid bone marrow chromosome analyses in newborns.

Decisions surrounding management of the profoundly ill and seriously malformed neonate involve many ethical dilemmas, as well as concern for the family and the quality of the infant's life. The more information available to parents and clinicians the better informed any decision can be regarding surgical intervention, life-support systems, or extraordinary resuscitative efforts. To provide accurate data in an appropriate time frame, a rapid analysis of bone marrow chromosomes can be of value. Over four years, 42 cases were studied by this method, yielding diagnostic information within four hou

Extracorporeal Cardiopulmonary Resuscitation as a Bridge to Cardiac Transplantation in Refractory Cardiac Arrest Due to Fulminant Myocarditis: A Case Report.

Extracorporeal cardiopulmonary resuscitation (ECPR) is increasingly used for refractory cardiac arrest, although evidence for a survival benefit remains heterogeneous and appears highly dependent on patient selection and systems of care. We report the case of a previously healthy 36-year-old woman who presented with acute fulminant myocarditis and cardiogenic shock. Following an in-hospital cardiac arrest unresponsive to conventional cardiopulmonary resuscitation, bedside ECPR was initiated using peripheral veno-arterial extracorporeal membrane oxygenation (VA-ECMO) as a bridge to longer-term

Does social support reduce bodily pain among African American women with SLE? Findings from a randomised controlled trial.

OBJECTIVE: Peer mentoring has been shown to be an effective intervention for chronic conditions with evidence to suggest that it might improve health-related bodily pain among African American women living with systemic lupus erythematosus (SLE). However, there is a lack of evidence to describe the intervention impact when adjusting for self-management of SLE. The present work aims to determine whether greater patient activation is associated with greater reductions in pain overall and within intervention groups. METHODS: Data were used from the Peer Approaches to Lupus Self-Management study,

Differences in the management of inflammatory bowel disease in children and adolescents compared to adults.

Managing IBD in children and adolescents requires attention to issues unique to these age groups. The spectrum of presenting signs and symptoms is broad and often, subtle. Physician awareness of intestinal and extra-intestinal features prompts earlier diagnosis and intervention. The focus of treatment is not limited to intestinal symptoms, but also involves assessing weight and height gains, sexual maturation, extra-intestinal manifestations and psychosocial well-being. Differences in selecting drugs for pediatric versus adult patients are based on: 1. lack of prospective trials establishing e

"This experience cannot be erased… it left a deep hole in me": perceptions of mothers of childhood cancer survivors on adapting to a new normal.

AIMS: This study explored how mothers of childhood cancer survivors (CCS) adapt to daily life following hospitalization, with emphasis on establishing a new sense of normal. It sought to provide insight into mothers' unique experiences and perceptions of post-treatment life. Specifically, it examined which resilience factors support psychosocial well-being and effective coping and which contribute to maladaptive functioning. METHODS: Guided by a phenomenological and interpretive epistemological stance, this qualitative study entailed in-depth, semi-structured interviews with five Estonian mot

Long-Term Care Insurance and Catastrophic Health Spending at the End of Life Among Older Adults: Evidence From China.

End-of-life (EOL) health spending imposes catastrophic financial burdens on elderly households, pushing them into poverty without corresponding improvements in health outcomes. Leveraging the quasi-experiment of China's public long-term care insurance (LTCI) pilot program, this study employs data from the China Longitudinal Healthy Longevity Survey to examine the impact of LTCI on catastrophic health spending (CHS) at the end of life. The findings indicate that over the 5-year EOL period, LTCI significantly decreases the probability of CHS by approximately 36.1-52.3 percentage points and reduc

Critical biological systems linking early-life stress to later-life outcomes: A systematic review of animal models.

Adverse and stressful early life events can impact on later-life cognitive, physiological and neurological outcomes, but direct mechanisms through which these so-called "programming effects" occur are still under investigation. In this review, we systematically examine early-life stress (ELS) paradigms in non-human animals, focusing on three key biological systems: the hypothalamic-pituitary-adrenal (HPA) axis, immune system and oxytocinergic system. We developed a ranking system to assess and compare stress severity across different ELS paradigms. We also consider how different theoretical fr

Trends and outcomes in heart transplantation: the Berlin experience.

Heart transplantation is an established procedure with acceptable, predictable long-term results and good quality of life for more than 20 years. However, it is only available for a limited number of patients. The fate of the patients is determined by the side effects of immunosuppressive drugs, increased tumor incidence and chronic vascular transplant disease. Patients living 10 to 20 years after heart transplantation show physical status equal to that of patients with various chronic illnesses. They suffer from chronic side effects of long-term medication and mainly report fears of renal ins
