Successful deep anterior lamellar keratoplasty following multiple failed limbal transplantations for chronic ocular burns.

A 9-year-old girl with severe unilateral ocular surface disease due to limbal stem cell deficiency following chemical injury underwent autologous cultivated limbal epithelial transplantation (CLET) twice in her right eye that failed to improve the condition of the ocular surface. On both occasions, limbal biopsies were obtained from the unaffected left eye. Since further attempts at CLET were not considered to be safe, deep anterior lamellar keratoplasty (DALK) was performed instead. DALK was complicated by postoperative Descemet's membrane detachment, which was successfully treated. Two years…

Translational research on ocular surface reconstruction using oral mucosal epithelial cell sheets.

Ocular surface reconstruction using autologous oral mucosal epithelial cell sheets has drastically changed the treatment of limbal stem-cell deficiency. The morphological and functional characteristics of oral mucosal epithelial cell sheets are similar to those of normal corneal epithelium. Ocular surface reconstruction can prevent potential problems associated with limbal transplantation, including immune rejection and donor tissue shortages. Thus far, ocular reconstruction using epithelial cell sheets has been limited to clinical research. Although the effectiveness and safety of this surgic

3D Culture Reverses Limbal Niche Cell Replicative Aging via FOSL1 Upregulation.

Limbal niche cells (LNCs) serve as essential regulators of limbal microenvironmental homeostasis and corneal epithelial wound repair, representing a promising therapeutic resource for limbal stem cell deficiency (LSCD). However, their clinical application is constrained by replicative aging during in vitro expansion. In this study, we investigated whether a three-dimensional (3D) Matrigel-based culture system could modulate replicative aging in LNCs. Compared with conventional two-dimensional (2D) culture, 3D-cultured LNCs restored stemness marker expression and enhanced proliferative capacity…

Understanding Immune Responses to Surgical Transplant Procedures in Stevens Johnsons Syndrome Patients.

Stevens Johnsons syndrome (SJS) is a mucocutaneous disorder caused by an autoimmune response most commonly to medications. Unless it is properly managed in the acute setting, this entity can affect the ocular surface causing chronic cicatrizing conjunctivitis with limbal stem cell deficiency and lid anomalies which ultimately result in corneal opacities that may limit patients' visual acuity. When this stage is reached, some patients might need to undergo some form of corneal and/or limbal stem cell transplantation that exposes an already sensitized immune system to a new alloantigen. While th…

Results of a phase I/II clinical trial: standardized, non-xenogenic, cultivated limbal stem cell transplantation.

BACKGROUND: To determine if a standardized, non-xenogenic, reduced manipulation cultivation and surgical transplantation of limbal stem cell grafts is a safe and effective treatment option for patients with total and partial limbal stem cell deficiency. METHODS: In vitro cellular outgrowth and phenotype of the limbal epithelial cell and composite grafts were validated using a new protocol. Patients received either autologous (n = 15) or allogenic (n = 3) explants cultured using a standardized protocol free from xenogenic products. The resulting grafts were transplanted using a reduced manipul…

Allogeneic Limbal Transplants Integrate into the Corneal Surface and Lead to an Improved Visual Acuity.

Limbal stem cell deficiency (LSCD) severely impairs vision and can lead to blindness. LSCD causes include chemical burns, infections, multiple previous operations and congenital malformations. Allogeneic limbal transplantation is a procedure for treating LSCD where prepared limbal tissue is attached using a double running suture during allogeneic penetrating keratoplasty (PKP). A total of 22 patients underwent ALT surgery between February 2019 and June 2022 at the University Hospital Halle (Saale). Regular follow-up was performed postoperatively every three months and included visual acuity te…
