Anterior decompression of a cervical ossified posterior longitudinal ligament (OPLL) which involves the dura mater occasionally results in a postoperative cerebrospinal fluid fistula. Bone window computed tomography (CT) was carried out to detect dural defects in 21 patients with cervical OPLL. Twelve of 21 patients had double-layer OPLL, and nine had single-layer OPLL. Dural defects were present in 10 of the 12 patients with double-layer OPLL compared with only one of the nine patients with single-layer OPLL. In addition, greater stenosis of the spinal canal by the OPLL was also associated wi

INTRODUCTION: Postoperative neuropathic pain (NeP) is common after surgery for cervical ossification of the posterior longitudinal ligament (cOPLL) and may affect clinical outcomes. The PainDETECT questionnaire (PDQ) is a widely used tool for evaluating NeP, while the Spine PDQ (SPDQ) was developed to improve specificity for spinal disorder-related NeP. This study aimed to evaluate postoperative NeP using PDQ and SPDQ and to examine their associations with clinical scores.
METHODS: A total of 234 patients (170 men and 64 women; mean age, 61.9 years) who underwent cOPLL surgery were analyzed a

Laminoplasty (LP) is a standard motion-preserving procedure for multi-segmental ossification of the posterior longitudinal ligament (OPLL), yet it may permit continued ossification progression due to retained segmental mobility. The anterior controllable antedisplacement and fusion (ACAF) technique offers ventral decompression with segmental stabilization, but its effect on OPLL fate remains unclear. To compare radiographic OPLL progression and neurological outcomes between ACAF and LP in patients with multilevel cervical OPLL. This retrospective study included 100 patients (50 ACAF, 50 LP) tr

OBJECTIVES: For the segmental ossification of cervical posterior longitudinal ligament (OPLL) accompanied by spinal cord compression, anterior cervical corpectomy and fusion (ACCF) can achieve direct decompression through removal of most of the involved vertebral body. However, reconstruction-related complications such as titanium mesh cage subsidence, loss of cervical lordosis, and poor bone fusion are common after surgery. This study aims to investigate the clinical efficacy of a novel anterior cervical mortise-tenon corpectomy and fusion (ACMTF) technique, designed based on principles of bi

BACKGROUND: The ossification of the posterior longitudinal ligament (OPLL) represents a disabling spinal condition. The specific function of miR-153-3p in the context of OPLL has not been elucidated.
AIM: To explore the function of miR-153-3p in OPLL and its regulatory mechanism related to KLF5.
METHODS: RT-qPCR was used to detect the transcriptional levels of miR-153-3p, osteogenic markers, and inflammatory cytokines in OPLL and normal populations. The interaction between miR-153-3p and KLF5 was validated through dual-luciferase reporter assays and RNA immunoprecipitation. Cell transfection

BACKGROUND: The ossification of the posterior longitudinal ligament (OPLL) is a progressive spinal disorder predominantly observed in Asian populations. Unfortunately, there is a limited availability of conservative therapies to impede the progression of OPLL. This study investigates the effects of cobalt chloride (CoCl2), which simulates an in vitro hypoxic microenvironment, on OPLL and explores its potential mechanisms, aiming to enhance our understanding of the pathogenesis of OPLL.
METHODS: Ligament cells were extracted from patients with OPLL and normal posterior longitudinal ligament (P

OBJECTIVE: The anterior controllable antedisplacement and fusion (ACAF) technique is a new procedure for the treatment of cervical ossification of the posterior longitudinal ligament (OPLL) that requires management of the disc adjacent to the ossification. This study describes a novel technique to reduce the number of fixed segments, namely, the "Klotski technique." The efficacy of ACAF using the Klotski technique was compared with that of anterior cervical corpectomy and fusion (ACCF) in the treatment of OPLL with en bloc type dural ossification (DO).

Ossification of the posterior longitudinal ligament (OPLL) of the spine is a disease of unknown etiology occurring frequently in individuals with metabolic disturbances. Obesity has been suggested as a potential risk factor for the severity of OPLL. We aimed to investigate whether non-alcoholic fatty liver disease (NAFLD) is associated with OPLL severity. We assessed the severity of NAFLD by a liver-to-spleen (L/S) ratio on computed tomography (CT) scans of 85 symptomatic OPLL patients at a single institution in Japan. We also assessed the severity of OPLL by CT reconstruction sagittal and axi

STUDY DESIGN: Retrospective cohort study.
OBJECTIVES: Vertebral body sliding osteotomy (VBSO) has previously been reported as a technique to decompress ossification of the posterior longitudinal ligament (OPLL) by translating the vertebral body anteriorly. This study aimed to evaluate the radiological and clinical efficacies of VBSO and clarify the surgical indications of VBSO for treating myelopathy caused by OPLL.
METHODS: Ninety-seven patients with symptomatic OPLL-induced cervical myelopathy treated with VBSO or laminoplasty who were followed up for more than 2 years were retrospectively

Ossification of the posterior longitudinal ligament (OPLL) can occur throughout the entire spine and can sometimes lead to spinal disorder. Although patients with OPLL sometimes develop physical limitations because of pain, the characteristics of pain and effects on activities of daily living (ADL) have not been precisely evaluated in OPLL patients. Therefore, we conducted a multi-center prospective study to assess whether the symptoms of cervical OPLL are different from those of cervical spondylosis (CS). A total of 263 patients with a diagnosis of cervical OPLL and 50 patients with a diagnos

BACKGROUND: Calcification of intervertebral disc is a common impairment, which has been considered as the degenerative condition of the spine. In clinical practice, we note that the onset of intervertebral disc calcification (IDC) and ossification of the posterior longitudinal ligament (OPLL) can exist simultaneously in some cases, especially in younger children. However, only 8 cases have been reported in detail previously. In addition, controversy remains in terms of the best way to treat this condition.
CASE DESCRIPTION: An 8-year-old female child was referred to our department in March 20
