Prosthodontic Management of a Patient with Moebius Syndrome: A Clinical Report.

Moebius/Mӧbius Syndrome (MS) is an extremely rare congenital disorder characterized by uni- or bilateral palsy of the abducens (VI) and the facial (VII) nerves, causing facial paralysis. Dysfunction of cranial nerves III through XII is common, most often the glossopharyngeal (IX) and hypoglossus (XII). Afflicted individuals seeking prosthodontic care, particularly removable prosthetics, present multiple challenges related to inherent facial and tongue muscle weakness and microstomia. Other extraoral anomalies may include congenital multiple arthrogryposis and malformations of the upper and low

Hemisection as a Conservative Management of Grossly Carious Permanent Mandibular First Molar.

Hemisection of a molar denotes removal or separation of a root along with its accompanying part of crown. It is a suitable treatment option when the caries, resorption, perforation, or periodontal damage is restricted to one root while the other root is relatively healthy. Hemisection of the affected tooth helps to retain the tooth structure, surrounding alveolar bone, and may also facilitate the placement of fixed prosthesis. This case report describes the hemisection as a successful treatment method to save a grossly carious mandibular first molar with periodontal and periapical involvement.

An alternative approach for the management of fractured implant abutment screws on a mandibular implant-retained overdenture: A clinical report.

In contemporary implant and prosthodontic practice, high levels of surgical and biologic success rates have been achieved. However, mechanical and technical complications continue to exist. Of these, one of the biggest challenges to the clinician is the conservative retrieval of fractured screw fragments from within the implants and/or abutments. While removal is desirable, the fragments cannot always be removed, and alternative solutions for the management of such clinical situations must be considered. This clinical report describes the replacement of 2 fractured, irretrievable, Locator abut

Tongue prosthesis after total glossectomy: a case report.

Squamous cell carcinoma (SCC) is the type of cancer that most frequently affects the oral cavity, mainly in men older than 50 years of age. Treatment for oral SCC often involves surgical excision of the affected margins, resulting in mutilation that affects the patient's quality of life. The objective of this case report is to describe the prosthetic treatment of a 56-year-old man who underwent total glossectomy, resulting in speech and swallowing difficulties. The proposed treatment was a tongue prosthesis retained by orthodontic clasps on the mandibular first molars. After maxillary and mand

Bare bone graft with vascularised iliac crest for mandibular reconstruction.

BACKGROUND: "Bare bone graft" with a vascularised iliac crest is a procedure involving no soft tissue for intraoral lining, and the intraoral defect is covered with epithelial cells through secondary healing of the exposed bone. METHODS: A vascularised iliac crest flap is transferred to a segmental mandibular defect intraorally in the position of the osteotomized stump upwardly. Granulation tissue is usually observed on the stump of the bone graft about 1 week after surgery. When sufficient granulation is observed after approximately 4 weeks, "resurfacing" is performed to prevent interference

Micro-costing of dental prosthetics in jaw reconstruction.

INTRODUCTION: Dental rehabilitation has a significant impact on health-related quality of life (HRQOL) for patients undergoing microvascular reconstruction of the jaw. Virtual surgical planning (VSP) and computer-aided design and computer-aided manufacturing (CAD/CAM) technology allows for precise and timely delivery of an implant-retained dental prosthesis. This study characterizes the cost to produce a point of care (in-house) 3D-printed resin dental prostheses for patients undergoing osseous free flap reconstruction. METHODOLOGY: Implant-retained dental prostheses for patients undergoing o

The use of cutting/positioning devices for custom-fitted temporomandibular joint alloplastic reconstruction: current knowledge and development of a new system.

Alloplastic replacement of the temporomandibular joint (TMJ) is the treatment of choice in cases of TMJ end-stage disease. Improvements in computer-aided design/computer-aided manufacturing (CAD/CAM) translated into the possibility ongf designi very precise TMJ prostheses based on the anatomy of each single patient. Custom-made TMJ prostheses are described in the most recent literature and provide facilitations in terms of ease of placement and accuracy. Although before the era of custom-made surgical guides, they did not play a prominent role in the field of TMJ surgery, their use has become

Accuracy of custom-fitted temporomandibular joint alloplastic reconstruction and virtual surgical planning.

The purpose of this study was to evaluate the accuracy of temporomandibular joint (TMJ) custom-made replacements by means of virtual surgical planning. The authors review 11 TMJ custom-made prostheses made of both mandibular and fossa components. Surgeries were virtually planned and patient-specific devices were designed together with surgical cutting and positional guides. Three-dimensional models for both preoperative planning and postoperative computed tomography scans were generated and overlapped in order to evaluate differences in measurements. Correlation between virtual preoperative an

Alloplastic temporomandibular joint replacement systems: a systematic review of their history.

This systematic review provides an overview of the historical evolution of the prosthetic temporomandibular joint and addresses the challenges and complications faced by engineers and surgeons, in an effort to shed light on why only a few systems remain available. A better understanding of the history of temporomandibular joint prostheses might also provide insights into the origin of the negative public opinion of the prosthesis, which is based on outdated information. A computerized search using the PubMed Central, ScienceDirect, Wiley Online, Ovid, and Cochrane Library databases was perform

Bone augmentation versus 5-mm dental implants in posterior atrophic jaws. Four-month post-loading results from a randomised controlled clinical trial.

PURPOSE: To evaluate whether short (5 mm) dental implants could be a suitable alternative to augmentation and placement of longer implants (10 mm) in posterior atrophic jaws. MATERIALS AND METHODS: Thirty partially edentulous patients with bilateral posterior edentulism were included: 15 patients having 5 to 7 mm of residual crestal height above the mandibular canal, and 15 patients having 4 to 6 mm of residual crestal height below the maxillary sinus and bone thickness of at least 8 mm measured on a CT scan. The patients were randomised either to receive one to three submerged 5-mm-long Resc

Five years of radiographic evaluation for the peri-implant bone changes of all-on-four implant prostheses constructed from different framework materials using different digital construction techniques.

BACKGROUND: There is insufficient evidence recommending a framework material and a CAD/CAM manufacturing technique for mandibular implant-supported prostheses. The study objective was to evaluate the clinical application of different materials and construction techniques used for mandibular All-on-4 prosthesis on circumferential peri-implant bony changes after 5 years. METHODS: Thirty-six male patients with all-on-4 mandibular implant-supported prostheses were recalled and divided into three groups. Effect of the number and localization of implants placed in the atrophic mandible on the fragility of the symphysis region.

BACKGROUND: Implant-supported overdenture prostheses are the preferred treatment option for rehabilitating patients with atrophic edentulous conditions. However, limited research has examined the impact of these implant applications on the structural endurance of the mandible, which is already compromised due to atrophy, particularly when subjected to frontal traumatic forces. This study aimed to evaluate the effects of traditional full dentures, two-implant, and three-implant-supported overdenture prosthesis systems in atrophic, fully edentulous mandibles on their resistance to frontal trauma

Technique for secondary modification after maxillary resection and reconstruction for soft tissue flap fixation before prosthesis addition: a case report.

BACKGROUND: The removal of maxillary carcinoma causes various types of tissue defects, which can be corrected by free flap reconstruction. In flap reconstruction after maxillary cancer resection, ensuring prosthesis stability is frequently difficult owing to the flap's weight. Therefore, a second modification technique is required for improvement of configuration. This case where flap suspension and flap modifying surgery were performed using anchor system for the extensive complete maxillectomy case. CASE PRESENTATION: The patient was a 56-year-old male, who underwent an extensive total maxi

Synovial entrapment in alloplastic temporomandibular joint replacement.

Complications of alloplastic temporomandibular joint (TMJ) prostheses can lead to stress and anxiety for the patient and the surgical team, and prosthesis substitution is sometimes required. The aim of this case report is to describe the surgical finding of synovial entrapment with interposed fibrosis in a postoperative alloplastic TMJ revision, managed effectively with adequate surgical debridement. The authors believe that synovial entrapment needs to be considered as a possible postoperative complication of total joint replacement when no clear symptoms of infection, metal hypersensitivity,

Previously Reported Approach Now Used for Temporomandibular Joint Replacement.

Over time, different techniques have been described to obtain broad access to the temporomandibular joint (TMJ), in order to allow good visibility, minimizing post-operative complications at the same time. Most of these techniques have been very useful to perform joint replacement using custom made prostheses, varying in its extension, aesthetics and functional results. The aim of this study was to present the authors' experience using the Blair approach modified by Fernández in 2015, to replace failed TMJ prostheses, when distortion of the surgical reference points has occurred.
