Incomplete palmar fracture of the proximal extremity of the third metacarpal bone in horses: ten cases (1981-1986).

In 4 adult horses, simple, nondisplaced, incomplete fracture of the proximal extremity of the third metacarpal bone (MC3) was identified radiographically only on the dorsopalmar projection. Lameness was slight to moderate. Although nerve blocks of the foot and fetlock did not alter the lameness, high palmar regional nerve block improved the gait in 1 of the 2 horses on which it was performed. Pain on palpation or swollen distal accessory (inferior check) ligament, flexor tendons, and suspensory ligament were not found in any horse. The fracture was localized to the palmar surface of the proxim

'Punch' actinomycosis causing osteomyelitis of the hand.

Osseous actinomycosis usually results from direct invasion of bone from adjacent soft-tissue infection. Involvement of the jaw and vertebral column has been frequently reported, but involvement of the bones of the wrist or hand is rare. A patient with osteomyelitis of the distal right first metacarpal bone due to Actinomyces israelii following a punch injury during fisticuffs is described. Review of similar cases revealed a striking association of this type of infection with punch injuries; hence the term, punch actinomycosis. Analysis of the pus in these infections typically reveals sulfur gr

Brachymetacarpia and brachyphalangia.

The x-ray features of 325 patients, with shortening of the metacarpals and/or phalanges were analyzed. Most patients with congenital hand anomalies (syndactyly, cleft hand, etc.) had a shortened middle phalanx of the small finger only; however, some patients also had a shortened middle phalanx of the index finger. The patients with brachymesophalangia of two and five rays also had a tendency to have shortened middle phalanges of the index and small fingers. Some patients' anomalies, such as shortening of the middle phalanges in both the index and small fingers, did not adequately fit Bell's cl

Fracture-dislocation of the base of the fifth metacarpal associated with injury to the deep motor branch of the ulnar nerve: a case report.

Traumatic neuropraxia of the deep motor branch of the ulnar nerve after a fracture-dislocation of the fifth metacarpal is reported. Salient anatomy is discussed and the diagnostic tests necessary to identify this lesion are described. Residual effects of the injury consisted of only a mild wasting of the first dorsal interosseous muscle. The patient had complete functional recovery 4 months after the initial injury.

Giant cell tumor of the tendon sheath with a pathologic phalangeal fracture. A rare association.

Giant cell tumor of the tendon sheath is a soft tissue tumor which sometimes affects the adjacent bone structure, producing erosions. In certain rare instances it may manifest as a pathologic fracture.

Classification and functional management of congenital central defect of the hand.

Classification of central defect of the hand includes three general categories--Type I (typical), Type II (atypical), and Type III (two, three, and four digit hand). These three types of central defect have one common denominator--central metacarpal deficiency or absence. Otherwise, these three distinct types differ completely in inheritance pattern, characteristic features, bilaterality, and functional management. Functional management of the Type I central defect combines release of the tethered thumb metacarpal from its adduction contracture and simultaneous closure of the cleft using that

Isolated volar dislocation of the fifth carpometacarpal joint. Case report.

A rare volar-ulnar dislocation of the base of the fifth metacarpal in a 73-year-old man was correctly diagnosed early and treated by closed reduction. The reduction was unstable and percutaneous skeletal fixation was applied to ensure stability. The functional results were excellent within four months of surgery.

Auxiliary osseous findings in fetlocks of non-racing sports horses with sagittal groove disease of the proximal phalanx on low-field magnetic resonance imaging.

BACKGROUND: Sagittal groove disease of the proximal phalanx in equine athletes is commonly considered a bone stress injury. Repetitive hyperextension of the fetlock under high load is thought to contribute to its development. Concurrent changes are often reported in the dorsal sagittal ridge of the third metacarpus/metatarsus (MC3/MT3). OBJECTIVES: To describe the spectrum of associated osseous abnormalities that are present in the fetlock in a large group of horses diagnosed with sagittal groove disease on low-field magnetic resonance imaging (MRI). STUDY DESIGN: Retrospective, cross-sectio…

Determination of correlation of proximal sesamoid bone osteoarthritis with high-speed furlong exercise and catastrophic sesamoid bone fracture in Thoroughbred racehorses.

OBJECTIVE: To examine whether proximal sesamoid bone (PSB) articular cartilage and bone osteoarthritic changes or palmar osteochondral disease (POD) scores were associated with exercise history and catastrophic PSB fracture in Thoroughbred racehorses. SAMPLES: PSBs from 16 Thoroughbred racehorses (8 with and 8 without PSB fracture). PROCEDURES: Exercise history was collected, and total career high-speed furlongs was used as the measure of total exercise per horse. At necropsy, medial and lateral condyles of the third metacarpus from each forelimb were assigned a POD score, followed by imagin…

Adults with Prader-Willi syndrome have weaker bones: effect of treatment with GH and sex steroids.

Obesity has been considered to have a protective effect against the risk of fractures in adults. However, a high frequency of fracture is de … teroids a negative effect on bone size and strength. PWS subjects showed a reduced bone size at the metacarpus leading to a reduced strength, while bone density was appropriate for size. GH treatment improves bone geometry but not bone density. Bone strength was significantly reduced in PWS patients who did not receive GH and had been treated with sex steroi …

First ray reconstruction with distraction osteogenesis.

Disarticulation of the thumb at the metacarpophalangeal joint level is not beneficial either aesthetically or functionally without additiona … f Disability of the Arm, Shoulder and Hand (DASH) score and pick-up test. It was concluded that the metacarpus lengthening by callus distraction technique may be a functionally and cosmetically effective reconstruction method for traumatic thumb amputations. Equine Standing Multidetector Computed Tomography of the Distal Thoracic Limb and Tarsus Has a Lower Cumulative Radiation Dose than Digital Radiography.

Recent technological advancements in CT have improved the ability to scan standing sedated horses. However, the impact of radiation exposure … antaromedial oblique and dorso45medial-planterolateral oblique). The MDCT scans included the distal metacarpus to the foot and the tarsus. Noninferiority testing showed lower radiation exposure to the imaging technician during MDCT of the distal thoracic limb and tarsus compared with DR, measured at the thyroid gland, hand, and feet. Mosaic arthroplasty in equine stifle and fetlock joints: A retrospective study of 31 cases between 1998 and 2023.

OBJECTIVE: To evaluate the clinical application of equine mosaic arthroplasty for joint surface repair, including outcomes and complications. STUDY DESIGN: Retrospective clinical study. ANIMALS: A total of 31 horses diagnosed with subchondral bone cysts (SBCs) in the femoral condyle (22/31), distal metacarpus (7/31), or metatarsus (2/31). METHODS: Medical records of horses that underwent autologous or allogeneic osteochondral graft transplantation were reviewed. Follow-up lasted at least 12 months. Success was determined in terms of improvements in lameness and post-surgical athletic perfor…
