Rejection of metal to metal prosthesis and skin sensitivity to cobalt.

Metal to metal prostheses give satisfactory results in 90% of patients. About half of the failure rate may be due to allergic reaction to the metals involved, particularly cobalt. A total of 35 patients in this unsatisfactory group have been patch-tested; 16 were positive to metals, 13 to cobalt, 4 to nickel, and 2 to chromate. Only two patients showed any skin lesions - one a localized dermatitis round the knee joint from nickel sensitivity, and one to cobalt who had a widespread scattered circular erythematous lesion suggestive of a generalized allergic vasculitis. Patients requiring a metal…

Nonbonding Metal-Metal Interaction in Metal-Nitrogen-Carbon Single-Atom Catalysts Boosts CO Electroreduction.

Metal-nitrogen-carbon single-atom catalysts (SACs) have recently emerged as selective electrocatalysts for the reduction of CO2 to CO, but their ability to further electroreduce CO is poor. Here, based on constant-potential density functional theory simulations, we predict that Co-N-M (M = Fe, Co) SACs with nonbonding metallic centers bridged by a common nitrogen atom can catalyze four-electron reduction of CO to methanol at an ultralow overpotential of 220-310 mV. We show that the metal atoms in the SACs are terminated by H species which prevent the formation of σ bonding between CO and the m

Influence of implant design on blood metal ion concentrations in metal-on-metal total hip replacement patients.

PURPOSE: Most factors considered to affect blood metal ions following metal-on-metal hip replacement are based on hip resurfacing patients. The study aims were to determine which factors affect blood metal ion concentrations following metal-on-metal total hip replacement (MoM THR). METHODS: All unilateral MoM THR patients at one centre with whole-blood cobalt (Co) and chromium (Cr) concentrations measured up to May 2013 were included. Blood sampling was at a mean of 4.5 years (range 1.1-11.8 years) postoperatively. RESULTS: Of 496 patients (mean age 59.1 years; 52.8% male), blood metal ions

Revision of metal-on-metal hip replacements and resurfacings for adverse reaction to metal debris: a systematic review of outcomes.

PURPOSE: This systematic review assessed evidence on outcomes following revision of MoM hip resurfacings (HRs) and total hip replacements (THRs) for adverse reaction to metal debris (ARMD). METHODS: Four electronic databases were searched between January 2009 and July 2013 to identify studies reporting clinical outcomes following revision of MoM HRs and THRs for ARMD. Only studies reporting cohorts with more than 10 metal-on-metal (MoM) hips revised for ARMD were included. Outcomes of interest following ARMD revision were: (1) complication rates; (2) re-revision rates; (3) surgical interventi

Concomitant infection and local metal reaction in patients undergoing revision of metal on metal total hip arthroplasty.

Total hip arthroplasty (THA) with conventional polyethylene bearings is traditionally the standard operative treatment for endstage arthritis of the hip. This design has excellent survivorship in most populations, with a low occurrence of infection and other associated complications. Due to concern over increased wear in younger, more active populations, other bearing surfaces have been evaluated, particularly metal-on-metal with wear rates theorized to be lower than conventional THA. Unique to metal-on-metal THA, however, is the possibility of local soft tissue reactions that can mimic infect

[Various factors influencing toxicity and metabolism of metals--metal-metal interactions and host factors (author's transl)].

This paper is a brief overview on metal-metal interactions and various other factors involved in the toxicity and metabolism of metals, including host factors, nutrition, habits, etc. Humans and other organisms are exposed to various metals in many ways, and even in an occupation in which workers are exposed to a metal such as lead, they are at the same time also exposed to other metals, although concentrations of these latter metals are usually not very high. This can result in effects different from those produced by a single metal by itself. Furthermore, individual differences which are cau…

In vivo response of heme-oxygenase-1 to metal ions released from metal-on-metal hip prostheses.

Metal ion release and accumulation is considered to be a factor responsible for the high failure rates of metal-on-metal (MoM) hip implants. Numerous studies have associated the presence of these ions, besides other factors, including a hypoxia‑like response and changes in pH due to metal corrosion leading to the induction of the oxidative stress response. The aim of the present study was to verify whether, in patients with a MoM hip prosthesis, mRNA and protein expression of HMOX‑1 was modulated by the presence of metal ions and whether patients without prostheses exhibit a different expressi

What are the predictors and prevalence of pseudotumor and elevated metal ions after large-diameter metal-on-metal THA?

BACKGROUND: Soft tissue masses, or "pseudotumors," around metal-on-metal total hip arthroplasty (MoM THA) have been reported frequently; however, their prevalence remains unknown. Several risk factors, including elevated metal ion levels, have been associated with the presence of pseudotumor, although this remains controversial. QUESTIONS/PURPOSES: The goals of this study were to (1) determine the prevalence of pseudotumors after large-diameter head MoM THA; (2) identify risk factors associated with pseudotumor formation and elevated metal ion levels; and (3) determine the early failure rate

Cr-Cr Quintuple Bonds: Ligand Topology and Interplay Between Metal-Metal and Metal-Ligand Bonding.

Chromium-chromium quintuple bonds seem to be approaching the lower limit for their bond distances, and this computational density functional theory study tries to explore the geometrical and electronic factors that determine that distance and to find ways to fine-tune it via the ligand choice. While for monodentate ligands the Cr-Cr distance is predicted to shorten as the Cr-Cr-L bond angle increases, with bridging bidentate ligands the trend is the opposite, since those ligands with a larger number of spacers between the donor atoms favor larger bond angles and longer bond distances. Compared

Metal-on-metal hip resurfacing: correlation between clinical and radiological assessment, metal ions and ultrasound findings.

PURPOSE: We report the clinical, radiological and wear analysis of 52 consecutive MoM hip resurfacings (performed on 49 younger patients) to a mean follow-up of 9.2 years. METHODS: Every patient underwent X-ray and clinical evaluation (HHS). Ultrasonography of the hip was performed in all patients in order to identify possible cystic or solid mass in periprosthetic tissue. In case of mass >20 mm, further MRI was performed to better analyse the characteristics of lesion. RESULTS: Five patients (five hips) had a revision. The overall survival rate was 90.38 %. The average HHS at follow-up exam

Influence of head size on the development of metallic wear and on the characteristics of carbon layers in metal-on-metal hip joints.

BACKGROUND AND PURPOSE: Particles originating from the articulating surfaces of hip endoprostheses often induce an inflammatory response, which can be related to implant failure. We therefore analyzed the metal content in capsular tissue from 44 McKee-Farrar metal-on-metal hip prostheses (with 3 different head sizes) and we also analyzed the morphological structure of layers located on articulating surfaces. METHODS: Atomic absorption spectrometry (AAS) was used to analyze the metal content in capsular tissue. Visually detectable carbon layers located on the articulating surfaces were evaluat

[Metal poisoning].

Metals are amongst the oldest toxic substances known to man. In today's industrialized world the sources of exposure to metals are ubiquitous both in the field of work and from polluted water, foodstuffs and the environment. Their toxicity is characterized by the metallic element in question, but this is modified by the type of compound, whether organic or inorganic, and its characteristics of hydrosolubility and liposolubility, which determines its toxicokinetics and thus the possibilities of it reaching its targets. The biomolecules most affected by metals are the proteins with enzymatic act…

Periprosthetic tissue metal content but not serum metal content predicts the type of tissue response in failed small-diameter metal-on-metal total hip arthroplasties.

BACKGROUND: Tissue responses to periprosthetic metal wear debris are complex and poorly understood. There are two predominant tissue responses: a nonspecific macrophage-mediated granulomatous response and lymphocyte-dominated response, which has immunological memory and is mediated by T cells. Delayed hypersensitivity-type responses may accelerate aseptic loosening of arthroplasty implants. We hypothesized that the metal content of periprosthetic tissue but not of serum would be predictive of the type of tissue response to metal wear debris. METHODS: We examined twenty-eight total hip arthrop

Assessment of leached heavy metals from construction and demolition waste.

Construction and demolition waste (CDW) is a substantial environmental concern due to the presence of heavy metals that leach into soils and groundwater. This study investigates the concentrations and spatial migration patterns of heavy metals leached from CDW at a construction site in Kumasi, Ghana. Samples of CDW and underlying soil were collected and analyzed for heavy metals, including lead (Pb), arsenic (As), zinc (Zn), copper (Cu), manganese (Mn), and cadmium (Cd). The findings of the study indicate that the CDW largely consists of sand, which helps leachate move easily. Inverse distance…

The effect of shungite pretreatment on quantitative extraction of metals.

One of the most interesting natural materials with a rich composition of various elements, including rare earth elements, is shungite. Inductively coupled plasma mass spectrometry (ICP-MS) was successfully used to detect metals in various ores, which has high sensitivity for determining elements even in trace amounts. ICP-MS analysis revealed 53 elements in shungite, the total content of which in the original sample did not exceed 3 wt%. The effect of preliminary treatment with various solvents (extraction) on the isolation and determination of the maximum content of elements in shungite was c…
