Chevron osteotomy for bunionette.

Numerous procedures have been described for the surgical treatment of symptomatic bunionettes. We describe the technique, results, and follow-up of patients treated with a chevron osteotomy of the distal fifth metatarsal. This surgical approach to the treatment of bunionette is presented as a viable alternative to other surgical procedures. Sixteen distal fifth metatarsal chevron osteotomies were performed on 12 patients. Follow-up was from 15 months to 6 years, with an average follow-up of 3.2 years. A 100-point scoring system was devised and the average score improved from 44 points before s

Triggering of the lesser toes at a previously undescribed distal pulley system.

We have treated five patients with metatarsalgia and triggering of the lesser toes. This paper describes a mechanism of triggering that involves a previously undescribed flexor pulley system in the toes that is very similar to the pulley system in the fingers. The specific anatomy of the toe flexor pulleys is described based upon the dissections of 50 fresh-frozen cadaver toes. These pulleys were composed of transverse to obliquely oriented fibrous bands within the flexor sheaths from the metatarsal heads to the distal phalanges. We found a system of pulleys in the lesser toes and the great to

Forefoot Pain in the Lesser Toes: Anatomical Considerations and Magnetic Resonance Imaging Findings.

Various conditions may result in forefoot pain. Magnetic resonance (MR) imaging allows accurate assessment of many of these conditions. We provide an overview of forefoot disorders divided into bones, capsule and plantar plate, musculotendinous structures, neurovascular structures, and subcutaneous tissue. We review normal anatomical features as well as MR imaging findings of common disorders.

[Personal experience with Wolf's metatarsal osteotomy].

The author presents longterm experience with the surgical treatment of metatarsalgia by subcapital osteotomy. Operated on were 903 metatarsals in 281 patients. The operation brought relief in 96.5 per cent of cases. The article presents the surgical technique, indications for and complications of the method.

Metatarsophalangeal joint instability of the lesser toes.

Metatarsophalangeal joint instability of the lesser toes is a common finding and a common cause of metatarsalgia. The clinical presentation can include swelling without digital deformity; however, often, this can progress to the development of coronal and transverse plane malalignment. In some cases, frank metatarsophalangeal joint dislocation can develop. The treatment regimen has historically focused on indirect surgical realignment using soft tissue release, soft tissue reefing, tendon transfers, and periarticular osteotomies. An improved understanding of the plantar plate has recently led

A new elevating metatarsal osteotomy: geometrical design and applications.

For metatarsalgia and lesser toe deformities, a commonly performed procedure is the Weil osteotomy, which inevitably depresses the metatarsal head, changing the intrinsic muscle axis and generating phalanx extension. The objective of this study is to present a new metatarsal osteotomy that shortens or lengthens and elevates the metatarsal head, the elevating metatarsal osteotomy (EMO), with its geometrical analysis and possible applications. A geometrical model was designed, planning an oblique subcapital osteotomy with slice resection and distal displacement. A trigonometric analysis was done

I'm 76 and an avid walker. Lately, though, I've noticed a burning sensation and pain on the ball of my foot when I walk or stand--it's even worse if I'm barefoot or in socks. It feels like I'm walking on pebbles. The pain goes away when I'm off my feet. What might cause this?

[Surgical correction of an unusual forefoot deformity in a patient with rheumatoid arthritis].

Authors report on a foot deformity, that can be considered as literary rarity and on its operative treatment. The alteration described was unusual because of the flexion contracture beside the valgus position of the hallux. The aim of the operative procedure described was the correction of the position of the hallux and the improvement of the metatarsalgia, causing nearly complete inability of walk.

Results of flexor-to-extensor and extensor brevis tendon transfer for correction of the crossover second toe deformity.

Between 1990 and 1995, 38 patients (42 feet) underwent repair for crossover toe deformity, 31 (35 feet) of whom returned for final examination at an average of 51.6 months (range, 24-81 months). Causes included trauma, iatrogenic, and unknown. Presenting complaints included dorsal pain with either metatarsalgia or joint pain, isolated metatarsophalangeal (MP) joint pain, metatarsalgia, painful plantar callus, metatarsalgia and joint pain, and painful dorsal callus. All patients were treated with one of two operative techniques, either the flexor-to-extensor tendon transfer or the extensor brev

[Keller-Brandes operation: long-term outcome in young patients with hallux valgus].

PURPOSE: Aim of this retrospective study was to analyse the long term results (10-16 years) after Keller-Brandes' operation for correction of hallux valgus in the younger patient. METHODS: From 1980 to 1986 fifty-one patients (77 feet) at the age of 40 years or younger at the time of surgery (19-40 years, [symbol: see text] 34 years), were operated according to a resection-arthroplasty at our institution. 24 patients (37 feet) were evaluated retrospectively after 10 to 16 years (median: 13 years) in respect to their clinical outcome. Radiological evaluation was performed preoperatively in 23

Distal Oblique Metatarsal Osteotomy for Hallux Valgus Deformity: A Clinical Analysis.

We compared the outcomes of the distal oblique metatarsal (DOM) osteotomy, which is parallel to the articulation surface of the proximal phalanx, with those of the chevron osteotomy and evaluated whether displacement and shortening of the first metatarsal have any effect on the incidence of metatarsalgia and patient satisfaction. Patients treated with the DOM osteotomy (n = 30) or distal chevron osteotomy (n = 31) were evaluated retrospectively. The chevron and DOM osteotomies both provided significant improvement in the first intermetatarsal angle (p < .001), hallux valgus angle (p < .001), d

Is there a tolerance for extension of the first metatarsal after the Lapidus procedure? A clinical, functional, and hindfoot radiographic analysis.

PURPOSE: Based on the tripod concept of load distribution, our study aimed to evaluate whether a slight extension of first metatarsal (M1) that may occur after the Lapidus procedure (LP) could alter the radiographic measurements of the hindfoot and influence clinical and functional outcomes. METHODS: Twenty-five patients (27 feet) were reviewed. Hindfoot radiographic analysis was based on seven measurements. Clinical and functional outcomes were evaluated with self-reported questionnaires. Transfer metatarsalgia was also assessed. Correlation analysis was performed according to variations of

Distal metatarsal osteotomy for intractable plantar keratoses.

A relatively simple distal osteotomy of the metatarsal necks is presented. Twenty-one feet were followed for an average of 31 months with a diagnosis of intractable plantar keratosis causing metatarsalgia. A treatment plan is given that indicates which metatarsals to osteotomize, depending on location of the keratosis. Results and complications are discussed.

High-resolution Ultrasound of the Foot and Ankle.

High-resolution ultrasound (US) can be used to assess soft tissue abnormalities in the foot and ankle. Compared to MRI, it has lower cost, is widely available, allows portability and dynamic assessment. US is an excellent method to evaluate foot and ankle tendon injuries, ligament tears, plantar fascia, peripheral nerves, and the different causes of metatarsalgia.

The aging foot.

In the aging patient changes that compromise mobility may occur in the feet. Common foot pathologies are heel pain, metatarsalgia, hammertoes and clawtoes, bunions, hallux rigidus, corns and calluses, nail pathologies, arthritis, and neuropathies. To educate patients and families, nurses must be familiar with these conditions and their prevention, conservative treatment, surgical procedures, nursing interventions, and patient education.
