BACKGROUND: Milk provides a readily available diagnostic fluid collected daily or more frequently on an individual animal or herd basis. Milk, as an aggregated sample in bulk tank milk (BTM) represents the status of a herd instead of a single animal. Background: Dicloxacillin is a beta-lactam antibiotic that is commonly used in the treatment of lactational mastitis in breastfeeding women. Although penicillins have long been considered safe for breastfeeding mothers and their infants, there is almost no data on the transfer of dicloxacillin into human breast milk despite the fact that it is commonly used for mastitis. Linezolid, a broad-spectrum antibiotic used primarily for treatment of methicillin-resistant Staphylococcus aureus (MRSA) infections, is the first oxazolidinone approved for clinical use. This is a case report of a 30-year-old woman who was exclusively breastfeeding her infant prior to taking linezolid 600 mg orally every 12 hours to treat a MRSA mastitis. For a five month period, infants examined on admission and discharge from the newborn nursery and at two-week and two-month well-baby visits were examined for breast nodule size and the presence of galactorrhea. Whole bovine milk should not be fed to infants during the first year of life because of its association with occult gastrointestinal bleeding, iron deficiency anemia, and cow's milk allergy. Thirty antibiotics, including penicillins, cephems, chloramphenicol, tetracyclines, macrolides, and aminoglycosides, were given p.o., i.m. or i.v. once to 70 puerperal women to compare milk drug levels with serum levels over a 6-h period. Antibiotic transfer into the milk was small. No drugs except for a few (chloramphenicol, tetracyclines, macrolides) reached the level of 1 mcg/ml or greater. Penicillins and cephams detected in trace amounts in the milk are likely to have little effect on infants.

The concentration of verapamil in milk and serum was determined in samples taken within the same hour 3-5 days after delivery of a 25 year old women treated with verapamil (Isoptin) 240 mg daily for paroxysmal supraventricular tachycardia. The average concentration of verapamil in milk was found to be 23% of that in serum, and the total excretion in milk was less than 0,01% of the administered dose.
