AAEM minimonograph #41: neuromuscular diseases associated with HIV-1 infection.

Neuromuscular diseases occur in as many as 50% of patients infected with human immunodeficiency virus type 1 (HIV-1). All forms of neuromuscular disease have been reported, including axonal neuropathy, demyelinating neuropathy, mononeuropathy multiplex, polyradiculitis, ALS-like syndromes, disorders of neuromuscular transmission, myopathy, and toxic neuropathies due to medication side effects. Neuromuscular disease is often the presenting manifestation of HIV-1 infection. Infection with cytomegalovirus (CMV) is associated with different types of neuropathy including mononeuritis multiplex and

Mononeuritis multiplex secondary to sarcoidosis.

Peripheral nerve involvement is a rare, yet treatable neurological manifestation of sarcoidosis. Most patients respond well to corticosteroids, but relapses are common and the long-term prognosis remains unpredictable. We present a patient with an asymmetrical neurological presentation of previously undiagnosed sarcoidosis. She presented with paresthesias and predominantly distal extremity weakness. Other possible causes of neuropathy were ruled out and she was found to have an elevated serum ACE level. A nerve/lip biopsy demonstrated non-caseating granulomas consistent with sarcoid. Her clini

Neurosarcoidosis Mimicking the Recurrence of Malignant Lymphoma.

A 67-year-old woman with a recurrent history of malignant lymphoma (ML) presented with muscle weakness and paresthesia of the fingertips and feet. Due to the elevated level of serum soluble interleukin-2 receptor and increased 18F-fluorodeoxyglucose uptake in a mediastinal lymph node, neurolymphomatosis was initially suspected. Neurological and electrophysiological examinations were consistent with mononeuropathy multiplex. A diagnosis of neurosarcoidosis was made based on the presence of noncaseating epithelioid granulomas in the mediastinal lymph node, along with the presence of the uveitis,

Neuromuscular complications associated with liver transplantation.

We studied neuromuscular complications in a cohort of 520 patients with liver transplantation. Perioperative mononeuropathy developed in 9 p … liver transplantation are uncommon (less than 1%) and do not significantly contribute to morbidity. Mononeuropathies may have iatrogenic perioperative causes, and rhabdomyolysis may be an important cause of generalized muscle weakness after liver transplantation.

Peripheral facial palsy as an initial symptom of Lyme neuroborreliosis in an Austrian endemic area.

BACKGROUND: The objective of this study was to analyze the percentage as well as clinical and laboratory characteristics of Lyme neuroborreliosis (LNB) in patients admitted with peripheral facial palsy. Additionally, we looked for diagnostic criteria to distinguish Bell's palsy from facial palsy due to LNB. METHODS: Data collection was done retrospectively from 2007 until 2012. We identified 278 consecutive patients, who were admitted to the department of Neurology due to peripheral facial palsy. Patients were routinely investigated for LNB including clinical neurological examination and cere

Diabetic neuropathy.

Neuropathies related to diabetes mellitus can affect 60-70% of patients with diabetes. These can include peripheral polyneuropathies, mononeuropathies, and autonomic neuropathies. Control of glucose, lipids, and hypertension and cessation of smoking can limit onset and progression of these neuropathies. Besides control of the above listed risk factors, we do not have effective medications to treat the pathophysiologic mechanisms of diabetic neuropathies. Treatment is limited to ameliorating pain and correcting the end organ consequences of the neuropathic processes.

Focal and entrapment neuropathies.

Of the many patterns of peripheral nerve disorders in diabetes mellitus (DM), isolated clinical involvement of single nerves, though less common than distal symmetric polyneuropathy and perhaps polyradiculoneuropathy, constitute an important collection of characteristic syndromes. These fall into four anatomical regions of the body: cranial, upper limb, truncal, and lower limb territories. Each of these groups of mononeuropathies has its own ensemble of epidemiologic patterns, clinical presentations, laboratory and radiologic findings, differential diagnosis, management principles and prognosi…

Multiple enlarged nerves on neurosonography: an unusual paraneoplastic case.

Multiple nerve enlargements at non-entrapment sites are usually caused by hereditary or acquired immune-mediated neuropathies. We describe a case of multifocal hypertrophic mononeuropathies detected by nerve sonography with a clinical picture of progressive mononeuritis multiplex caused by a paraneoplastic syndrome associated with anti-Hu antibodies. This case illustrates an unusual but important paraneoplastic differential diagnosis of progressive multifocal hypertrophic neuropathies. It emphasizes the role of nerve ultrasound in the diagnostic work-up of peripheral nervous system disorders.

Neuromuscular complications in uremics: a review.

BACKGROUND: Uremia may be associated with various neurologic manifestations, particularly a polyneuropathy, but also with focal neuropathies such as carpal tunnel syndrome and shunt-related neuropathies. Myopathies can also be caused by uremia and its metabolic disarrangements. REVIEW SUMMARY: This article reviews the clinical presentation, pathogenesis, and treatment of uremic polyneuropathy, focal neuropathies, and uremic myopathies. CONCLUSION: Recognizing the presentation and pathogenesis of uremic polyneuropathies, mononeuropathies, and myopathies are important for their prevention and …

DENGUE WITH CENTRAL NERVOUS SYSTEM INVOLVEMENT.

Dengue has spread to new geographic areas affecting both children and adults, and it has become a global threat. Dengue with central nervous system involvement includes febrile seizures, encephalopathy, encephalitis, aseptic meningitis, intracranial hemorrhages, intracranial thrombosis, subdural effusions, mononeuropathies, polyneuropathies, Guillain-Barré syndrome, and transverse myelitis. These manifestations may be associated with co-infections, co-morbidities, or complications of prolonged shock. It is important to consider dengue as a cause for the above neurological presentations, partic…

Diabetic neuropathy, A review of clinical manifestations.

Diabetic neuropathy in some form or other afflicts a majority of patients with diabetes mellitus. Neuropathic disturbance of sensory, motor or autonomic nerves may occur singly or in combination. Cranial nerve and other mononeuropathies generally resolve spontaneously. Autonomic neuropathy which can result in orthostatic hypotension, gastroparesis diabeticorum, nocturnal diarrhea, atonic bladder and impotence, although chronic, may wax and wane in clinical severity. Neuritis, disesthesias and painful sensory neuritis may resolve with good diabetic control; on occasion, diphenylhydantoin has be…

Neuralgic amyotrophy: an increasingly diverse entity.

We describe nine cases of neuralgic amyotrophy whose clinical and electrophysiologic findings suggest lesions of individual peripheral nerves or peripheral nerve branches occurring singly (mononeuropathy) or in various combinations (mononeuropathy multiplex). There were four occurrences of isolated denervation of the pronator teres muscle; four occurrences of anterior interosseous nerve lesions; three occurrences of lateral antebrachial cutaneous nerve lesions; two occurrences of long thoracic nerve lesions; and one occurrence each of a median nerve trunk lesion, a median palmar cutaneous bran

Nerve Ultrasonography as an Additive Tool to Clinical Examination and Electrodiagnostics in Sporadic Mononeuritis - Imaging is the Key.

PURPOSE: Sporadic mononeuropathies without trauma or compression are challenging to diagnose. Nerve ultrasound has recently proven its usefulness in the diagnosis of traumatic neuropathies, tumors and polyneuropathies. However, its role in mononeuropathies currently remains unclear. We describe ultrasonography follow-up results in 12 patients with suggested spontaneous, monophasic mononeuritis without signs of generalization. MATERIALS AND METHODS: Nerve conduction studies (NCS), ultrasonography of the affected nerves and the contralateral side, laboratory analysis, and if possible magnetic r…

Unexplained delayed nerve impairment in leprosy after treatment.

The objective of this study was to examine the clinical signs, symptoms and course of neuropathies in patients with leprosy who after treatment developed nerve impairment, not explained by relapse or reversal reactions. We searched the case-records of leprosy patients, seen between 1985 and 2002 at the department of dermatology at our centre. Included in the study were patients who had developed nerve impairment after treatment of leprosy in the absence of relapse, erythema nodosum leprosum, or reversal reactions, and who were referred to a neurologist. In these patients, we recorded age, onse

Mononeuritis Multiplex as an Initial Presentation of Rheumatoid Arthritis: A Rare Case Report from Bangladesh.

Rheumatoid vasculitis (RV) is an uncommon extra-articular manifestation of Rheumatoid Arthritis (RA) that typically affects patients with a more aggressive form of the disease. The diagnosis of rheumatoid vasculitis is suggested based on clinical presentation, laboratory findings, and confirmed through biopsy. We describe the case of a 28-year-old man who initially presented with myalgia and weakness in all four limbs. Subsequently, he developed hypertension and testicular tenderness. Neurological examination revealed painful sensory-motor neuropathy involving the bilateral median and ulnar ne
