[Infant botulism].

Infant botulism is a rare disease that affects infants below the age of 12 months following absorption of neurotoxins produced by ingested Clostridium botulinum spores. The clinical manifestations are caused by symmetrical cranial nerve palsies followed by descending, symmetric flaccid paralysis of voluntary muscles. Presenting symptoms include constipation, lethargy, mydriasis and ptosis. The diagnosis is made on the basis of clinical examination and confirmed by isolating the toxin in serum or stools. Treatment consists of supportive intensive care and treatment with antitoxins.

Toxic endothelial cell destruction syndrome after intraocular lens repositioning with intracameral epinephrine.

A 66-year-old Chinese man developed corneal endothelial decompensation after intraocular lens (IOL) repositioning using intracameral epinephrine was performed. The presentation was compatible with toxic endothelial cell destruction syndrome, which is caused by prolonged, direct exposure of the corneal endothelium to relatively high concentrations of intracameral epinephrine. Despite its effective and immediate mydriatic effect, intracameral epinephrine is not recommended for intraoperative mydriasis in procedures such as IOL repositioning or secondary IOL implantation in which minimal irrigati…

Effect of cataract extraction on the pupil response to mydriatics.

Pupil diameters in the dark, in the light, and after mydriasis with tropicamide+ phenylephrine have been measured in 25 patients before and six months after cataract surgery. The aphakic pupil showed reduced mobility as evidenced by a smaller diameter in the and a larger one in the light. In patients who had intracapsular extraction or extracapsular extraction with intraocular lens implantation the pupils dilated less well than preoperatively. The difference in response to mydriatics is of practical importance in the assessment and treatment of peripheral retinal disorders in aphakic and pseud…

Familial iridoplegia.

Fixed, dilated pupils in childhood are usually due to interruption of parasympathetic innervation as a result of neurologic impairments or occur following pharmacologic mydriasis. We present a familial form of iridoplegia without structural defects of the eye which is consistent with a probable autosomal dominant mode of inheritance. Examination of other family members should be part of an affected individual's evaluation particularly when this finding occurs as an isolated defect. Recognition of such an entity may avoid unnecessary neurodiagnostic studies in an affected individual.

The effects of phenylephrine on pupil diameter and accommodation in rhesus monkeys.

PURPOSE: Phenylephrine is used to dilate the iris through alpha-adrenergic stimulation of the iris dilator muscle. Sympathetic stimulation of the ciliary muscle is believed to be inhibitory, decreasing accommodative amplitude. Investigations in humans have suggested some loss of functional accommodation after phenylephrine. It is unclear whether this loss is due to direct action of phenylephrine on the ciliary muscle or to secondary optical factors associated with mydriasis. The purpose of this study was to determine whether phenylephrine affects Edinger-Westphal (EW)-stimulated accommodation …

Selective suppression by bunazosin of alpha-adrenergic agonist evoked elevation of intraocular pressure in sympathectomized rabbit eyes.

PURPOSE: To determine whether there is alpha 1-adrenergic receptor heterogeneity associated with the regulation of intraocular pressure (IOP) and mydriasis in rabbits, the authors tested the hypothesis by characterizing the ability of the selective alpha 1-adrenergic antagonist, bunazosin, to block the ocular hypertensive and mydriatic responses to alpha 1-adrenoceptor stimulation by either norepinephrine (NE) or phenylephrine (PE). METHODS: The effects of topical application of bunazosin on IOP and pupillary diameter were measured in unilateral superior cervical ganglionectomized (SCGX) rabb…

[Autonomic effects of scaritoxin in the cat and guinea pig. Cardiac protection by an anti-arrhythmia agent].

A purified extract from the muscles of the fish Scarus gibbus (93% scaritoxin and 7% ciguatoxin) was tried on anesthetized cats and isolated guinea-pig atria. Intra-venous injection to cats (10, 20 and 40 mg/kg) induced deep respiratory depression, inhibition of duodenal contractions, increased salivary secretion, mydriasis and lacrymal secretion. Heart rate was slowed by 17 to 44 per cent. Impaired cardiac conduction led to different types of blocks. On the contrary, cardiac excitability was enhanced, as evidenced by the occurence of many auricular and mostly ventricular extra-systoles, often…

Probable Ciliary Nerve Within Capsule of an Orbital Cavernous Venous Malformation.

Removal of an orbital cavernous venous malformation in a 39-year-old white female resulted in a significant intraoperative sectoral mydriasis. White fascicles were recorded as running into the mass at surgery, and one of these was confirmed histologically to be peripheral nerve within the capsule of a markedly fibrous orbital cavernous venous malformation. Most efferent pupillary anomalies after orbital surgery are probably due to ischemic or tractional neuropraxia, but the resected nerve raises the possibility of unplanned excision of a few fascicles of the postganglionic (parasympathetic) sh…

Clinical and neuroimaging features of a familial pathogenic ACTA2 variant as a model of a vascular neurocristopathy.

The clinical and neuroimaging findings of a family with a variant ACTA2 gene (c351C > G), presenting with smooth muscle dysfunction in structures of neural crest derivation, are discussed. The combination of aortic abnormalities, patent ductus arteriosus, congenital mydriasis and distinctive cerebrovascular and brain morphological abnormalities characterise this disorder. Two sisters, heterozygous for the variant, and their mother, a mosaic, are presented. Brain parenchymal changes are detailed for the first time in a non-Arg179His variant. Radiological features of the petrous canal and extern…

Persistent troublesome floaters necessitating the explantation of XtraFocus Pinhole IOL (Morcher).

A 41-year-old patient presented with blurred vision and photophobia in the left eye with an uncorrected visual acuity of 20/150, improving to 20/30 with pinhole and diagnostic rigid gas permeable lens trial. He had a history of trauma with subsequent cataract extraction with residual irregular astigmatism and traumatic mydriasis. XtraFocus Pinhole intraocular lens (Morcher) was implanted in the left eye and the vision improved to 20/40. Postoperatively, the patient experienced significant floaters which persisted to the extent of necessitating explantation of implant.

[Clinico-physiologic characteristics of brain stem disorders in comatose states].

Informative signs for classification of disorders of consciousness were distinguished on the basis of clinico-physiological comparisons. Coma is characterized by failure of attempts to wake the patient, deep coma by muscular atonia, and protective inhibition coma by bilateral mydriasis. Opening of the eyes in response to a sound or pain is characteristic for the apallic syndrome, carrying out of instructions for deep stunning, answering questions for stunning. Clear consciousness is characterized by the patient's ability to orientate. Protective inhibition coma is always characterized by coars…

[Voluntuary stramonium poisoning. 2 clinical cases].

Two cases of acute voluptuary poisoning by infusions of dry leaves of stramonium are reported. Symptomatology was characterized by confusion and hallucination (visual hallucinations, loss of space-time relationships, etc.) associated with neurovegetal symptomatology ascribable to the active principles contained in the preparation used: mydriasis, disturbances in visual accomodation, dryness of the fauces. Tachycardia was observed in one case, bradycardia in the other. Symptoms were resolved in one case by administration of neuroleptics, and spontaneously in the other after about 12 hours. A br…

Ocular manifestations of acute pandysautonomia.

A 33-year-old white man developed premature presbyopia and anisocoria as initial manifestations of acute pandysautonomia. Nine months later signs and symptoms of generalized, severe autonomic dysfunction developed, and six years later only paralysis of pupillary reactions, presbyopia, and orthostatic hypotension were unresolved. Pharmacologic testing of the pupils demonstrated no mydriasis to cocaine 4% or hydroxyamphetamine 1% and hypersensitivity to epinephrine 0.1%, methacholine 2.5%, and pilocarpine 0.0625%, suggesting the presence of sympathetic and parasympathetic, postganglionic blockag…

Can Pupil Changes Detect Autonomic Dysregulation After Spinal Cord Injury? A Systematic Review.

OBJECTIVES: The autonomic neural circuitry of the pupil makes it a unique target for identifying impaired autonomic regulation following spinal cord injury (SCI). This systematic review characterizes the evidence on pupillary changes after SCI. METHODS: A systematic search was conducted in MEDLINE, Embase, and CINAHL. Articles were independently assessed by 2 reviewers and included studies reporting on pupillary changes (e.g., miosis, mydriasis, anisocoria) following traumatic or nontraumatic SCI at any level. Data on study design, patient demographics, injury characteristics, and pupillary c…

Generalized Convulsive Status Epilepticus Secondary to Jarisch-Herxheimer Reaction in Neurosyphilis: A Case Report and Literature Review.

INTRODUCTION: Jarisch-Herxheimer reaction (JHR) is defined as a transient immunologic reaction, classically seen in patients during syphilis treatment. JHR is a rare cause of status epilepticus (SE) in neurosyphilis. We describe a case of generalized convulsive SE (GCSE) probably secondary to JHR in a patient with neurosyphilis. CASE REPORT: A 23-year-old man presented with progressive behavioral changes with 4 months of onset and pupillary alteration with 9 months of onset. He had experienced blurred vision and had noticed mydriasis in the left eye. Brain magnetic resonance imaging was norma…
