Revascularization Strategies in ST-Segment Elevation Myocardial Infarction With Multivessel Disease: Temporal Trends, Patient Profiles, and Outcomes.

BACKGROUND: Randomized trials support angiographic and physiology-guided complete revascularization (CR) vs incomplete revascularization (IR) in ST-segment elevation myocardial infarction (STEMI) with multivessel disease (MVD). The implementation of these strategies in clinical practice is uncertain. OBJECTIVES: In patients undergoing percutaneous coronary intervention for STEMI with MVD, we assessed temporal trends in the utilization of different revascularization strategies and associated outcomes. METHODS: We included 20,131 patients from the SWEDEHEART (Swedish Web-system for Enhancement

Health-Status Outcomes in Older Patients With Myocardial Infarction: Physiology-Guided Complete Revascularization Versus Culprit-Only Strategy.

BACKGROUND: The FIRE trial (Functional Assessment in Elderly Myocardial Infarction Patients With Multivessel Disease) enrolled 1445 older (aged ≥75 years) patients with myocardial infarction and multivessel disease in Italy, Spain, and Poland. Patients were randomized to physiology-guided complete revascularization or treatment of the only culprit lesion. Physiology-guided complete revascularization significantly reduced ischemic adverse events at 1 year. This prespecified analysis investigated the changes between the 2 study groups in angina status, quality of life, physical performance, and

Standard versus rapid-deployment aortic valve replacement and concomitant myocardial revascularization: 5-year bi-centre clinical outcomes.

OBJECTIVES: Clinical outcomes of 2 generations of pericardial bioprostheses in concomitant aortic valve and coronary artery bypass graft surgery were analysed. METHODS: Patients were recruited from 2 European centres and divided into 2 groups based on the type of aortic bioprosthesis used: Edwards Intuity Elite™ rapid-deployment (RD) bioprostheses or standard Edwards Magna Ease (ME). A propensity score weighting approach was used for data analysis. RESULTS: A total of 285 patients were included: 144 (50.5%) in the RD group and 141 (49.5%) in the ME group. Thirty-day mortality was 2.8% (RD) a

Assessment of optimal renin-angiotensin-system inhibition strategy in Asian patients with STEMI after primary myocardial revascularization.

BACKGROUND: For the Asian patients with STEMI undergoing PCI, ACEIs are known to have a better outcome than ARBs. However, there is limited evidence to suggest so. METHODS: Among the STEMI registry consist of 1142 Korean patients, we compared the MACE, the composite of myocardial infarction, stoke, death, admission for heart failure, and target vessel revascularization, between the ACEI and ARB groups (Set 1). Further, we defined adequate medication as the administration of a dose equal to or higher than the initiation dose of ACEI according to the heart failure guideline recommendation with

Transplantation of telomerase/myocardin-co-expressing mesenchymal cells in the mouse promotes myocardial revascularization and tissue repair.

AIM: Cell therapies are hampered by poor survival and growth of grafts. We tested whether forced co-expression of telomerase reverse transcriptase (TERT) and myocardin (MYOCD) improves post-infarct revascularization and tissue repair by adipose tissue-derived mesenchymal stromal cells (AT-MSCs). METHODS AND RESULTS: We transplanted AT-MSCs overexpressing MYOCD and TERT in a murine model of acute myocardial infarction (AMI). We characterized paracrine effects of AT-MSCs. When transplanted into infarcted hearts of C57BL/6 mice, AT-MSCs overexpressing TERT and MYOCD decreased scar tissue and the

Comparison of long-term clinical outcomes between revascularization versus medical treatment in patients with silent myocardial ischemia.

BACKGROUND: There have been limited and conflicting results regarding the prognostic impact of revascularization treatment on the long-term clinical outcomes of silent ischemia. The current study aimed to determine whether revascularization treatment compared with medical treatment (MT) alone reduces long-term risk of cardiac death of asymptomatic patients with objective evidence of inducible myocardial ischemia. METHODS: A total of 1473 consecutive asymptomatic patients with evidence of inducible myocardial ischemia were selected from a prospective institutional registry. All patients showed

Clinical outcomes in myocardial infarction and multivessel disease after a cardiac rehabilitation programme: Partial versus complete revascularization.

BACKGROUND: Current guideline recommendations encourage culprit vessel only percutaneous coronary intervention (PCI) in patients with ST-segment elevation myocardial infarction (STEMI) and multivessel disease. However, recent studies have shown a better clinical outcome in patients who receive multivessel PCI. AIM: To measure and compare clinical outcomes between partial revascularization (PR) versus complete revascularization (CR) in patients with STEMI and multivessel disease who underwent a cardiac rehabilitation programme. METHODS: We retrospectively reviewed the medical records of 282 p

Predischarge exercise electrocardiogram and stress echocardiography can predict long-term clinically driven revascularization following acute myocardial infarction.

BACKGROUND: Predischarge stress testing provides suboptimal prediction of spontaneous hard events following uncomplicated acute myocardial infarction (AMI). HYPOTHESIS: This study was aimed at assessing whether soft cardiac ischemic events requiring late revascularization could be predicted more accurately. METHODS: In all, 428 patients undergoing exercise electrocardiography (ECG) and stress echocardiography (SE, 345 dobutamine and 83 dypiridamole) within 15 days of uncomplicated AMI were followed up for 425 (range 20-2220) days. Improvement of myocardial function by trifluoperazine, a calmodulin antagonist, after acute coronary artery occlusion and coronary revascularization.

Activation of an intracellular calcium-calmodulin complex may play an important role in myocardial injury induced by ischemia and reperfusion. Trifluoperazine, a calmodulin antagonist, was used before ischemia to enhance myocardial preservation by preventing intracellular calcium accumulation. The experimental model used an isolated in situ pig heart (19 control animals and 15 trifluoperazine-treated animals) subjected to occlusion of the left anterior descending coronary artery for 60 minutes followed by 60 minutes of hypothermic potassium crystalloid cardioplegic arrest and 60 minutes of rep

Effect of prompt revascularization on outcomes in diabetic patients with stable ischemic heart disease and previous myocardial infarction in the Bypass Angioplasty Revascularization Investigation 2 Diabetes (BARI 2D) trial.

BACKGROUND: Survivors of a myocardial infarction (MI) are at a considerable risk of developing further cardiovascular events, including recurrent MI, heart failure, stroke, and death. Patients with type 2 diabetes mellitus and stable ischemic heart disease (SIHD) have worse outcomes than their nondiabetic counterparts, and those with previous MI may be at particularly high risk. Yet, little is known about the effect of adding prompt revascularization to intensive medical therapy in this high-risk group. PATIENTS AND METHODS: We carried out a post-hoc analysis of the Bypass Angioplasty Revascu

The influence of chronic kidney disease and age on revascularization rates and outcomes in acute myocardial infarction - a cohort study.

BACKGROUND: There is a paucity of data on the complex interaction between chronic kidney disease, age and its impact on management and outcomes in acute myocardial infarction. METHODS: A state based claims dataset that collects data on all hospitalizations (representing 32.3% of the Australian population) was used to identify all patients admitted with a principal diagnosis of acute myocardial infarction (ICD10 codes: I21.0-I21.4) over a four-year period. Patients were linked to the state death registry and followed until death or end of follow-up (31 December 2009). Chronic kidney disease wa

Increasing benefit from revascularization is associated with increasing amounts of myocardial hibernation: a substudy of the PARR-2 trial.

OBJECTIVES: We sought to determine: 1) whether F-18-fluorodeoxyglucose (FDG) positron emission tomography (PET) parameters identify high-risk patients who gain benefit from revascularization; 2) whether there is a cut point for such benefit; and 3) predictors of outcome in patients with severe left ventricular (LV) dysfunction due to coronary artery disease. BACKGROUND: Patients with ischemic LV dysfunction might benefit from revascularization but not without risk. The FDG PET imaging can detect viable myocardium that recovers after revascularization. In the PARR-2 (PET and Recovery Following

Aortic valve replacement combined with myocardial revascularization. Late results and determinants of risk for 471 in-hospital survivors.

Among 500 patients consecutively undergoing primary aortic valve replacement combined with coronary bypass grafting from 1967 to 1981, there were 29 (5.8%) perioperative deaths. Follow-up of the late survivors ranged from 43 to 181 months (mean 85 months); actuarial survival rates were 88%, 77%, and 52% and event-free survival rates were 80%, 65%, and 32% at 2, 5, and 10 postoperative years. Cox proportional hazard analyses were used to identify determinants of late risk. Patient-related variables associated with decreased late survival rates included advanced age, cardiothoracic ratio 50% or

FFR-Guided Complete versus Culprit-Only Revascularization in Patients With Myocardial Infarction: A Systematic Review and Meta-Analysis.

In patients with myocardial infarctions (MI) and multivessel coronary artery disease (CAD), the effect of fractional flow-reserve (FFR)-guided complete revascularization during index hospitalization versus culprit-only revascularization remains unclear. Our objective is to determine whether FFR-guided complete revascularization during index hospitalization reduces major adverse cardiac events (MACE) among patients with MI and multivessel CAD. We systematically searched MEDLINE, EMBASE, and the Cochrane Library for randomized controlled trials (RCTs) comparing FFR-guided complete versus culprit

Acute ST-Elevation Myocardial Infarction Complicating Infective Endocarditis: A Case Report and Review of Reported Outcomes After Revascularization.

Acute myocardial infarction in the context of infective endocarditis is a dreadful complication. Various mechanisms have been proposed to explain this complication; embolization of fragments of valvular vegetations into the coronary tree is one possible mechanism. Early mortality with this complication is reportedly high. Moreover, outcomes after treatment, either with percutaneous or surgical intervention, are not well-described in the literature. We report a case of acute ST-elevation myocardial infarction complicating bacterial native aortic valve endocarditis. Despite aspiration of infecti

Factors associated with myocardial infarction among patients without prior history of coronary artery diseases following carotid artery revascularization.

BACKGROUND: Postoperative myocardial infarction (MI) contributes to the overall mortality and morbidity associated with carotid revascularization. Current guidelines regarding preoperative cardiac evaluation are limited to patients with a history of coronary artery disease (CAD). This study aimed to identify factors associated with postoperative MI in patients without a prior history of CAD undergoing carotid revascularization. METHODS: We performed a retrospective analysis of all patients undergoing carotid artery revascularization without a prior history of CAD in the VQI database from 2016

Right ventricular-pulmonary arterial coupling in patients with first acute myocardial infarction: an emerging post-revascularization triage tool.

BACKGROUND: The tricuspid annular plane systolic excursion/pulmonary artery systolic pressure (TAPSE/PASP) ratio is a non-invasive surrogate for right ventricular-pulmonary arterial (RV-PA) coupling, studied in chronic RV pressure overload syndromes. However, its prognostic utility in patients with acute myocardial infarction (AMI), which may cause acute RV pressure overload, remains unexplored. OBJECTIVE: This study aimed to determine predictors of RV-PA uncoupling in patients with first AMI and examine whether it could improve risk stratification for cardiovascular in-hospital mortality aft
