Narcolepsy and Other Central Hypersomnias.

PURPOSE OF REVIEW: This article focuses on the clinical presentation, pathophysiology, diagnosis, differential diagnosis, and management of narcolepsy type 1 and narcolepsy type 2, idiopathic hypersomnia, Kleine-Levin syndrome, and other central disorders of hypersomnolence, as defined in the International Classification of Sleep Disorders, Third Edition (ICSD-3). RECENT FINDINGS: In ICSD-3, the names of some central disorders of hypersomnolence have been changed: narcolepsy with cataplexy and narcolepsy without cataplexy have been renamed narcolepsy type 1 and narcolepsy type 2, respectively…

Narcolepsy during Childhood: An Update.

Narcolepsy type 1 (NT1) is a rare central disorder of hypersomnolence characterized by excessive daytime sleepiness, cataplexy, sleep paralysis, hallucinations, and fragmented nocturnal sleep usually arising in adolescence or young adulthood. Recently, the childhood NT1 diagnoses have increased for improved disease awareness and for several cases occurring after the H1N1 pandemic influenza or vaccination. As in adults, the occurrence of NT1 in individuals with a genetic predisposition of the immune system (e.g., human leukocyte antigen, HLA-DQB1*0602) together with the role of environmental tr…

Management of narcolepsy during pregnancy.

BACKGROUND: The management of pregnancy in patients with narcolepsy poses many questions regarding therapy, including the risk to the mother and fetus related to the disease, potential risks at the time of conception, the risk to both the mother and the fetus of the medications used to treat narcolepsy, and the risk to the infant from medications that might be secreted in breast milk. There are no detailed practice parameters on the treatment of narcolepsy patients during pregnancy. We surveyed narcolepsy specialists from around the world to determine their clinical approach to the management …

Disrupted nighttime sleep in narcolepsy.

STUDY OBJECTIVES: Characterize disrupted nighttime sleep (DNS) in narcolepsy, an important symptom of narcolepsy. METHODS: A panel of international narcolepsy experts was convened in 2011 to build a consensus characterization of DNS in patients with narcolepsy. A literature search of the Medline (1965 to date), Medline In-Process (latest weeks), Embase (1974 to date), Embase Alert (latest 8 weeks), and Biosis (1965 to date) databases was conducted using the following search terms: narcolepsy and disrupted nighttime sleep, disturbed nighttime sleep, fragmented sleep, consolidated sleep, sleep …

Neuropsychological findings in childhood narcolepsy.

Narcolepsy with cataplexy is a severely disabling disorder very often arising in childhood. Data on neuropsychological impairment in children are scant. We administered standardized neuropsychological tests to 13 children with narcolepsy with cataplexy. Overall, our patients displayed multiple patterns of cognitive and behavioral dysfunction, and often academic failure (7 cases out of 13). All children had a normal full intelligence quotient (IQ), but 3 patients presented a significantly higher and 2 a significantly lower Verbal IQ compared to Performance IQ, respectively. Mean sleep latency w…

Obstructive sleep apnea in narcolepsy.

STUDY OBJECTIVES: Narcolepsy and obstructive sleep apnea syndrome (OSAS) are two conditions associated with excessive daytime sleepiness (EDS). They may coexist in the same patient but the frequency of this association and its clinical significance is unknown. The presence of obstructive sleep apnea (OSA) in a narcoleptic patient may interfere with the diagnosis of narcolepsy. The aim of the study was to determine the prevalence of OSA in narcolepsy. DESIGN AND SETTING: University hospital sleep clinic series of narcoleptic patients diagnosed with nocturnal polysomnography and multiple sleep …

Amygdala dysfunction in narcolepsy-cataplexy.

The blink reflex of acoustic startle reflex (ASR) is modulated by emotions and a loss of physiological aversive ASR potentiation is reported in humans following amygdala lesions. Patients with narcolepsy-cataplexy (NC) were found to have normal ASR, but they failed to exhibit startle potentiation during unpleasant stimuli. This absence of aversive ASR potentiation gives support to the hypothesis of an amygdala dysfunction in human NC.

[Hypocretins, peptides associated with narcolepsy].

Narcolepsy is a chronic disease characterized by excessive somnolence, abrupt transitions from wakefulness to rapid eye movement sleep stage and cataplexy. Experimental evidence show that narcolepsy in humans is a neurodegenerative disease associated with the lost of hypocretin (HCRT) neurons in the lateral hypothalamus. Narcoleptic patients also display a significant diminution in HCRT contents of cerebrospinal fluid. In order to study narcolepsy, several experimental models have been developed. Murine and canine models currently allow us to study this disease. Our laboratory has developed a …

Secondary narcolepsy following neurocysticercosis infection.

The authors describe a case of secondary narcolepsy resulting from neurocysticercosis infection. Neurocysticercosis is a common central nervous system parasitic disease endemic to Central and South America caused by ingestion of eggs from a host with intestinal taeniasis. Neurological deficits depend on lesion location. Our case developed a hypothalamic cyst causing severe sleepiness and sleep-onset rapid eye movement periods on both polysomnography and multiple sleep latency testing. The cerebrospinal fluid hypocretin-1 level was normal indicating secondary narcolepsy can result without failu…

TNFA promoter polymorphisms and narcolepsy.

BACKGROUND/PURPOSE: Vitamin D3 plays important roles in the absorption of calcium and phosphorus from the gastrointestinal tract and in the treatment of rickets; in addition, it facilitates the deposition of minerals in bones, thus minimizing the possibility of developing osteomalacia. Sunlight naturally induces vitamin D3 photosynthesis. Such a process is affected by a number of factors such as age, geographical location, skin color, sunscreen application and clothing. It is intended in the present investigation to study in vitro the effect of clothing on the solar photoproduction of vitamin

Hypocretin/orexin, sleep and narcolepsy.

The discovery that hypocretins are involved in narcolepsy, a disorder associated with excessive daytime sleepiness, cataplexy and unusually rapid transitions to rapid-eye-movement sleep, opens a new field of investigation in the area of sleep control physiology. Hypocretin-1 and -2 (also called orexin-A and -B) are newly discovered neuropeptides processed from a common precursor, preprohypocretin. Hypocretin-containing cells are located exclusively in the lateral hypothalamus, with widespread projections to the entire neuroaxis. Two known receptors, Hcrtr1 and Hcrtr2, have been reported. The f…

Neuronal degeneration in canine narcolepsy.

Narcolepsy is a lifelong illness characterized by persistent sleepiness, hypnagogic hallucinations, and episodes of motor paralysis called cataplexy. We have tested the hypothesis that a transient neurodegenerative process is linked to symptom onset. Using the amino-cupric silver stain on brain sections from canine narcoleptics, we found elevated levels of axonal degeneration in the amygdala, basal forebrain (including the nucleus of the diagonal band, substantia innominata, and preoptic region), entopeduncular nucleus, and medial septal region. Reactive neuronal somata, an indicator of neuron…

Pontine lesions in idiopathic narcolepsy.

Three patients had longstanding (37 to 50 years), highly disabling narcolepsy, poorly controlled by treatment. The clinical histories were typical, consisting of sleepiness, cataplexy, sleep paralysis, hypnagogic hallucinations, disturbed nocturnal sleep, and HLA-DR2 tissue typing. Polygraphic findings confirmed the diagnosis. Neurologic examination, spinal fluid, and evoked potentials were normal. On MRI scanning, all three patients showed overlapping bilateral and symmetric brainstem T2 hyperintensities circumscribed to the ventrolateral aspect of the midrostral pons. The nature of the lesio…
