Barriers to early diagnosis of cervical cancer: a mixed-method study in Côte d'Ivoire, West Africa.

BACKGROUND: Cervical cancer, a major public health problem in many developing countries, is usually associated with a poor survival related to an advanced disease at diagnosis. In Côte d'Ivoire and other developing countries with high cervical cancer prevalence, little is known about factors associated with advanced cervical cancer stages in a context of limited access to screening services. METHODS: From May to July 2019, we conducted a cross-sectional study using a mixed, quantitative and qualitative method. Information on socio-demographic and history of the disease was extracted from a ra

Effects of a mucoadhesive phytomedicine (Curcuma longa L. and Bidens pilosa L.) on radiotherapy-induced oral mucositis and quality of life of patients undergoing head and neck cancer treatment: randomized clinical trial.

PURPOSE: To assess the effect of a mucoadhesive herbal medicine containing curcuminoids and a glycerinated extract of Bidens pilosa L. (FITOPROT) in association with photobiomodulation (PBM) therapy and a Preventive Oral Care Program (POCP) compared to PBM and POCP in the treatment of radiotherapy (RT)-induced oral mucositis (ROM) and in the quality of life of these patients. METHODS: A double-blind clinical trial was performed with head and neck cancer patients undergoing RT or chemoradiotherapy. Participants were randomized into two groups: Group 1 (n=27): PBM and POCP; and Group 2 (n=25):

Characterization and genotype-phenotype correlation of patients with Fanconi anemia in a multi-ethnic population.

Fanconi anemia (FA), an inherited bone marrow failure (BMF) syndrome, caused by mutations in DNA repair genes, is characterized by congenital anomalies, aplastic anemia, high risk of malignancies and extreme sensitivity to alkylating agents. We aimed to study the clinical presentation, molecular diagnosis and genotype-phenotype correlation among patients with FA from the Israeli inherited BMF registry. Overall, 111 patients of Arab (57%) and Jewish (43%) descent were followed for a median of 15 years (range: 0.1-49); 63% were offspring of consanguineous parents. One-hundred patients (90%) had

Reasons for truly negative cytology reports preceding the diagnoses of invasive cervical cancer-Results of a false-negative cytology audit in Polish Cervical Cancer Screening Programme.

BACKGROUND: False-negative (FN) results in cervical cancer (CC) screening pose significant risk for participants and should be audited. The aim of the study was to analyse the results of audit of FN slides collected in 2010-2013 in Polish Cervical Cancer Screening Program (CCSP) and to seek for risk factors of obtaining true-negative result (TN; not containing abnormal cells as confirmed in audit) before CC diagnosis. METHODS: Screening database was merged with National Cancer Registry to identify negative slides preceding histologically confirmed CC diagnosis up to 42 months. Two blinding sl

Kidney replacement therapy as a key performance indicator for population health. A call to action by REDYT, ALCER, EKPF, S.E.N. and RICORS2040-renal.

The 2023 Spanish Registry of Dialysis and Transplantation (REDYT) real data exceeded previous estimates and forecasts for the prevalence of kidney replacement therapy (KRT). This trend will continue as the baby boom generation reaches the age of peak incidence of chronic kidney disease (CKD) and KRT. REDYT disclosed heterogeneity in clinical practice regarding initiation of KRT in the very elderly across Autonomous Communities, which should lead to benchmarking studies that dissect the drivers and outcomes. REDYT also disclosed an inverse correlation between KRT incidence/prevalence and life e

A nationwide study on the adoption and short-term outcomes of transanal total mesorectal excision in the UK.

BACKGROUND: The interest and adoption of transanal total mesorectal excision (TaTME) is growing worldwide, but evidence on nationwide implementation and short-term outcomes is scarce. This study aims to evaluate national results for this relatively new technique in the UK. METHODS: All TaTME procedures performed in the UK and recorded on the international TaTME registry between January 2013 and January 2018 were analyzed. Surgeons who received training on TaTME in the UK were sent a survey regarding their experience with implementation of TaTME in their unit. Primary endpoint was a composite

What Does Palliative Care Delivery Look Like?: A Retrospective Claims-Based Analysis.

CONTEXT: Timely palliative care (PC) is a marker of high-quality care for patients with poor-prognosis cancer, yet real-world delivery falls short of guideline-concordant care. Clinician type is an underexplored factor. OBJECTIVES: This study characterizes PC delivery by clinician type among decedents with poor-prognosis cancer at hospitals with PC programs. METHODS: Using Medicare claims linked to the National Palliative Care Registry, we identified PC encounters in the last six months of life among decedents with poor-prognosis cancer. Billing clinicians were classified into team-based phy

Study protocol for a multi-site randomized clinical trial of an intervention program to promote self-management among head and neck cancer survivors with lymphedema and fibrosis [PROMISE trial].

BACKGROUND: Lymphedema and fibrosis (LEF) secondary to head and neck cancer (HNC) treatment is a chronic process and a lifelong commitment to self-care. Although prospective data is lacking, a program of self-management may reduce LEF progression and minimize associated functional impairments and symptom burden. Barriers to long-term LEF self-care include lack of standardized self-care protocols as well as lack of LEF-related knowledge, skills, self-efficacy and adherence among HNC survivors. The objective of this study is to evaluate the effectiveness of a standardized lymphedema and fibrosis

Asbestos in High-Risk Communities: Public Health Implications.

Asbestos-related diseases (ARDs)-mesothelioma, lung cancer, and asbestosis-are well known as occupational diseases. As industrial asbestos use is eliminated, ARDs within the general community from para-occupational, environmental, and natural exposures are more prominent. ARD clusters have been studied in communities including Broni, Italy; Libby, Montana; Wittenoom, Western Australia; Karain, Turkey; Ambler, Pennsylvania; and elsewhere. Community ARDs pose specific public health issues and challenges. Community exposure results in higher proportions of mesothelioma in women and a younger age

Measuring effectiveness of the cervical cancer vaccine in an Australian setting (the VACCINE study).

BACKGROUND: The quadrivalent human papillomavirus vaccine has been provided in Australia through the National Human Papillomavirus Vaccination Program since April 2007. National registry data demonstrates good coverage of the vaccine, with 73% of school-aged girls having received all three doses. To evaluate the effectiveness of the program, we propose a two-pronged approach. In one (sub study A), the prevalence of the vaccine-targeted human papillomavirus genotypes in a population cohort is being estimated, and will be analysed in relation to vaccination status, cervical cytology screening st
