We describe the case of a 72-year-old man with chronic kidney disease (CKD) 3b due to hypertensive nephrosclerosis, noted to have persistent hypophosphatemia and mildly elevated fibroblast growth factor-23 (FGF-23). His serum phosphorus level was chronically low, averaging 1.6 mg/dL, despite taking six to eight 250 mg packets of an oral elemental phosphate supplement daily. His parathyroid hormone (PTH) level was 204 pg/mL (normal range: 10-65 pg/mL), and his serum alkaline phosphatase (ALP) level was within the normal range at 55 U/L. His fractional excretion of phosphate was found to be 59%,…

Diabetic nephropathy (DN) is a leading cause of end-stage renal disease (ESRD). Hypertension increases kidney stress, which deteriorates function, and leads to peripheral renal vascular resistance. Long-term hypoperfusion promotes interstitial fibrosis and glomerular sclerosis, resulting in nephrosclerosis. Although hypertension and DN are frequent ESRD complications, relevant animal models remain unavailable. We generated a deoxycorticosterone acetate (DOCA)-salt hypertensive uni-nephrectomized (UNx) KKAy mouse model demonstrating hypertension, hyperglycemia, cardiac hypertrophy, kidney failu…

A review of 693 renal transplant recipients revealed 77 (11%) in whom persistent, heavy proteinuria (greater than 2 g/24 hr) developed. Renal histology was available in all 77 patients. Twenty-one patients had received kidneys from living-related donors, the remaining 56 from cadaveric donors. The cause of proteinuria in these 77 patients was as follows: transplant glomerulopathy (30), allograft glomerulonephritis (22), chronic rejection (21), renal vein thrombosis (2), diabetic glomerulosclerosis (1), and hypertensive nephrosclerosis (1). Of the 22 patients who developed glomerulonephritis in…

Essential hypertension is a clinical diagnosis based on the presence of an elevated systemic blood pressure on physical examination without a clear inciting cause. It has multiple etiologies and is not a homogeneous disorder. Hypertension contributes to the development and progression of atherosclerotic cardiovascular diseases, and antihypertensive treatment reduces the risk of fatal and nonfatal myocardial infarction, stroke, and congestive heart failure. Although hypertension is frequently present in nondiabetic individuals with low levels of proteinuria and chronic kidney disease, reducing

Surgical nephrectomy is a procedure that has been performed for nearly 100 years. In the presence of a normal contralateral kidney, such as in a renal transplant donor or child with Wilms tumor, it is a benign procedure without deleterious consequences on the remaining kidney. However, many adults and some children postnephrectomy will develop chronic kidney disease. The non-neoplastic kidney in tumor resections may harbor a large number of developmental and acquired diseases predictive of this outcome or that convey other medically significant information. Examination of the non-neoplastic ki

The authors share their eight-and-a half-year experience in the treatment of 136 patients with terminal chronic renal insufficiency (CRI), treated via programmed hemodialysis. The programmed hemodialysis was established to considerably prolong the lives of the patients with terminal CRI. Many of them were rehabilitated to a state, making them feel complete citizens. The authors established that the patients with Balkan endemic nephropathy, as compared with those with chronic pyelo-nephritis and glomerulonephritis were adapted and rehabilitated far faster. It was also established that the earli

Wnt/β-catenin signaling is essential in the pathogenesis of renal fibrosis. We previously reported inhibition of the Wnt O-acyl transferase porcupine, required for Wnt secretion, dramatically attenuates kidney fibrosis in the murine unilateral ureteral obstruction model. Here, we investigated the tissue-specific contributions of porcupine to renal fibrosis and inflammation in ureteral obstruction using mice with porcupine deletion restricted to the kidney tubular epithelium or infiltrating myeloid cells. Obstruction of the ureter induced the renal mRNA expression of porcupine and downstream ta

The lack of uniform diagnostic criteria and effective biomarkers for hypertensive nephrosclerosis makes early detection challenging. This study aimed to explore renal dysfunction biomarkers in hypertensive patients to identify early renal injury. The current study enrolled 106 hypertensive patients divided into 3 groups according to their urine microalbumin/creatinine ratio (ACR) to investigate the association between tubular injury biomarkers and early renal impairment. The serum levels of human and animal models were measured by ELISA, and the statistical analysis was conducted by SPSS. The d…

BACKGROUND Acute oxalate nephropathy (AON) is a rare complication of diabetes mellitus (DM), resulting in kidney damage. This report describ … hemodialysis was commenced. A biopsy was performed to establish the cause, which revealed diabetic nephrosclerosis and calcium oxalate crystals in the tubules. A detailed review of the patient’s medical history was conducted, yet no definitive factor emerged. We inferred that diabetes triggered acute oxalate nephropathy due to increased oxalate excretion, …

Non-diabetic chronic kidney disease (ND-CKD) refers to the progressive and irreversible decline in kidney function occurring in the absence of diabetes mellitus-a distinction that sets it apart from the more prevalent diabetic CKD. While diabetic nephropathy remains the leading cause of CKD globally, ND-CKD encompasses a heterogeneous group of etiologies, including hypertensive nephrosclerosis, glomerulonephritis, and interstitial nephritis. Its incidence and prevalence are steadily increasing, particularly in aging populations, and are often underrecognized. Importantly, ND-CKD is not a benig…
