Neurophysiological features of primary lateral sclerosis.

Primary lateral sclerosis (PLS) is a motor neuron disease characterized by spinobulbar spasticity, absence of progressive lower motor neuron (LMN) dysfunction and marked by a slow functional decline. Electromyography is essential to exclude significant LMN involvement, particularly in the context of distinguishing PLS from amyotrophic lateral sclerosis (ALS), given that the prognosis is substantially better, and respiratory complications are unusual, in PLS. Nevertheless, minor neurogenic changes and occasional fasciculation potentials can be observed in PLS. The most useful technique for the

Corticobasal syndrome: neuroimaging and neurophysiological advances.

Corticobasal degeneration (CBD) is a neurodegenerative condition characterized by 4R tau protein deposition in several brain regions that clinically manifests itself as a heterogeneous atypical parkinsonism typically expressed in adulthood. The prototypical clinical phenotype of CBD is corticobasal syndrome (CBS). Important insights into the pathophysiological mechanisms underlying motor and higher cortical symptoms in CBS have been gained by using advanced neuroimaging and neurophysiological techniques. Structural and functional neuroimaging studies often show asymmetric cortical and subcorti

Intraoperative neurophysiological monitoring in spinal surgery.

Recently, many surgeons have been using intraoperative neurophysiological monitoring (IOM) in spinal surgery to reduce the incidence of postoperative neurological complications, including level of the spinal cord, cauda equina and nerve root. Several established technologies are available and combined motor and somatosensory evoked potentials are considered mandatory for practical and successful IOM. Spinal cord evoked potentials are elicited compound potentials recorded over the spinal cord. Electrical stimulation is provoked on the dorsal spinal cord from an epidural electrode. Somatosensory

[The endophenotype concept in the neurophysiological studies of schizophrenia].

The endophenotype concept is considered as theoretical basis for the study of pathobiological mechanisms of schizophrenia and for the development of the complex of instrumental diagnostic methods. The contemporary state of the problem and the most significant results of the study of three neurophysiological schizophrenia endophenotypes are presented in the review: P50 auditory evoked potential suppression, prepulse inhibition of the startle reflex and antisaccade task. The current understanding of the underlying neurophysiological and neurochemical mechanisms is described for each measure. The

[Mixed depressions: clinical and neurophysiological biomarkers].

Epidemiological studies of major depressive episodes (MDE) highlighted the frequent association of symptoms or signs of mania or hypomania with depressive syndrome. Beyond the strict definition of DSM-IV, epidemiological recognition of a subset of MDE characterized by the presence of symptoms or signs of the opposite polarity is clinically important because it is associated with pejorative prognosis and therapeutic response compared to the subgroup of "typical MDE". The development of DSM-5 took into account the epidemiological data. DSM-5 opted for a more dimensional perspective in implementi

Neurophysiological background for physical therapies in fibromyalgia.

This paper describes the techniques for controlling pain by the physical means that are most widely used clinically, particularly in the case of fibromyalgia. They are grouped on the basis of the physical energy used: mechanical, thermal (including magnetic and electromagnetic), and light (LASER). The main underlying neurophysiological mechanisms are gate activation, the stimulation of descending systems of pain control, and the endogenous opiate system.

Emerging neurophysiological specialization for letter strings.

In adult readers, printed words and other letter strings activate specialized visual functions within 200 msec, as evident from neurophysiological recordings of brain activity. These fast, specialized responses to letter strings are thought to develop through plastic changes in the visual system. However, it is unknown whether this specialization emerges only with the onset of word reading, or represents a precursor of literacy. We compared 6-year-old kindergarten children who could not yet read words to adult readers. Both age groups detected immediate repetitions of visually presented words,

Neurophysiological diagnosis of acquired sensory ganglionopathies.

We examined 29 patients with chronic progressive ganglionopathy of different etiology. Neurophysiological abnormalities were dominated by a widespread decrease in sensory nerve action potential amplitudes, which involved both upper and lower limb nerves, even in patients with asymmetrical or patchy clinical presentation. This impairment of sensory nerve conduction, reflecting a nonlenght-dependent pattern of peripheral axon degeneration, should be considered the hallmark of ganglionopathies. The evidence of central sensory pathway impairment, which allows to localize the pathology to the dorsa

Neurophysiological approach to primary headache pathophysiology.

Electroencephalography (EEG), evoked potentials (EP), and electromyography (EMG) techniques are useful tools in the clinical assessment of headache and in understanding the pathophysiological mechanisms of these pathologies. EEG and EP studies in migraine revealed functional abnormalities in cortical electrical activity and in sensory processing. EMG studies resulted in pain syndromes involving nerves or myofascial structures such as tension-type headache and cluster headache. Moreover, it was possible to test the effect of old and new drugs with the help of these neurophysiological techniques

Neurophysiologic study of beta-thalassemia patients.

Neurophysiologic investigations were performed in 34 Chinese patients with beta-thalassemia major maintained on long-term desferrioxamine treatment to look for subclinical toxicity in the auditory, visual, peripheral, or central neural pathways. In the auditory pathway study, four patients (12%) had mild sensorineural hearing impairment. Two patients (6%) had increased P 100 latencies in the visual evoked potential study, and nine patients (26%) had abnormal electroretinogram results. All had normal electrooculograms. Ophthalmoscopic examination was abnormal in three patients (9%), and three (

[Polyneuropathy in hyperthyroidism--a clinical neurophysiologic study].

In this prospective study, 27 patients with hyperthyroidism were evaluated neurophysiologically. There were 24 women and 3 men: the mean age was 56.4 years (range 21 to 84 years). Neurologic complaints of the patients were muscle weakness (59.3%), muscle cramps (18.5%) and sensory symptoms (11.1%). Tendon reflexes were increased in 18.5% and decreased in 14.8%, respectively. While the motor nerve conduction velocities (NCV) of the tibial and the peroneal nerve revealed normal results in the majority of patients, there were pathological findings of the sensory NCV of the sural nerve in 29.6% of

Neurophysiologic monitoring of patients with head injuries.

Despite new technologic developments designed to analyze the brain's electrical activity, monitoring the electroencephalogram or evoked potentials has not yet provided important information with regard to acute management of patients with head injury. Measurement of cerebral blood flow as well as jugular oxygen saturation is of more practical importance. Particularly when monitored continuously, these measures can provide useful information about the use of hyperventilation and control of elevated systemic arterial pressure.

[Acupuncture analgesia. Neurochemical and neurophysiologic aspects].

In some parts of the world, acupuncture has been employed as a method of obtaining analgesia for thousands of years without the mechanism of its action being understood. During the past two decades, evidence has accumulated indicating that acupuncture activates an intrinsic neural network which monitors and modifies the activity of pain-transmitting neurons. The in-suppressing action is partly mediated by endogenous opioid peptides and monoamines. The system is organized at three levels of the neuroaxis: spinal cord, medulla and the midbrain. The raphe magnus nucleus and the spinal cord consti

Neurophysiological characteristics of spinal arteriovenous malformations.

The aim of the study was to identify the clinical and neurophysiological pattern of deficits in spinal arteriovenous malformations (AVM) to allow an early diagnosis which is the prerequisite for effective treatment by early surgery or embolization. Among 26 patients with myelographic signs of a spinal AVM, selective spinal angiography disclosed 20 cases with a dural AV-fistula (dAVF) and 6 patients with an intradural AV-malformation (iAVM). Although the main pathogenetic factor in both disorders may be different, clinical and neurophysiological findings proved to be of limited value in differe

[Neurophysiologic and neurochemical principles of the effect of anticonvulsants].

This article surveys neurophysiological and neurochemical findings on the mechanism of action of clinically useful antiepileptics. Two mechanisms appear to be particularly important: reducing the hyperexcitability of the cell membranes by a direct action at ion channels; changes in synaptic transmission by means of an intervention in neurotransmitter systems; it seems that in this regard an important neurotransmitter is the inhibitory gamma-aminobutyric acid GABA. With these mechanisms as basis, antiepileptics can be classified into several groups. In anticonvulsively effective concentrations,

[From movements to tics: neurophysiologic aspects].

The definition and classification of tics is still based on clinical data. Furthermore, in Gilles de la Tourette syndrome, organogenetic and psychogenetic determinants are still opposed and the true automatic involuntary nature of tics is still debated. Neurophysiological assessments can provide objective data, useful in analysing the physiopathology of tics. Simple tics consist of short bursts of EMG activity (lasting usually less than 100 ms) whereas complex tics consist of prolonged EMG activity. In both cases, the reciprocal activation of antagonist muscles may be present or absent. EEG re
