Newborn hypoglycemia.

Neonatal hypoglycemia is a common problem encountered by both term and preterm infants. It can be either symptomatic or asymptomatic. Prolonged hypoglycemia may result in permanent neurologic impairment and death. Definitions of hypoglycemia vary and remain controversial. Underlying causes of hypoglycemia include (a) inadequate glucose production; (b) increased glucose utilization; (c) abnormalities of endocrine regulation; and (d) other causes. Nurse practitioners must be aware of infants at risk for hypoglycemia so they can provide appropriate interventions. In this article the clinical char

Clinical practices among healthcare professionals concerning neonatal jaundice and pale stools.

UNLABELLED: Jaundice and pale stools are major indicators of neonatal liver disease. Prognosis depends on timely diagnosis and management. We evaluated the clinical practices among healthcare professionals concerning jaundiced newborns and their ability to recognize pale stools. We supplied a questionnaire and a panel with eight photographs of stools, both locally validated, to physicians and nurses of the National Healthcare Service. Analysis was conducted according to professional status, specialization and years of experience of professionals and level of healthcare. Questionnaires were adm

Association of outcomes with organizational characteristics of neonatal intensive care units.

OBJECTIVE: Health outcomes may be influenced by organizational and management characteristics. We hypothesized that neonatal intensive care unit managerial practices and organizational processes could affect outcomes of births 500-1499 g, and we assessed this in eight neonatal intensive care units. DESIGN: Multiple-center cohort. SETTING: Eight acute care neonatal intensive care units in Washington, DC. PATIENTS: Infants weighing 500-1499 g, born from October 1, 1994, to February 19, 1997, to a resident of Washington, DC. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: Outcomes includ

The Utility of Simulation-Based Training in Teaching Frontline Providers Modified Sarnat Encephalopathy Examination: A Randomized Controlled Pilot Trial.

BACKGROUND: Limited training in targeted neurological examination makes it challenging for frontline providers to identify newborns with perinatal asphyxia eligible for therapeutic hypothermia. This training is important in the era of telemedicine, where the experts can remotely guide further care of these newborns. METHODS: This randomized controlled pilot study was conducted in a South Indian tertiary hospital. Neonatal nurses, who had no previous hands-on experience in MSEE, were trained in modified Sarnat staging by a didactic teaching session using online teaching module. The nurses were

Lung Ultrasound as a New Technique for Diagnosis of Neonatal Respiratory Diseases.

Point-of-care ultrasound in the NICU is becoming more commonplace and is now used for a number of indications. Over the past ten years, the use of ultrasound as an alternative to a chest x-ray for the diagnosis of neonatal lung disease has been explored, and protocols were developed to refine the interpretation of ultrasound images in neonatal lung disease. The purpose of this column is to briefly explain the physics of ultrasound and describe the application of ultrasound to neonatal lung assessment.

Caring for children with complex needs.

This article describes the role of the community neonatal nurse in the discharge of infants with chronic lung disease (CLD), or bronchopulmonary dysplasia. It also explores the use of a common assessment framework in the assessment of such children and development of a nursing care plan. The article includes a case study to illustrate the link between CLD and other diseases, and emphasises the importance of focusing on holistic care from admission, in the neonatal unit and at discharge.

Sixty golden minutes.

The golden hour concept started in the trauma setting but is becoming more familiar in the neonatal intensive care unit (NICU). For a premature baby, the first hour of life can make the difference between a good outcome, a poor outcome, and death. The golden hour is 60 minutes of team-oriented and task-driven protocols. The focus is on resuscitation, thermoregulation, early administration of antibiotics for suspected sepsis, early intravenous parenteral nutrition, hypoglycemia management, and completed admission within one hour of life. To a premature baby, the first 60 minutes of life are gol

High inter-rater reliability between physicians and nurses utilising modified Downes' scores in preterm respiratory distress.

AIM: To assess the inter-rater reliability of modified Downes' scores assigned by physicians and nurses in the Ethiopian Neonatal Network and to calculate the concordance of score-based treatment for preterm infants with respiratory distress. METHODS: We included preterm infants admitted from June 2020 to July 2021 to four tertiary neonatal intensive care units (NICUs) of the Ethiopian Neonatal Network that presented with respiratory distress. We calculated the kappa statistic to determine the nurse and physician correlation for each component of the modified Downes' score and total score on

Perinatal cocaine use: maternal, fetal, and neonatal effects.

The implications of cocaine use during pregnancy include an increased incidence of stillbirths, abruptio placentae, and an increased risk of other delivery complications. The neonate born to a woman using cocaine may show a lack of definite physical stigmata, absence of consistent withdrawal patterns, and high incidence of irritability during the neonatal period. Nursing implications for promoting optimal pregnancy and neonatal outcome primarily involve early pregnancy intervention, consistent care, education, and delivery management. During the neonatal period, the nurse should work to reduce
