[Advance of screening keratoconus before refractive surgery with machine learning].

Artificial intelligence (AI) has made important progress in image recognition and disease prognosis prediction in recent years. Along with the development of computer technology, the application scope of AI in the field of ophthalmology is expanding. Keratoconus screening is an important means to determine the indication of refractive surgery and avoid postoperative corneal ectasia, but the accuracy of traditional diagnostic methods is low, especially for subclinical keratoconus. Machine learning, a method to realize artificial intelligence, makes it possible to improve the accuracy of keratoc

Comparison of Two Conjunctival Incision Techniques in Strabismus Operations: Analysis of the Patient Population in 2008, 2010 - 2014.

BACKGROUND: Until 2010, the Halle university hospital used the limbal approach in strabismus operations to open the conjunctiva, as first described by Harms in 1949. In 2010, this was changed the to a modified radial incision technique, as inspired by the fornix incision of Harms and the "minimally invasive strabismus surgery" (MISS) technique of D. S. Mojon. The indication is the reduction in complications and protection of the conjunctiva. METHODS: A retrospective analysis was performed of cases between 2008 and March 2014 was performed, including a total of 258 patients. 109 patients were

Infectious Scleritis: Pathophysiology, Diagnosis, and Management.

Infectious scleritis (IS) is a rare and severe ocular disorder responsible for approximately 5%-15% of all scleritis cases. It is often associated with a poor prognosis due to its similar clinical presentation to autoimmune scleritis, resulting in a delayed diagnosis and treatment. Therefore, differentiating between infectious and noninfectious entities is critical. Several details extracted from the patient's history and clinical examination can raise suspicion for infection. The most common predisposing factor is previous ocular surgery, especially pterygium, cataract, and vitreoretinal surg

Algorithmic approach to diagnosis and management of post-refractive surgery dry eye disease.

Dry eye disease (DED) is a condition that is fast reaching epidemic proportions around the world. Dry eye post-refractive surgery is the leading cause of iatrogenically induced DED. The wide variety of presentations and the disparity between signs and symptoms in many patients make this a very challenging aspect of our clinical practice. There has been a paradigm shift in the way we approach and treat this condition. The International Dry eye workshop has added new knowledge and focus to our management of dry eye. A wide range of newer diagnostic modalities are available for the diagnosis of D

Triple procedure for coexistent aphakia, squint, and iris defect: single-stage strabismus correction, glued IOL, and single-pass 4-throw pupilloplasty.

Posttraumatic aphakia is a serious deterrent to visual rehabilitation that usually follows sensory exotropia, thereby mandating urgent surgical intervention. In addition, abnormal iris configuration and iris defects postocular trauma can lead to amblyopia and glare and require surgical correction. However, in individuals where such multiple posttraumatic extraocular and intraocular abnormalities coexist, confusion remains about whether to perform sequential or concomitant surgeries. In these situations, an unusual triple technique consisting of strabismus correction, glued scleral fixation of

Panretinal Photocoagulation: A Review of Complications.

Panretinal photocoagulation (PRP) is a mainstay of therapy for retinal ischemic disease. The procedure involves creating thermal burns in the peripheral retina leading to tissue coagulation, the overall consequence of which is improved retinal oxygenation. While highly effective, there have been concerns historically regarding the anatomic effects and visual complications following PRP, the most common of which include choroidal effusions, exudative retinal detachments, macular edema, visual field deficits, and night vision defects. The occurrence of these complications is closely tied to lase

The treatment outcomes of crescent-shaped conjunctiva resection combined with conjunctiva sclera fixation for severe conjunctivochalasis.

OBJECTIVE: To observe the treatment outcomes of a crescent-shaped conjunctiva resection combined with conjunctiva sclera fixation for severe conjunctivochalasis. PATIENTS AND METHODS: Eleven (22 eyes) patients with severe conjunctivochalasis were enrolled in this study. They consented to undergo crescent-shaped conjunctiva resection and conjunctiva sclera fixation, to evaluate their symptoms and inspect the lacrimal fluid system. This included the breakup time of the tear film, height and integrity of the lacrimal river, bulbus oculi fluorescein staining pre-operation and 6 m-post-operation.

Management of floppy eyelid associated with Down's syndrome: a case report.

PURPOSE: To report a case of floppy eyelid associated with Down's syndrome in a 2-year-old child. CASE REPORT: A 2-year-old child associated with Down's syndrome presented with a history of eversion of both upper lids during sleep and while crying since birth. There was no history of sleep apnea and no mechanical stimuli over eyelids. Examination under anesthesia revealed gross laxity with thick and rubbery tarsi in both the upper and lower lids. Vascularized corneal scar was present in both eyes. The laxity in the upper eyelids was corrected by pentagonal wedge resection and in the lower lid

Surgery of the conjunctiva.

BACKGROUND: Dry eye is characterized by aqueous tear deficiency or excessive tear evaporation at the ocular surface, both leading to tear film instability that results in ocular surface epithelial damage. In addition to the lacrimal gland, the meibomian gland, cornea, and conjunctiva also contribute to the formation of the tear film via the production of goblet-cellderived mucin and the expression of membrane-associated mucin. The conjunctiva can be linked with dry eye through various mechanisms such as inflammation, disturbance of the tear film, and conjunctival fibrosis. Thus, normalization

[Careful use of radial optic neurotomy for the treatment of retinal central vein occlusion].

Radial optic neurotomy (RON) is an attractive and controversy procedure. Commenting on some disputed issues, the authors think that it seems inappropriate to negate the therapeutic effects of RON completely. However, it should be careful to select the proper patients for performing the RON. The indiscriminate extension of the indications of RON should be avoided.

Anterior segment inflammation and hypotony after posterior segment surgery.

Recent advances in the instrumentation and surgical techniques for posterior segment surgery permit better anatomic and functional outcomes. Nevertheless, these procedures may be associated with complications involving the anterior segment, including postoperative inflammation and hypotony. One must differentiate infectious from noninfectious inflammation to initiate appropriate therapy promptly. Hypotony is a frustrating problem because it is difficult to reverse. The exclusion of eyes with other known causes of hypotony is important before establishing the putative mechanism of tractional ci

[Surgical treatment of complicated retinal detachment].

Treatment of retinal detachment advanced markedly during the 20th century. The surgical approach to rhegmatogenous retinal detachment was first established by Joules Gonin, and treatment of traction retinal detachment by vitrectomy was developed by Robert Machemer. Although favorable outcomes are obtained in most cases of retinal detachment using current vitreoretinal surgical techniques, several special types of complicated retinal detachment can be refractory to treatment. The biologic mechanism and the strategy for treatment of a complicated retinal detachment will be discussed in this lect
