Brain and retinal infarctions during sleep have been attributed to focal hypoperfusion caused by systemic hypotension combined with underlying arterial stenosis, rather than to embolism. Fluorophotometry was performed in 14 rabbits after exposure of one eye to white light with an energy insufficient to cause visible phototoxic retinal damage as determined by ophthalmoscopy and fundus photography. BACKGROUND: Malarial retinopathy refers to a constellation of changes seen in severe or complicated malaria cases. These include: retinal whitening, vessel changes-whitening, tramlining, retinal hemorrhages, and papilledema. There are very few Indian studies on this entity. CASE REPORT: The case of 5 members of a family who suffer from Von Hippel-Lindau disease (VHL) is presented. It is shown that retinal capillary hemangioma was the main ophthalmic symptom, with hemovitreous and tractional retinal detachment as the main complications.
DISCUSSION: VHL disease is rare, but very serious, even fatal, thus it is important to obtain an early diagnosis by ophthalmoscopy, in order to change the visual prognosis and life expectancy, not only for the patient, but also for the family.

A perinatally, somatically, and neurologically aggravated child born at 26 weeks weighing 860 g with preterm neuropathy is described. Typical difficulties of neonatologists and pediatric ophthalmologists in the treatment of preterm babies with this grave ocular disease are described: the diagnosis cannot be confirmed by ophthalmoscopy because the optic media are not transparent, cryo- or surgical treatment has to be delayed because of severe somatic and neurological status, and intra- and postoperative complications develop despite thorough preoperative preparation.

Outpatient sedation of infants, uncooperative children and, rarely, adults provides an opportunity to perform certain ophthalmologic diagnostic procedures such as tonometry, ophthalmoscopy and various electrophysiological studies (e.g., electroretinography and visual evoked response testing) without the expense and usage of an operating room. Many drugs have been recommended and used; all have limitations. The advantages and disadvantages of the more commonly used agents are reviewed. The known effects of these drugs on ocular function are presented.

A 60 year-old black man when seen in consultation for foreign body sensation and presbyopic complaints was noted on ophthalmoscopy to have bilateral pigment epithelial defects. These defects were curiously symmetrical and there were visual field defects corresponding to the areas showing loss of pigment epithelium. The electrooculogram was abnormal and fluorescein angiography showed normal large choroidal vessels but totally absent choriocapillaris in the involved areas and abnormal pigment epithelium at the edge of the defect.

As small astrocytic hamartomas can sometimes be missed during routine ophthalmoscopy in patients with tuberous sclerosis, fluorescein and/-or indocyanine green angiography maybe more helpful in identifying such small astrocytomas. In fluorescein angiography, astrocytomas show gradually increasing hyperfluorescence due to the vascular permeability of astrocytomas. In indocyanine green angiography, astrocytomas appear hypocyanescent, most prominently during the late phases. We report the indocyanine green angiographic findings of retinal astrocytomas in a patient with tuberous sclerosis.

8 cases of rhegmatogenous retinal detachment associated with vitreous hemorrhage were treated by first removing the blood through vitrectomy followed by routine retina reattachment procedures, such as searching for retinal breaks with indirect ophthalmoscopy and scleral buckling or encircling. Among 6 cases followed up, only 1 cases relapsed due to PVR traction. The points of note during operation were mentioned and the relationship between retinal breaks with vitreous hemorrhage and posterior vitreous detachment discussed.

Optic neuritis is clinically suspected when painful, rapidly progressive central visual loss occurs in a young patient who has a relative afferent pupillary defect. Ophthalmoscopy is normal in the majority of cases but papillitis or optic disc palor can occur. Several other diseases can mimic optic neuritis, such as ophthalmological conditions or other diseases affecting the visual pathways. Their appropriate clinical diagnosis is of paramount importance in order to avoid unnecessary or invasive procedures or treatments.

Thirty patients who had undergone long-term treatment with carotenoids (max. 14 years and 336 g of carotenoids) were examined. Ophthalmoscopy revealed retinal crystals in 20 patients. The retinal crystals were usually located around the macula, forming a ring, and in one case they were even found in the macula itself. No deterioration of visual acuity or visual field defects were found, but there was a markedly dose-dependent prolongation of the duration of dark adaptation.

A 26-year-old man with sex-linked juvenile central and peripheral retinoschisis showed on fluorescein angiography an avascular lower temporal periphery, bordered at the central side by abnormal capillaries in a dendrite-like structure. In dominant exudative vitreoretinopathy an identical picture without schisis can be seen in the periphery. This feature seems to be a rare phenomenon in sex-linked juvenile retinoschisis. However, routine peripheral ophthalmoscopy and fluorescein angiography (especially inferotemporal) may reveal more cases.

Cholesterol crystal embolism is observed in polyvascular patients usually following a medical procedure. We report a 93-year-old patient with lower limb mononeuropathy in whom cholesterol crystal embolism was confirmed by neuromuscular biopsy. Previously, an ophthalmoscopy and a livedo biopsy had been performed and the results were normal. Mononeuropathy simplex or multiplex is a clinical expression for cholesterol crystal embolism that may thus be confounded with polyarteritis nodosa. Neuromuscular biopsy can provide accurate diagnosis if other investigations are inconclusive.

A grand mal seizure is an unexpected, rare adverse event in a child receiving eye drops to dilate the pupils for an eye examination. A case is reported of a convulsion in a 23-month-old boy after he received Cyclogyl (cyclopentolate) and Neo-Synephrine (phenylephrine) eye drops before ophthalmoscopy. His serum sodium was 125 mEq/L, and he had low plasma pseudocholinesterase activity. Children exposed to organophosphate insecticides and other pseudocholinesterase inhibitors may be at risk for cyclopentolate toxicity.

The effect of acute ocular hypotony as a factor in the formation of serous choridal detachments was investigated in 45 eyes undergoing cataract extraction. A fundus examination was made with binocular indirect ophthalmoscopy immediately after lens removal and in the early postoperative period. No choroidal detachments occurred, either early or late. Full-thickness infoldings of the eye wall in the posterior segment were seen in 80 per cent of the eyes immediately after lens extraction. The orientation of these folds is described, and factors leading to their formation are discussed.
