An appraisal of common otologic disorders as seen in a deaf population in North-Western Nigeria.

BACKGROUND/OBJECTIVE: Deaf persons are equally susceptible to other otologic diseases as the rest of the population; however, there may be increased morbidity among the deaf population. The aim of the study is to determine common otologic problems and their prevalence among the deaf and to offer treatment where appropriate. MATERIALS AND METHODS: Six hundred and twenty students aged between 5 and 38 years with profound hearing loss were recruited from a special school for the deaf. Hearing level was assessed and full ENT examination including otoscopy was done. RESULTS: 22.4% [139] of the de

Melanoma of the middle ear: initial presentation, Fluoro-2-deoxy-D-glucose positron emission tomography/computed tomography imaging and follow up.

BACKGROUND: We present a rare case of primary mucosal melanoma of the middle ear imaged with 18F-fluoro-2-deoxy-D-glucose positron emission tomography/computed tomography (FDG-PET/CT). METHOD: Clinical, radiological, intra-operative and histological findings are discussed. RESULTS: An 88-year-old woman presented with intermittent otorrhoea of the left ear for several months. Otoscopy revealed a livid protrusion of the tympanic membrane. Melanoma was not suspected initially, but was established on transmembranous biopsy. Pre-operative 18F-fluoro-2-deoxy-D-glucose positron emission tomography/

Mechanical obstruction of the eustachian tube by the benign tumor of the parapharyngeal space does not cause otitis media with effusion.

OBJECTIVE: We are reporting on eustachian tube and middle ear pathologic findings in patients with parapharyngeal tumors. PATIENTS: Eleven cases of eustachian tube involvement as indicated by obstruction due to compression of the tumor were assessed in this study. MAIN OUTCOME MEASURES: Each patient underwent otoscopy, nasopharyngoscopy, a pure-tone hearing test, impedance audiometry, and sonotubometry. RESULTS: All of the tumors were diagnosed benign according to the histologic examination. In 9 of the 11 patients, the pharyngeal opening of the eustachian tube narrowed or was blocked by th

Disaster avoided: otalgia warns of potential electrode extrusion.

Cochlear implantation is usually a safe procedure but, as in most surgical interventions, it can carry some risk of complications. These can be divided into major and minor that, in turn, in turn can present early or late. This case report highlights a potential late, major complication of cochlear implantation namely extrusion, which was averted by prompt intervention. The patient presented with a three months' history of otalgia some six years after successful implantation. The posterior half of the tympanic membrane was found to be retracted on the electrode, which appeared to be on the poi

Occult infection in the facial area presenting as fever of unknown origin.

Dental infections, otitis media or sinusitis may present as fever of unknown origin (FUO), lacking local symptoms and signs. Physicians are often unaware of this possibility, since current textbooks and major articles do not mention occult facial infections among the causes of FUO. Patients with FUO typically undergo a laborious series of uncomfortable, time-consuming, expensive and often unnecessary investigations in the search for the cause of pyrexia. We report five such patients from a community hospital, representing 14% of cases in a series of patients with FUO. In most of these patients

Cholesteatoma medial to an intact tympanic membrane in 34 young children.

Thirty-five cholesteatomas medial to intact eardrums were treated in 34 children between 1976 and 1982. Six (18%) children had never had a documented episode of otitis media. Seventeen (50%) children, in whom the lesion was diagnosed at an early stage, underwent simple excision of the cholesteatoma without the need for extensive middle ear surgery. Findings from postoperative audiograms were normal for all such children. Cholesteatoma has recurred in eight (23%) children to date. Most recurrences were diagnosed 15 months or less after surgery. Routine careful otoscopic examination is essential

Epidemiology of otitis media with effusion in children.

The recently accepted international classification for otitis media was applied to a study population of 898 children less than 12 years of age having otitis media with effusion persisting at least three months. Mucoid effusion was aspirated from 48% of ears, was found more often in younger than older patients, was more often bilateral, and was a more stable state during longitudinal observation than was serous otitis media or purulent otitis media (POM). Serous otitis media occurred in only 10% of ears, but was found more often than in younger patients; POM was found in only 7% of ears. Otosc

Evaluation of Types of Vertigo With Electronystagmography: An Experience From a Tertiary Care Hospital in West Bengal, India.

Background Electronystagmography (ENG) is a diagnostic test that measures the electrical activity of the muscles that control eye movements. ENG has the potential to identify the cause of vertigo by assessing the function of the vestibular system. Vertigo can be of two types - peripheral or central. In addition, a combination of peripheral and central types may coexist. Peripheral vertigo is caused by pathology in the inner ear and central vertigo is caused by pathology in the brainstem or cerebellum. Objective This study aimed to evaluate the applicability of ENG in assisting the diagnosis of

Isolated Myxoma of the External Auditory Canal: A Case Report and Literature Review.

Myxoma is a benign myxoid tumor of connective tissue that develops primarily in the heart. At the level of the external auditory canal, it is extremely rare. It can be isolated or associated with Carney syndrome. Only 5 cases of isolated myxoma of the external auditory canal have been reported in the literature. We present the case of a 53-year-old patient who consulted for a hearing loss that has been evolving for 3 years. Otoscopy revealed a mass filling the external auditory canal. The scan showed a total filling of the external auditory canal with a homogenous sessile neoformation of 20 ×

Isolated primary squamous cell carcinoma of the tympanic membrane.

INTRODUCTION: Primary squamous cell carcinoma (SCC) of the tympanic membrane is exceptionally rare. We describe the history, investigation and management of this disease. PRESENTATION OF CASE: A 68-year-old woman presented with a three month history of intermittent otorrhoea and external ear canal (EAC) pruritus. Otoscopy revealed a polypoidal granular nodule, confined to the posterior aspect of the tympanic membrane. Examination under anaesthesia (EUA) confirmed that the lesion was confined to the tympanic membrane, with a surrounding rim of normal drum. Biopsies were consistent with well di

[Update: blast and explosion trauma].

In recent decades, acoustic shock and explosion traumas have increased in frequency in the general population. Beside the use of fireworks and firearms, airbag ignitions and explosions caused by terror or suicidal acts are also relevant. Depending on duration and strength of the sound pressure affecting the human ear, isolated inner ear damage or additional ear drum perforation and interruption of the middle ear ossicle chain can result. By means of otoscopy, pure tone audiometry, measurement of otoacoustic emissions, and other neurootological examinations, the severity of the trauma can be de

[Middle ear adenoma. Long-term course of a rare neoplasm].

Adenomas of the middle ear are rare benign glandular neoplasms arising from the middle ear mucosa. After previous operations 25 and 15 years before, a 67-year-old female complained about dizziness, tinnitus, and unilateral hearing loss on the left side. A tumor in the tympanum that was revealed by otoscopy could be removed completely. Histological examinations showed an adenoma of the middle ear with cholesteatoma. This was inconsistent with the histological result of the operation in 1983, which had described a hidradenoma. An exact analysis of the preparations confirmed that a middle ear ade

A study of the prevalence of and risk factors for ear diseases and hearing loss in primary school children in Hat Yai, Thailand.

2,184 primary school children in Hat Yai Municipality, Southern Thailand were screened for ear diseases and hearing loss using screening audiometry, portable tympanometry and otoscopy to identify children with possible ear problems, 754 children failed the initial screen, 728 were retested and a group of 243 were referred for ENT examination and standard audiometry-tympanometry. Children with hearing loss and otitis media were thus identified. A questionnaire to identify possible risk factors was administered. The prevalence of otitis media in this population was found to be 3.25 per cent (AOM

[Sudden deafness--diagnosis and therapy].

Sudden deafness is defined as acute inner ear hearing loss, in the rule one-sided, of unknown etiology. The tentative diagnosis can be made easily by otoscopy and simple audiological forke tests. To exclude symptomatic acute hearing losses during the first treatment period, f.e. acoustic neurinoma, rupture of the round window membrane, multiple sclerosis, infectious diseases like Borreliosis or Lues, but also psychogenic hearing disorders we recommend an immediate hospitalization. Neurological and internal check up should look for inflammatory or degenerative diseases of the vascular or nervou

Otitis media with effusion in preschool children.

The incidence, prevalence, and natural history of otitis media with effusion (OME) and middle ear high negative pressure (HNP) were investigated in a group of 2 to 6 year old preschool children. The children were examined monthly over a two year period from September 1981 to August 1983. The middle ear status was assessed using a decision-tree algorithm which combined the findings of pneumatic otoscopy, tympanometry, and acoustic reflex measurements. Fifty-three percent of the children in the first year and 61% in the second year developed OME; also during the two years, HNP was documented in

Management of chronic otitis media with effusion.

Chronic otitis media with effusion ('secretory') is one of the most common diseases of childhood. Pathogenesis related to Eustachian tube dysfunction. Bacteria have been isolated from approximately 50 percent of chronic middle ear effusions. Diagnosis by pneumatic otoscopy or tympanometry, or both. Efficacy yet to be shown for antimicrobial therapy, decongestants, antihistamines, hydrocortisone, myringotomy with or without tympanostomy tubes, and adenoidectomy with or without tonsillectomy. However, a 10-day trial with an antimicrobial agent, such as amoxicillin (erythromycin and sulfonamide,

Optical coherence tomography otoscope for imaging of tympanic membrane and middle ear pathology.

SIGNIFICANCE: Pathologies within the tympanic membrane (TM) and middle ear (ME) can lead to hearing loss. Imaging tools available in the hearing clinic for diagnosis and management are limited to visual inspection using the classic otoscope. The otoscopic view is limited to the surface of the TM, especially in diseased ears where the TM is opaque. An integrated optical coherence tomography (OCT) otoscope can provide images of the interior of the TM and ME space as well as an otoscope image. This enables the clinicians to correlate the standard otoscopic view with OCT and then use the new infor
