Causes of childhood diarrhea as perceived by mothers in the Punjab, Pakistan.

This study was carried out in the southern Punjab, Pakistan, to outline the causes of childhood diarrhea as perceived by mothers. Two hundred households in ten villages were randomly selected. Information was obtained from mothers, through a questionnaire, in-depth interviews, and direct observations. The focus was on obtaining information from mothers of children who were below five years of age. Causes of diarrhea reported by mothers were categorized in seven different domains. Causes relating to the digestive system, especially consumption of too much food, were the most important, followed…

Lay perceptions of genetic risks attributable to inbreeding in Pakistan.

Pakistan along with many other West and South Asian countries has a very high prevalence of consanguineous, especially close cousin, marriages. Although there is substantial empirical information on offspring morbidity and mortality attributable to parental consanguinity, population-based information on how communities in general, and women in particular, perceive the health risks associated with consanguineous unions is limited. This paper considers community perceptions of health effects associated with consanguineous marriages using qualitative data from 15 focus group discussions and 294 i…

The impact of consanguinity and inbreeding on perinatal mortality in Karachi, Pakistan.

Close consanguineous unions continue to be extremely common in much of West Asia, including Pakistan. However, the impact of inbreeding on offspring mortality, particularly perinatal mortality, remains poorly documented. This paper attempts to measure the mortality risks associated with consanguinity and inbreeding while controlling for the effects of other potential confounders. The study sample comprises a multi-ethnic population residing in selected squatter settlements of Karachi. The adjusted odds ratio for perinatal mortality in the offspring of women married to their first cousins was 2…

Treatment of diarrhoea in infants by medical doctors in Balochistan, Pakistan.

Diarrhoea is an important public health problem in Balochistan, the westernmost province of Pakistan. Although the use of oral rehydration solutions (ORS) has been widely promoted, no studies have been reported on the actual uses of ORS in treating infant diarrhoea by the medical doctors in this region. The medical practices of 30 doctors in Balochistan were surveyed. The surveyors posed as the mothers of infants with diarrhoea. The questions asked by the doctors, the physical examinations performed, and the treatments prescribed were noted. The histories and physical examinations were incompl…

Evaluation of the genotoxic potential of certain pesticides used in Pakistan.

The mutagenicity of fifteen insecticides, five fungicides, four herbicides, and an acaricide commonly used in Pakistan was evaluated by employing thirteen short-term bioassays. The genetic endpoints used included point or gene mutation, primary DNA damage, and chromosomal effects. Initially, all pesticides were tested in a "core" battery of four in vitro bioassays. A carefully selected group among these chemicals was retested in higher level test systems to confirm the results obtained in the initial phase. Of the pesticides tested, only a small portion consistently displayed mutagenicity acro…

Exploring Safety Outcomes Among Sanitary Workers in Punjab, Pakistan.

BACKGROUND: Sanitary work is the backbone of the municipal system. In Pakistan, it is performed manually, often under unhygienic conditions. As a result, workers are exposed to hazardous health and safety risks. OBJECTIVE: This study aimed to investigate the safety outcomes among sanitary workers in Punjab, Pakistan. METHOD: A qualitative research approach was used, employing focus group discussions (FGDs) for data collection. Two FGDs were conducted, involving a total of 19 sanitary workers and two managers from the Cantonment Board Taxila and the Lahore Waste Management Company. RESULTS: …

Strengthening one health approaches for rabies elimination in Pakistan.

Rabies, due to infection with the Lyssavirus of the Rhabdoviridae, is a virtually 100% lethal viral illness acquired mostly by bites from an infected animal, particularly by dogs, but also cattle, goats, cats, and feral animals like bats and foxes. The virus has a 1-3-month incubation period and infects the central nervous system, manifesting as either encephalitic rabies (hydrophobia, excitability, hypersalivation) or paralytic rabies (increasing muscle weakness, paralysis, coma). One documented case in Pakistan was a 18-year-old farmer who was bitten by his rabid cow, probably from a stray d…

Inflammatory bowel disease in Pakistan: Low prevalence or underdiagnosis?

Inflammatory bowel disease (IBD) has a low prevalence in the Global South, including Pakistan. While genetics and gut flora, influenced by diet and the environment, can contribute to this, we are exploring the possibility of underdiagnosis. Lack of facilities trained in IBD, scarcity of medications, limited health insurance for specialist referral and the current epidemic of abdominal tuberculosis could be the cause. The increasing diagnosis of IBD among Pakistani immigrants and colorectal carcinoma among the young population might be the result of this. Timely referrals by primary care physic…

Emerging novel sequence types of Staphylococcus aureus in Pakistan.

BACKGROUND: Despite an increasing incidence of Staphylococcus aureus infection and dissemination in Pakistan, the epidemiology of different Staphylococcus aureus research clones has been the subject of only a small number of investigations. By analyzing the collected data sequence, this study was designed to study the epidemiology of Staphylococcus aureus in the area using multilocus sequence typing (MLST). METHODS: A total of 1015 staphylococcus strains collected from the city's tertiary care facilities were biochemically screened, followed by antimicrobial susceptibility testing against a p…

Case of Carbapenem-Resistant Salmonella Typhi Infection, Pakistan, 2022.

Salmonella Typhi infection in a patient in Pakistan initially responded to standard treatment but failed to respond to subsequent treatment. The first strain was susceptible to carbapenems and azithromycin; subsequent strains harbored the NDM-5 gene. Treatment with a combination of intravenous meropenem and colistin was successful. Carbapenem-resistant Salmonella Typhi emergence will hinder treatment.

Mysterious dengue-like virus: A case report from Pakistan.

Recently, in Pakistan, several cases of mysterious dengue-like illness are being reported, which has concerned the authorities and requires prompt action. We present a case of a 52-year-old female patient presenting with a history of continuous fever, documented up to 104 F, for 5 days. The symptoms were associated with headache, nausea, retro-orbital headache, arthralgia, and myalgia. Currently, to the best of our knowledge, this is the first reported case in the literature for the endemic mysterious virus and may serve as the groundwork for future studies.

Progress Toward Poliomyelitis Eradication - Pakistan, January 2021-July 2022.

After reporting a single wild poliovirus (WPV) type 1 (WPV1) case in 2021, Pakistan reported 14 cases during April 1-July 31, 2022. Pakistan and Afghanistan are the only countries where endemic WPV transmission has never been interrupted (1). In its current 5-year strategic plan, the Global Polio Eradication Initiative (GPEI) has set a goal of interrupting all WPV1 transmission by the end of 2023 (1-3). The reemergence of WPV cases in Pakistan after 14 months with no case detection has uncovered transmission in southern Khyber Pakhtunkhwa province, the most historically challenging area. This …

Systematic Review of Reported HIV Outbreaks, Pakistan, 2000-2019.

Unsafe injection practices and injection drug use have been linked to multiple HIV outbreaks in Pakistan since 2003; however, few studies have systematically analyzed the causes of these outbreaks. We conducted a systematic review of published English-language literature indexed in bibliographic databases and search engines and a focused gray literature review to collate and analyze all reported HIV outbreaks in Pakistan during 2000–2019. Of 774 unique publications reviewed, we identified 25 eligible publications describing 7 outbreaks. More than half occurred during 2016–2019. The primary sou…

Molecular genetic landscape of hereditary hearing loss in Pakistan.

Approximately 14.5 million Pakistani individuals have a hearing loss and half of these cases may be due to genetic causes. Though significant progress has been made in uncovering genetic variants for recessively inherited nonsyndromic deafness, Pendred syndrome, and Usher syndromes, the same is not true for dominantly inherited hearing loss, most syndromic cases and deafness with complex inheritance patterns. Variants of 57 genes have been reported to cause nonsyndromic recessive deafness in Pakistan, though most are rare. Variants of just five genes GJB2, HGF, MYO7A, SLC26A4, and TMC1 togethe…
