Health outcomes associated with emergency department visits by adolescents for self-harm: a propensity-matched cohort study.

BACKGROUND: Self-harm is increasing among adolescents, and because of changing behaviours, current data are needed on the consequences of self-harm. We sought to investigate the trends related to hospital presentation, readmission, patient outcome and medical costs in adolescents who presented with self-harm to the emergency department. METHODS: We used administrative data on 403 805 adolescents aged 13-17 years presenting to Ontario emergency departments in 2011-2013. Adolescents with self-harm visits were 1:2 propensity matched to controls with visits without self-harm, using demographic, m

Harm reduction stories: leveraging graphic medicine to engage veterans in substance use services within the VA.

BACKGROUND: Harm reduction strategies can decrease morbidity and mortality associated with substance use. Various barriers limit conversation around substance use between clinicians and patients. Graphic medicine techniques can inform and encourage patient-centered conversations about substance use. We describe the co-development of a harm reduction-focused graphic medicine comic that depicts the infectious risks associated with injection drug use and patient-centered approaches to providing education about potential risk mitigation strategies. METHODS: We formed a co-design group of veterans

Self-harm and life problems: findings from the Multicentre Study of Self-harm in England.

PURPOSE: Self-harm is a major clinical problem and is strongly linked to suicide. It is important to understand the problems faced by those who self-harm to design effective clinical services and suicide prevention strategies. We investigated the life problems experienced by patients presenting to general hospitals for self-harm. METHODS: Data for 2000-2010 from the Multicentre Study of Self-harm in England were used to investigate life problems associated with self-harm and their relationship to patient and clinical characteristics, including age, gender, repeat self-harm and employment stat

How useful are clinical reports concerning the consequences of therapist-patient sex?

The harmfulness and prevalence of sex between therapist and patient are difficult to research. One reason for this is the difficult of establishing a rate of harm; general population trends cannot be established within reasonable limits. A second problem involves determining the cause of observed harm. Therapists who treat patients who have had sex with a therapist may incorrectly conclude that symptoms resulted from that inappropriate relationship, while another therapist might attribute them to childhood sexual abuse or other causes. Patients with borderline or histrionic personality disorde

Self-harm injury hospitalisations: an analysis of case selection criteria.

BACKGROUND: This study explores the impact of using different criteria to identify nonfatal hospitalisations with self-harm injuries using 2017-2018 Wisconsin discharge data. METHODS: Using International Classification of Diseases, 10th Revision, Clinical Modification codes, we classified records by three mutually exclusive selection criteria: subset A--principal diagnosis of injury, and any code for self-harm, initial encounter only; subset B--non-injury principal diagnosis, and any code for self-harm, initial encounter only; subset C--any principal diagnosis, and any code for self-harm, sub

Open access to pathology reports: potential harms and proposed solutions.

The Cures Rule permits patients to unfettered, immediate access of their documented health information unless medical providers 'block' this information. While the transparency of open notes has perceived benefits for clinic-based progress notes, the extension of the Cures Rule in its current form to include pathology reports could lead to unintended harms. Several consequences for both patient and provider are identified by examining hypothetical cases, inspired by patient and provider experiences, of misunderstood diagnoses and unexpectedly discovered cancers in seemingly benign tissue. Iden

Mitigating and preventing perioperative opioid-related harm.

PURPOSE OF REVIEW: Although necessary for treatment of acute pain, opioids are associated with significant harm in the perioperative period and further intervention is necessary perioperatively to mitigate opioid-related harm. RECENT FINDINGS: Opioid-naive patients are often first exposed to opioids when undergoing surgery, which can result in significant harm. Despite their benefits in reducing acute postsurgical pain, they are also associated with risks ranging from mild (e.g., pruritis, constipation, nausea) to potentially catastrophic (e.g. opioid-induced ventilatory impairment, respirato

Evaluating the effects of drugs on behavior and quality of life: an alternative strategy for clinical trials.

Conventional clinical trials involve tests of hypotheses with statistics computed from values of dependent variables alone. An alternative is to test hypotheses with statistics computed from benefit/harm scores that measure longitudinal associations between dose and values of the dependent variables. The proposed standardized measure of benefit/harm quantifies the strength of evidence that a patient did either better or worse while on treatment. A benefit/harm score, particularly when obtained from a randomized, N-of-1 trial, indicates a beneficial or harmful treatment effect for the individua

Characteristics of small areas with high rates of hospital-treated self-harm: deprived, fragmented and urban or just close to hospital? A national registry study.

BACKGROUND: Previous research has shown an inconsistent relationship between the spatial distribution of hospital treated self-harm and area-level factors such as deprivation and social fragmentation. However, many of these studies have been confined to urban centres, with few focusing on rural settings and even fewer studies carried out at a national level. Furthermore, no previous research has investigated if travel time to hospital services can explain the area-level variation in the incidence of hospital treated self-harm. METHODS: From 2009 to 2011, the Irish National Registry of Deliber

Overlap between harm reduction and HIV service utilisation among PWID in India: Implications for HIV combination prevention.

BACKGROUND: In some regions, HIV incidence is rising among people who inject drugs (PWID). Combination prevention approaches are well suited to PWID who face multiple sources of HIV risk. This analysis investigates patterns of utilisation to basic HIV services (HIV counselling and testing [HCT], antiretroviral therapy [ART]) as well as harm reduction programs (needle and syringe exchange programs [NSEP] and opioid agonist therapy [OAT]) among PWID and how utilisation of harm reduction services is associated with HIV-related care seeking behaviours. METHODS: Respondent-driven sampling was used

The functions of self-harm in young people and their perspectives about future general practitioner-led care: A qualitative study.

BACKGROUND: Self-harm in young people is a serious concern but a deeper understanding of the functions of self-harm in young people can tailor care and inform new clinical interventions to reduce repeat self-harm and suicide risk. General practitioners (GPs), as frontline healthcare professionals, have an important role in managing self-harm in young people. This study aimed to explore the functions of self-harm in young people and their perspectives on future GP-led care. METHODS: A qualitative study using interviews with young people aged between 16 and 25 years with a personal history of s

The spectrum of hospitalization-associated harm in the elderly.

Acutely ill patients are not infrequently referred to the hospital and admitted, when they could be diagnosed and managed in the ambulatory setting or by hospital-level care at home. Avoidable admissions are particularly regrettable when the wide spectrum of hospitalization-associated patient harm is considered. It includes acute discomfort to the patient due to multiple disturbing hospital stressors; an emotional trauma; the burden of multiple redundant tests begetting false-positive and incidental findings triggering further testing and cascades; highly prevalent adverse events and serious h…

Unplanned Rehospitalisation due to Medication Harm following an Acute Myocardial Infarction.

INTRODUCTION: The contribution of medication harm to rehospitalisation and adverse patient outcomes after an acute myocardial infarction (AMI) needs exploration. Rehospitalisation is costly to both patients and the healthcare facility. Following an AMI, patients are at risk of medication harm as they are often older and have multiple comorbidities and polypharmacy. This study aimed to quantify and evaluate medication harm causing unplanned rehospitalisation after an AMI. METHODS: This was a retrospective cohort study of patients discharged from a quaternary hospital post-AMI. All rehospitalis

Pretreatment cognitive performance is associated with differential self-harm outcomes in 6 v. 12-months of dialectical behavior therapy for borderline personality disorder.

BACKGROUND: Recent findings suggest that brief dialectical behavior therapy (DBT) for borderline personality disorder is effective for reducing self-harm, but it remains unknown which patients are likely to improve in brief v. 12 months of DBT. Research is needed to identify patient characteristics that moderate outcomes. Here, we characterized changes in cognition across brief DBT (DBT-6) v. a standard 12-month course (DBT-12) and examined whether cognition predicted self-harm outcomes in each arm. METHODS: In this secondary analysis of 240 participants in the FASTER study (NCT02387736), cog

Imputation and characterization of uncoded self-harm in major mental illness using machine learning.

OBJECTIVE: We aimed to impute uncoded self-harm in administrative claims data of individuals with major mental illness (MMI), characterize self-harm incidence, and identify factors associated with coding bias. MATERIALS AND METHODS: The IBM MarketScan database (2003-2016) was used to analyze visit-level self-harm in 10 120 030 patients with ≥2 MMI codes. Five machine learning (ML) classifiers were tested on a balanced data subset, with XGBoost selected for the full dataset. Classification performance was validated via random data mislabeling and comparison with a clinician-derived "gold stand

Harm reduction as a treatment model for alcohol use disorder: A concept analysis.

BACKGROUND: Alcohol use disorder (AUD), the most prevalent substance use disorder globally, remains underdiagnosed and undertreated, highlighting the urgent need for expanded access to evidence-based prevention, treatment, and harm reduction strategies. Harm reduction has gained recognition as a viable alternative to abstinence-based treatment models. However, its application in the treatment of AUD remains ambiguous and is often subject to misconceptions. PURPOSE: The purpose of this concept analysis is to provide clarification on the concept of harm reduction within the context of AUD treat

Opioid harm reduction: A scoping review of physician and system-level gaps in knowledge, education, and practice.

Background: Harm reduction includes treatment and prevention approaches rather than abstinence, as a public health strategy for mitigating the opioid epidemic. Harm reduction is a new strategy for many healthcare professionals, and gaps in knowledge and practices may lead to barriers to optimal treatment. Our objective was to identify and describe gaps in physicians' knowledge, education, and practice in harm reduction strategies related to opioid overdose. Methods: We searched the PubMed, CINAHL, and Web of Science databases for articles published between 2015 and 2021, published in English,

"We're Not Gonna Aid You in Shooting Up": Stigma's Relationship to Harm Reduction in People Who Inject Drugs.

BACKGROUND: Harm reduction, when applied to drug use, prioritizes improving patient-centered health outcomes and reducing drug-related harm. In order for harm reduction strategies to be adopted by people who inject drugs (PWID), they need to be promoted, accessible, and accepted in that population and the community-at-large. While PWID face stigma at multiple levels, less is known about how stigma influences uptake and acceptance of harm reduction services and strategies among PWID. OBJECTIVE: We aim to characterize the stigmatizing experiences PWID have had related to harm reduction and the
