Efficacy of MitraClip and continued multidisciplinary cardiac rehabilitation in preventing readmission of an older heart failure patient with severe multivalvular disease: A case report.

UNLABELLED: The recommendations for the treatment of heart failure patients with severe multivalvular disease are not clear. We present a successful case of an older patient with heart failure and severe multivalvular disease in whom rehospitalization was prevented by a combination of MitraClip implantation (Abbott Vascular, Santa Clara, CA, USA) and continued multidisciplinary cardiac rehabilitation. An 85-year-old male patient presented with New York Heart Association (NYHA) class III severe mitral regurgitation (MR), severe aortic valve regurgitation (AR), and severe tricuspid valve regurgi

Impact of a prescriber and patient educational intervention on discharge analgesia prescribing and hospital readmission rates following elective unilateral total hip and knee arthroplasty.

INTRODUCTION: Pain management after elective, unilateral total hip and knee arthroplasty (THA and TKA) should use a multimodal approach. At discharge, challenges include ensuring correct prescribing practices to optimise analgesia and rationalise opioid use as well as ensuring patients are adequately educated to take these medications safely and effectively in the community. This audit cycle reports on a prescriber and patient education intervention using printed guidelines, educational outreach and prescription standardisation along with a patient information sheet to address the high unplann

Stakeholder experiences and expectations of transition of care services to reduce medication-related hospital readmission for cardiology patients.

BACKGROUND: Transition of care (ToC) for patients with cardiovascular diseases is a complex, high-risk period often leading to medication-related harm (MRH) and hospital readmissions. While specialised programs exist, a gap in individualised medication management services exist for specific cardiology patient populations. Pharmacist-led, interdisciplinary ToC services have demonstrated a positive impact on patient outcomes. The REducing hospital re-admission for high-risk CARDiology patients (RECARD) program aims to address this by co-designing, implementing, and evaluating a new pharmacist-le

Identifying High-Risk Patients: Clinical and Demographic Characteristics Associated With Readmission in Adolescents Hospitalized for Eating Disorders.

PURPOSE: Adolescents with eating disorders (EDs) are increasingly hospitalized in the United States and are at high risk for readmission for at least 1 year thereafter. We examined clinical and demographic factors associated with readmission in adolescents hospitalized for EDs between 2010 and 2024. METHODS: This study built upon a prior study evaluating trends in patient hospitalization for EDs. We used the PEDSnet database to pull data from eight children's hospitals on patients aged 12-21 years hospitalized for an ED and analyzed the group readmitted for the ED within 1 year of initial hos

Construction and evaluation of a 180-day readmission prediction model for chronic heart failure patients based on sCD40L.

The high readmission rate of patients with chronic heart failure (HF) can cause waste of medical resources and economic losses. Establishing an effective HF readmission model can effectively alleviate medical pressure and improve the quality of treatment. In this study, we conducted a comprehensive analysis of clinical and laboratory data from 248 patients with chronic HF who received treatment at our medical center between January 2021 to January 2022. We also measured soluble CD40 ligand (sCD40L) levels to determine their association with readmission due to HF during follow-up. To analyze th
