Load-induced increase in muscle activity during 30° abduction in patients with rotator cuff tears and control subjects.

BACKGROUND: Rotator cuff muscles stabilise the glenohumeral joint and contribute to the initial abduction phase with other shoulder muscles. This study aimed to determine if the load-induced increase in shoulder muscle activity during a 30° abduction test is influenced by asymptomatic or symptomatic rotator cuff pathologies. MATERIALS AND METHODS: Twenty-five patients with unilateral rotator cuff tears (age, 64.3 ± 10.2 years), 25 older control subjects (55.4 ± 8.2 years) and 25 younger control subjects (26.1 ± 2.3 years) participated in this study. Participants performed a bilateral 30° arm

Surgical treatment of winged scapula.

Injuries to the long thoracic and spinal accessory nerves present challenges in diagnosis and treatment. Palsies of the serratus anterior and trapezius muscles lead to destabilization of the scapula with medial and lateral scapular winging, respectively. Although nonoperative treatment is successful in some patients, failures have led to the evolution of surgical techniques involving various combinations of fascial graft and/or transfer of adjacent muscles. Our preferred method of reconstruction for serratus anterior palsy is a two-incision, split pectoralis major transfer without fascial graf

Peripheral selective neurotomy of the brachial plexus collateral branches for treatment of the spastic shoulder: anatomical study and clinical results in five patients.

A new type of peripheral selective neurotomy involving the collateral branches of the brachial plexus has been perfected for treatment of the spastic shoulder. Anatomical study of six cadaveric shoulders led to the specification of a surgical approach to the pectoralis major and teres major nerves, which innervate the main muscles implicated in shoulder spasticity. Between August 1994 and September 1995, five patients (four men and one woman) underwent two to four associated neurotomies of the upper limb, which included neurotomies of the pectoralis major (all five patients) and the teres majo

Complex Intratendinous Pectoralis Major Rupture After Heavy Bench Press.

A pectoralis major rupture is a rare injury with important strength and cosmetic consequences, most often described in young, active men after a high-load eccentric contraction during the heavy bench press. Magnetic resonance imaging (MRI) is commonly used to confirm the diagnosis and localize the tear, but the location described on MRI may not fully define tendon continuity, tissue quality, or the tendon available for fixation. We report the case of a 23-year-old male powerlifter who developed acute right chest pain, swelling, ecchymosis, deformity, and pectoralis retraction during a 500-lb b

Utility of the Pectoralis Major Index in the Diagnosis of Structurally Significant Pectoralis Major Tears.

BACKGROUND: Diagnosis of pectoralis major tears early in the acute phase is important for optimizing surgical repair and outcomes. However, physical examination of pectoralis major injuries can be misleading, often resulting in a potentially detrimental delay in surgical treatment. PURPOSE: To establish and validate a quantifiable clinical diagnostic test for structurally significant pectoralis major tears. STUDY DESIGN: Cohort study (diagnosis); Level of evidence, 2. METHODS: A total of 50 healthy male participants (mean age, 43.3 ± 11.9 years) with normal uninjured pectoralis major anatom

Use of electromyographic signals for characterization of voluntary coughing in humans with and without spinal cord injury-A systematic review.

INTRODUCTION: Individuals with spinal cord injury (SCI) frequently have an ineffective cough ability due to dysfunctions in expiratory muscles. In such cases, several articles have reported the occurrence of residual muscular activity in muscles that are accessory to coughing. The knowledge about this activity may be useful for building cough assistance devices. The goal of this review is to investigate and to describe the electromyographic signals generated during voluntary coughing in healthy people and in patients with SCI. METHODS: Two researchers performed, blindly and independently, a s

An unusual course and relations of the human axillary artery.

The axillary artery was studied by dissection of 50 human cadavers over a two-year period. Except in one instance, the course and relations of the axillary artery of all the other cadavers conformed to the normal description found in standard textbooks of Anatomy. In the exceptional cadaver, the axillary artery of the right side showed two unique features: (1) it pursued a tortuous course and made two sharp bends, and (2) it was situated superficial, rather than deep to, the pectoralis minor muscle. Its branches and relationship to the brachial plexus were also altered. On the left side, the a

CT Attenuation and Cross-Sectional Area of the Pectoralis Are Associated With Clinical Characteristics in Chronic Obstructive Pulmonary Disease Patients.

Purpose: Muscle wasting is associated with prognosis in patients with chronic obstructive pulmonary disease (COPD). Computed tomography (CT) could serve as a method for muscle assessment due to its ability to measure both muscle quantity (e.g., cross-sectional muscle area) and muscle quality (e.g., muscle attenuation). Our study aimed to compare the differences in CT-derived pectoralis muscle cross-sectional area (PMA) and pectoralis muscle attenuation (PMT) between COPD patients and healthy controls and explore the association between PMA and PMT measurements and clinical characteristics in p

K+, Na+, Mg2+, Ca2+, and water contents in human skeletal muscle: correlations among these monovalent and divalent cations and their alterations in K+ -depleted subjects.

None of previous studies had simultaneously analyzed the K(+), Na(+), Mg(2+), and Ca(2+) contents in human skeletal muscle. We examined extensively and simultaneously the levels of all these cations and examined water content in vastus lateralis and pectoralis major muscles in 30 northeastern Thai men who were apparently healthy but died from an accident. Specimen collection was performed within 6 h of death. We used atomic absorption or flame photometry to measure the level of muscle cation. Histopathology of muscle and kidney was also evaluated. Gross anatomical description of the extrinsic and intrinsic scapular and brachial muscles of Sapajus apella (Linnaeus, 1758).

BACKGROUND: The robust brown capuchin monkey (Sapajus apella) is a South American primate with preferences for arboreal locomotion, which requires specific thoracic limb muscle adaptations. The present investigation studied the gross anatomy of the extrinsic and intrinsic scapular and brachial muscles. METHODS: Gross dissections were performed in both thoracic limbs of four formaldehyde-fixed specimens. RESULTS: Three rhomboideus muscles were present (capitis, cervicis, and thoracis). The trapezius muscle was divided into two parts (cervicis and thoracis). The pectoralis abdominalis and omot

Assessment of copy number variations in 120 patients with Poland syndrome.

BACKGROUND: Poland Syndrome (PS) is a rare congenital disorder presenting with agenesis/hypoplasia of the pectoralis major muscle variably associated with thoracic and/or upper limb anomalies. Most cases are sporadic, but familial recurrence, with different inheritance patterns, has been observed. The genetic etiology of PS remains unknown. Karyotyping and array-comparative genomic hybridization (CGH) analyses can identify genomic imbalances that can clarify the genetic etiology of congenital and neurodevelopmental disorders. We previously reported a chromosome 11 deletion in twin girls with p

Technical considerations in pectoralis major transfer for treatment of the paralytic elbow.

Modification of pectoralis major transfer as originally described by Clark in 1946 have not addressed concerns such as diminished strength and excursion of the transfer, along with obligatory supination of the forearm. Postoperative scarring from the long oblique chest incision further compounds the psychological impairment that accompanies brachial plexopathy. One hundred forty-three brachial plexopathies were seen over a five-year period. Seven pectoralis major transfers were done to restore elbow flexion in patients with C5-6 and C5-6-7 cord injuries. Mean age and follow-up were 26 years an

Computational sensitivity analysis to identify muscles that can mechanically contribute to shoulder deformity following brachial plexus birth palsy.

PURPOSE: Two mechanisms, strength imbalance or impaired longitudinal muscle growth, potentially cause osseous and postural shoulder deformity in children with brachial plexus birth palsy. Our objective was to determine which muscles, via either deformity mechanism, were mechanically capable of producing forces that could promote shoulder deformity. METHODS: In an upper limb computational musculoskeletal model, we simulated strength imbalance by allowing each muscle crossing the shoulder to produce 30% of its maximum force. To simulate impaired longitudinal muscle growth, the functional length

Botulinum toxin as an adjunct to motor learning therapy and surgery for obstetrical brachial plexus injury.

Following obstetrical brachial plexus injury, infants are unable to learn specific patterns of movement due to the disruption of neural pathways. Even with successful reinnervation (spontaneously or post surgical reconstruction), function can be suboptimal due to overactivity in antagonist muscles preventing movement of reinnervated muscles. Botulinum toxin type A (BTX-A) was used to temporarily weaken antagonistic muscles early in the reinnervation process following brachial plexus injury, with the aim of facilitating functional improvement. A case series of eight children (five females, thre

Scapular Winging Caused by Combined Palsy of the Spinal Accessory Nerve and the Long Thoracic Nerve: A Case Report.

CASE: A 37-year-old man with scapular winging, caused by combined palsy of the spinal accessory nerve and the long thoracic nerve, was successfully treated with a pectoralis major transfer to substitute for the serratus anterior muscle, and with levator scapulae and rhomboid muscle transfers to substitute for the trapezius muscle. CONCLUSION: The serratus anterior paralysis was thought to have occurred secondary to traction of the long thoracic nerve by the unsupported scapula following the spinal accessory nerve palsy. Even with combined paralyses of the serratus anterior and trapezius muscl

An uncommon, unilateral motor variation of the intercostobrachial nerve.

The intercostobrachial nerve (ICBN) is commonly defined as a purely sensory nerve supplying the skin of the lateral chest wall, axilla, and medial arm. However, numerous branching patterns and distributions, including motor, have been reported. This report describes an uncommon variant of the right ICBN observed in both an 86-year-old white female cadaver and a 77-year-old white male cadaver. In both cases the ICBN presented with an additional muscular branch, termed the "medial pectoral branch", piercing and therefore innervating the pectoralis major and minor muscles. Clinically, the ICBN is

Muscle function during takeoff and landing flight in the pigeon (Columba livia).

This study explored the muscle strain and activation patterns of several key flight muscles of the pigeon (Columba livia) during takeoff and landing flight. Using electromyography (EMG) to measure muscle activation, and sonomicrometry to quantify muscle strain, we evaluated the muscle function patterns of the pectoralis, biceps, humerotriceps and scapulotriceps as pigeons flew between two perches. These recordings were analyzed in the context of three-dimensional wing kinematics. To understand the different requirements of takeoff, midflight and landing, we compared the activity and strain of

The respiratory system influences flight mechanics in soaring birds.

The subpectoral diverticulum (SPD) is an extension of the respiratory system in birds that is located between the primary muscles responsible for flapping the wing1,2. Here we survey the pulmonary apparatus in 68 avian species, and show that the SPD was present in virtually all of the soaring taxa investigated but absent in non-soarers. We find that this structure evolved independently with soaring flight at least seven times, which indicates that the diverticulum might have a functional and adaptive relationship with this flight style. Using the soaring hawks Buteo jamaicensis and Buteo swain

Comparison of immunohistochemistry and Ziehl-Neelsen staining for detecting the distribution of Mycobacterium avium subsp avium in naturally infected domestic Pekin ducks (Anas platyrhynchos domestica).

In order to detect the distribution of Mycobacterium avium subsp avium (MAA) in naturally infected domestic Pekin ducks, immunohistochemistry (IHC) and Ziehl-Neelsen (ZN) staining were used and compared. Six organs, the liver, spleen, lung, kidney, duodenum and pectoralis muscle, were collected from naturally infected Pekin ducks. Paraffin embedded tissues were examined, and the results were compared. Statistical analysis was performed using Chi-Square test. The results showed that the detection rates by IHC were similar with ZN staining in liver, lung, spleen and pectoralis muscle, but the de
