Chlamydia pecorum Ovine Abortion: Associations between Maternal Infection and Perinatal Mortality.

Chlamydia pecorum is a common gastrointestinal inhabitant of livestock but infections can manifest in a broad array of clinical presentations and in a range of host species. While C. pecorum is a known cause of ovine abortion, clinical cases have only recently been described in detail. Here, the prevalence and sequence types (STs) of C. pecorum in ewes from a property experiencing high levels of perinatal mortality (PNM) in New South Wales (NSW), Australia, were investigated using serological and molecular methods. Ewes that were PNM+ were statistically more likely to test seropositive compare…

Perinatal mortality in Lagos University Teaching Hospital: a five year review.

BACKGROUND: An overwhelming proportion of the annual global perinatal deaths occur in developing countries. However few data on perinatal mortality are available from these countries. Reducing perinatal deaths requires adequate data that can be used to develop appropriate strategies. OBJECTIVES: To determine the perinatal mortality rate at the Lagos University Teaching Hospital (LUTH), the associated risk factors and the causes of perinatal deaths; and make recommendations to help reduce perinatal mortality. METHODS: A retrospective study of all deliveries in LUTH in the period between Janua…

Reducing infant mortality rates using the perinatal periods of risk model.

Despite decreases in the last 50 years, infant mortality rates in the United States remain higher than in other industrialized countries. Using overall infant mortality rates to determine the effectiveness of interventions does not help communities focus on particular underlying factors contributing to static, and sometimes increasing, community rates. This study was designed to determine and rank contributing factors to fetal-infant mortality in a specific community using the Perinatal Periods of Risk (PPOR) model. The PPOR model was used to map fetal-infant mortality for 1995 to 1998 in the

Perinatal mortality and morbidity rates among singleton, twin, and triplet gestations.

Objective: To determine rates of perinatal mortality and morbidity from 24 to 43 weeks gestation among singletons, twins, and triplets. Methods: Successfully linked data from 1992 Californian maternal and infant discharge records as well as birth and death certificates from acute care civilian hospitals were examined for perinatal mortality and morbidity. Perinatal mortality was defined as the sum of all stillbirths and neonatal deaths. Deliveries from 24 to 43 weeks gestation among singleton, twin, and triplet pregnancies were collected as separate data sets. Perinatal mortality was identified…

Improved perinatal mortality rates in 1001 patients with severe pre-eclampsia.

OBJECTIVE: To ascertain the change in perinatal mortality (PNM) rate over a period of 10 years in 1001 patients with severe pre-eclampsia. METHODS: Patients with severe pre-eclampsia before a gestational age of 34 weeks were managed expectantly. Initial treatment consisted of the administration of magnesium sulphate to prevent convulsions and dihydralazine to reduce blood pressure. Methyldopa alone or in combination with other oral antihypertensive drugs was started soon after admission. In order to prevent fetal death from abruptio placentae, the fetal heart rate was monitored at least four …

Perinatal mortality at the Ogun State University Teaching Hospital, Sagamu, Nigeria.

A study of perinatal mortality at the Ogun State University Teaching Hospital during the first 30 months (March 1989 to August, 1991) of the establishment of a separate neonatal unit was made. One-hundred-and-sixty-two (87.5/1000) of the 1852 deliveries were stillborn and 60 (35.5/1000) of the 1690 live births died in the early neonatal period. The overall perinatal mortality rate was 119.9/1000 deliveries. Of stillbirths, 82 per cent occurred in unbooked mothers and were diagnosed at presentation. The major causes of stillbirths were teenage pregnancy, elderly motherhood, high maternal parity…

Perinatal factors, periventricular haemorrhage and mortality in very low birthweight infants.

In a population of 225 very low birthweight infants born over a 21 month period the cerebroventricular system was scanned by ultrasound. One third of the infants developed a periventricular haemorrhage; in 41% of infants the haemorrhage was detected before an hour of age and 66% of all haemorrhages occurred within the first 24 hours. Statistically significant associations with periventricular haemorrhage included vaginal delivery, endotracheal intubation and intravenous sodium bicarbonate when this was administered in the first 24 hours. In a stepwise regression analysis, however, these and ot

Abruptio placentae and placenta previa: frequency, perinatal mortality, and cigarette smoking.

Abruptio placentae and placenta previa are responsible for at least half the excessive fetal and neonatal death associated with maternal smoking during pregnancy. Data from a large prospective study were analyzed to determine if stopping smoking reduced the frequency of these disorders. Mothers who stopped smoking had a 23% lower frequency of abruptio placentae and a 33% lower frequency of placenta previa than women who continued to smoke during pregnancy. The mothers who stopped smoking had only about half as many fetal and neonatal deaths due to abruptio placentae and one third fewer losses

Perinatal mortality in twin pregnancy: an analysis of birth weight-specific mortality rates and adjusted mortality rates for birth weight distributions.

The objective of this study is to compare the fetal mortality rate (FMR), early neonatal mortality rate (ENMR) and perinatal mortality rate (PMR) of twin and single births. It is based on a survey which was carried out in 22 Hospital Centers in Spain in 1980, and covered 1,956 twins born and 110,734 singletons born. The FMR in twins was 36.3/1000 and 8.8/1000 for singletons. The ENMR in twins was 36.1/1000 and 5.7/1000 for singletons. The PMR in twins was 71.1/1000 and 14.4/1000 for singletons. When birthweight-specific PMR in twin and singletons births are compared, there were no differences …

Association Between Income and Perinatal Mortality in the Netherlands Across Gestational Age.

IMPORTANCE: The association between household income and perinatal health outcomes has been understudied. Examining disparities in perinatal mortality within strata of gestational age and before and after adjusting for birth weight centile can reveal how the income gradient is associated with gestational age, birth weight, and perinatal mortality. OBJECTIVES: To investigate the association between household income and perinatal mortality, separately by gestational age strata and time of death, and the potential role of birth weight centile in mediating this association. DESIGN, SETTING, AND …

Decreasing trend in preterm birth and perinatal mortality, do disparities also decline?

BACKGROUND: In the Netherlands, several initiatives started after the publication of the PERISTAT findings that showed the perinatal mortality risk was higher than in other European countries. The objective of this study is 1) to report recent trends in perinatal mortality and in intermediate risk groups (preterm birth, congenital anomalies and small for gestational age (SGA)), 2) describing perinatal mortality risk among children born preterm, with congenital anomalies or SGA, and born in maternal high risk groups (parity, age, ethnicity and socio-economic status (SES)). METHODS: A nationwid…
