Gastroenterology and Neurology.

PURPOSE OF REVIEW: Just as gastrointestinal dysfunction may develop in the setting of neurologic disease, neurologic dysfunction may become evident in the setting of gastrointestinal disease. This article describes the range of neurologic features that have been described in three primary gastrointestinal diseases: celiac disease and gluten-related disorders, inflammatory bowel disease, and Whipple disease. Particular emphasis is placed on the controversial and evolving clinical picture of neurologic dysfunction in disorders of gluten sensitivity. RECENT FINDINGS: Gluten-related disorders, in

Directional spread of alphaherpesviruses in the nervous system.

Alphaherpesviruses are pathogens that invade the nervous systems of their mammalian hosts. Directional spread of infection in the nervous system is a key component of the viral lifecycle and is critical for the onset of alphaherpesvirus-related diseases. Many alphaherpesvirus infections originate at peripheral sites, such as epithelial tissues, and then enter neurons of the peripheral nervous system (PNS), where lifelong latency is established. Following reactivation from latency and assembly of new viral particles, the infection typically spreads back out towards the periphery. These spread e

Novel Cell-Based Assay for Alpha-3 Nicotinic Receptor Antibodies Detects Antibodies Exclusively in Autoimmune Autonomic Ganglionopathy.

BACKGROUND AND OBJECTIVES: Autoantibodies against α3-subunit-containing nicotinic acetylcholine receptors (α3-nAChRs), usually measured by radioimmunoprecipitation assay (RIPA), are detected in patients with autoimmune autonomic ganglionopathy (AAG). However, low α3-nAChR antibody levels are frequently detected in other neurologic diseases with questionable significance. Our objective was to develop a method for the selective detection of the potentially pathogenic α3-nAChR antibodies, seemingly present only in patients with AAG. METHODS: The study involved sera from 55 patients from Greece,

Acute dysautonomia and polyneuropathy.

A patient experienced an acute dysautonomia manifested by an inability to eat without vomiting, severe constipation, dry mouth, and orthostatic hypotension. One month later, a severe polyneuropathy supervened. The patient subsequently experienced an atonic bladder. Biopsy specimens obtained from the bowel wall disclosed an inflammatory infiltrate within the autonomic nerves consisting of lymphocytes and plasma cells. To our knowledge, this is the first time that inflammatory infiltrates have been demonstrated within the autonomic nerves in a patient with an acute dysautonomia, suggesting a rel

Evans syndrome associated with sterile inflammation of the central nervous system: a case report.

INTRODUCTION: Evans syndrome is a rare hematological disease commonly defined as Coombs-positive hemolytic anemia and immune thrombocytopenia. Pathophysiology of this disease involves decreased cluster of differentiation (CD)4+ T-helper cell counts, increased CD8+ T-suppressor cell counts, a decreased CD4/CD8 ratio, and reduced serum immunoglobulin G, M and A levels - indicating a complex immune dysregulation. Association with other autoimmune diseases has been described although involvement of the central nervous system has not been reported so far. CASE PRESENTATION: We here present a case

Beyond the joints: neurological involvement in rheumatoid arthritis.

Although arthritis is the most notable component, rheumatoid arthritis (RA) is a systemic inflammatory disorder where extra-articular manifestations are common; among them, central and peripheral nervous system involvement is frequent and associated with significant morbidity and, in some cases, reduced life span. It may produce a myriad of symptoms and signs ranging from subtle numbness in a hand, to quadriparesis and sudden death. Central and peripheral neurologic manifestations may arise from structural damage produced by RA in diarthroidal joints, by the systemic inflammatory process of th

Factor VIII prophylactic therapy reduces neurological complications in patients with Hemophilia A.

BACKGROUND: Bleeding in hemophiliacs can cause complications in the central and peripheral nervous system (CNS and PNS). The incidence of intracranial hemorrhage has reduced after the introduction of prophylactic treatment with factor VIII or IX, but the benefits of this therapy have not yet been evaluated on PNS complications. OBJECTIVE: The aim of this study was to determine the prevalence of neurological complications in hemophiliacs and verify the effect of prophylactic therapy in these patients, including PNS disorders. METHODS: We retrospectively evaluated the prevalence of CNS and PNS

[Motor neuron disease: primary lateral sclerosis and amyotrophic lateral sclerosis].

AIM: Primary lateral sclerosis and amyotrophic lateral sclerosis are amongst motor neuron diseases. Differences between these two disorders are stressed by this paper. SOURCE OF DATA: Articles pertinent to this subject from the past 10 years. CONCLUSION: Both disorders are of neurodegenerative pathogenesis, and motor neurons are selectively involved. Unless only motor neurons from central nervous system are involved in primary lateral sclerosis, in amyotrophic lateral sclerosis motor neurons are involved both in central and in peripheral nervous system. Clinical neurophysiological and radiol

Role of calpains in the injury-induced dysfunction and degeneration of the mammalian axon.

Axonal injury and degeneration, whether primary or secondary, contribute to the morbidity and mortality seen in many acquired and inherited central nervous system (CNS) and peripheral nervous system (PNS) disorders, such as traumatic brain injury, spinal cord injury, cerebral ischemia, neurodegenerative diseases, and peripheral neuropathies. The calpain family of proteases has been mechanistically linked to the dysfunction and degeneration of axons. While the direct mechanisms by which transection, mechanical strain, ischemia, or complement activation trigger intra-axonal calpain activity are

[Advances in the role of meningeal immunity in viral infectious diseases].

The brain is protected by the blood-brain barrier and surrounded by the meninges, which are classically composed of dura, arachnoid and pia mater from superficial to deep layers. In recent years, a series of scientific breakthroughs has demonstrated the presence of the lymphatic system and immune cells in the meninges. Meningeal lymphatic vessels have recently been recognized as important participants in exchange of materials between cerebrospinal fluid and peripheral blood, as well as in immune surveillance. These studies suggest that the brain is no longer considered an "immune-privileged or

Movement disorders with autoimmune neuromuscular origin: an overview of Isaacs' syndrome, stiff person syndrome, immune-mediated rippling muscle disease.

Movement disorders associated with neuromuscular diseases are often underrecognized, yet they represent a distinct clinical entity. Abnormalities in areas of the peripheral nervous system, i.e., nerves and muscles, can stem from dysfunction of ion channels and proteins involved in membrane excitability. Hyperexcitability of peripheral motor nerves presents as cramps, stiffness, abnormal posture and gait, as well as changes in motor unit potentials during electromyography studies; hence, it may be classified as a movement problem. Etiologies range from hereditary, immune-mediated, or may be sec

Updates on the Diagnosis and Treatment of Peripheral Autonomic Neuropathies.

PURPOSE OF REVIEW: Autonomic neuropathies are a complex group of disorders and result in diverse clinical manifestations that affect the cardiovascular, gastrointestinal, urogenital, and sudomotor systems. We focus this review on the diagnosis and treatment of peripheral autonomic neuropathies. We summarize the diagnostic tools and current treatment options that will help the clinician care for individuals with peripheral autonomic neuropathies. RECENT FINDINGS: Autonomic neuropathies occur often in conjunction with somatic neuropathies but they can also occur in isolation. The autonomic refl

Histopathological evaluation of the nervous system in National Toxicology Program rodent studies: a modified approach.

This article outlines the changes and underlying rationale for modifications to the histopathological evaluation of the nervous system during toxicology and carcinogenesis studies conducted by the National Toxicology Program (NTP). In the past, routine evaluation of the nervous system was mostly limited to three sections of brain, and occasionally the spinal cord and peripheral nerves. Factors such as the increasing occurrence of human neurological diseases and associated economical cost burden, the role of unidentified environmental stressors in neurodegenerative disorders, multiple therapeut

Acute and chronic neuropathies: new aspects of Guillain-Barré syndrome and chronic inflammatory demyelinating polyneuropathy, an overview and an update.

During the last 15 years new information about clinical, electrophysiological, immunological and histopathological features of acute and chronic inflammatory neuropathies have emerged. Thus, the Guillain-Barré syndrome (GBS) is no longer considered a simple entity. Subtypes of the disorder besides the typical predominant motor manifestation, are recognized, i.e. a cranial nerve variant with ophthalmoplegia, ataxia and areflexia, an immune-mediated primary motor axonal neuropathy (AMAN), and a motor-sensory syndrome (AMSAN). Also, the clinical pattern of GBS is related to preceding viral or bac

Peripheral Neuropathy: A Phenotype-Driven Review for Diagnosis and Management.

The purpose of this narrative review is to provide a clinic-ready synthesis of contemporary concepts in peripheral neuropathy, spanning epidemiology, diagnosis, and treatment, with emphasis on high-yield advances applicable to daily practice. The authors integrate pragmatic tools-including a diagnostic algorithm, suggested initial laboratory panels, and commonly used outcome measures-to support clinical decision-making. However, this review is intended as a clinic-oriented synthesis rather than a formal practice guideline. Peripheral neuropathy can be systematically categorized into seven path

Peripheral neuropathies.

The present chapter is an overview of peripheral neuropathies for which placebo and nocebo responses and/or placebo and nocebo effects have been investigated to date. Even if most of our knowledge in this field comes from pain studies, a rapidly expanding picture emerges from the original research works conducted over the past 7 years on: painful diabetic neuropathy, postherpetic neuralgia, neuropathic complications of thoracotomy, carpal tunnel syndrome, Guillain-Barré syndrome, and chronic inflammatory demyelinating polyneuropathy (CIPD). Otherwise, very little, if anything, is known for the

Top 10 Clinical Pearls in Vasculitic Neuropathies.

Vasculitic neuropathies are a diverse group of inflammatory polyneuropathies that result from systemic vasculitis (e.g., polyarteritis nodosa, granulomatosis with polyangiitis, eosinophilic granulomatosis with polyangiitis, microscopic polyangiitis), vasculitis resulting from rheumatological disorders (e.g., rheumatoid arthritis and Sjögren's syndrome), paraneoplastic conditions, viruses, and medications. Occasionally, vasculitis is restricted to the peripheral nerves and termed nonsystemic vasculitic neuropathy. Presenting with an acute or subacute onset of painful sensory and motor deficits,
