The effect of medical therapy and antireflux surgery on dysphagia in patients with gastroesophageal reflux disease without esophageal stricture.

BACKGROUND: Poor esophageal body motility and trapping of the hernial sac by the hiatal crura are the major pathomechanisms of gastroesophag … al esophageal body motility and the laparoscopic Toupet fundoplication in 29 patients with impaired peristalsis. Dysphagia was assessed prior to treatment, at 6 months of medical therapy, and at 6 months after surgery. RESULTS: Heartburn and esophagitis were effectively treated by medical and surgical therapy. Only surgery improved regurgitation. Dysphagia i …

Isoperistaltic gastric tube: a surgical alternative in a pharyngoesophageal burn - a clinical case.

INTRODUCTION: Caustic burns still cause complex esophageal lesions in the pediatric population. However, therapeutic possibilities in severe … r a neoesophagus of appropriate length, an optimal vascularization for the graft, and physiological peristalsis. COMMENTS: The surgical approach proposed allows the esophagus to be irrigated from the right gastro-omental artery, thus preserving irrigation of the greater curvature. It also allows for a longer esophagus, and thanks to anatomical positioning, f …

Interstitial cells of Cajal in the normal human gut and in Hirschsprung disease.

Hirschsprung disease (HD) is the most prevalent congenital gastrointestinal motility disorder. The pathogenesis of HD is defined as a functional intestinal obstruction resulting from a defect in the intrinsic innervation of the distal bowel. In addition to the enteric nervous system, the interstitial cells of Cajal (ICC) play an important role in the generation of coordinated gastrointestinal peristalsis. The major function of the ICCs is the generation of slow waves that allow these cells to act as specialised pacemaker cells within various tissues. ICCs have additional functions in the gastr…

[Effect of synthetic motilin on gastric motility (author's transl)].

The effects of the synthetic motilin on the gastric motility were investigated in the dogs with innervated antral pouches in fasting state. The results were as follows: 1) The synthetic motilin stimulated markedly the gastric motor activity in both the contraction pressure and the frequency of the peristalsis. 2) Tetragastrin produced more regular and higher frequent contractions compared with synthetic motilin, but acted to weaker the contraction pressure. 3) In simultaneous administration of the synthetic motilin and the tetragastrin, the synthetic motilin acted more strongly in the contract…

Small bowel motility assessment with magnetic resonance imaging.

PURPOSE: To assess primarily the feasibility of magnetic resonance imaging (MRI) as a tool to monitor small bowel peristaltic motion and sec … inetic motion enhancers were investigated. RESULTS: Small bowel diameter measurement resulted in a peristalsis of 10.96 (SD = +/-2.51) waves per minute, oscillating regularly with mean amplitudes of 6.65 mm (SD = +/-1.15 mm). Peristaltic frequency in normal individuals is consistent with that observed with other techniques. Intravenous administration of scop …

Ex vivo motility in the base of the rabbit caecum and its associated structures: an electrophysiological and spatiotemporal analysis.

We examined the coordination between contractile events at different sites in the basal portion of the rabbit caecum and its associated structures that were identified by electrophysiological recordings with simultaneous one-dimensional, and a novel two-dimensional, spatiotemporal mapping technique. The findings of this work provide evidence that the caecum and proximal colon/ampulla coli act reflexly to augment colonic outflow when the caecum is distended and mass peristalsis instituted, the action of the latter overriding the inherent rhythm and direction of haustral propagation in the adjac…

The esophagus in collagen disease. Radiologic findings.

A prospective cineradiographic and manometric study of pharyngeal and esophageal dysfunction was performed in 54 patients with rigidly defined scleroderma, polymyositis, systemic lupus erythematosus, and rheumatoid arthritis. Cineradiography was inaccurate in pharyngeal dysfunctions specific for polymyositis. Manometry was an accurate but insensitive test. Manometry was also significantly more accurate than cineradiography in identifying patients with smooth muscle dysfunction (scleroderma). Cineradiographic finding of absent esophageal peristalsis accurately identified patients with scleroder…

The rationale is that the cessation of peristalsis should yield a more stable intragastric visual field to enhance the detection of early GC. Hyoscine and glucagon are commonly used as antiperistaltic agents. Both, however, must be given either intravenously or via intramuscular injection. They both …

Optimization of the Multi-Channel Surface Electrogram Acquisition for Noninvasive Uterine Electrophysiology Imaging.

PURPOSE: The goal of optimizing electrogram acquisition in electrophysiology imaging is to minimize the number of electrodes used, determine their optimal placement, and ensure accurate imaging. METHODS: We quantitatively evaluated the impact of the electrode number and placement on the accuracy of human uterine peristalsis imaging (UPI) using adapted minimum nonzero eigenvalue pursuit (MNEP), adapted maximal projection on minimum eigenspace (MPME), and a distance-based baseline method. Specifically, we assessed the accuracy of the uterine surface electrogram imaged by UPI while maintaining t…

Major mixed motility disorders: An important subset of esophagogastric junction outflow obstruction.

BACKGROUND: Esophagogastric junction outflow obstruction (EGJOO) is an esophageal motility disorder characterized by a lack of relaxation of the esophagogastric junction (EGJ), with preserved esophageal body peristalsis. We propose new terminology for the coexistence of EGJOO with hypercontractile esophagus and distal esophageal spasm as a major mixed motility disorder (MMMD), and normal peristalsis or a minor disorder of peristalsis such as ineffective esophageal motility with EGJOO as isolated or ineffective EGJOO (IEGJOO). METHODS: We reviewed prior diagnoses of EGJOO, stratified diagnoses…

Implantation failure of endometrial origin: what is new?

PURPOSE OF REVIEW: To review recent findings related to possible causes of recurrent implantation failure of endometrial origin in normal uterus. RECENT FINDINGS: Recent evidences suggest that in apparently normal endometria, RIF may associate with molecular and functional changes in the uterus such as abnormal endometrial microbiota, including the presence of chronic endometritis, poor synchronization between the blastocyst and endometrium, and/or excessive uterine peristalsis. An altered endometrial microbiota detected by molecular techniques has been recently related to poorer embryo impla…

Changes in the Treatment of Primary Esophageal Motility Disorders Imposed by the New Classification for Esophageal Motility Disorders on High Resolution Manometry (Chicago Classification 4.0).

The Chicago Classification is the most used classification for primary esophageal motility disorders (PEMD). This classification was recently updated to the 4.0 version. This opinion piece focuses on the possible implications for the treatment of PMED determined by the new classification. Chicago Classification 4.0 included two new concepts for the diagnosis of achalasia: (1) type III achalasia diagnosis demands 100% absent peristalsis defined as either failed peristalsis or spasm; (2) "inconclusive diagnosis of achalasia" was added as a possibility. Both may decrease unnecessary treatment. Es…

A bioinspired apparatus for modeling peristaltic pumping in biophysical flows.

In this study, we present a novel, bioinspired experimental apparatus, its construction, data acquisition methodology, and validation for the study of peristaltic flows. The apparatus consists of a series of stepper motor actuators, which deflect a deformable membrane to produce peristaltic flows. We show that this apparatus design has significant advantages over previous designs that have been used to study peristaltic flows by offering a much wider range of modeling capabilities. Comparisons between the capabilities of our apparatus and previous ones show our apparatus spanning a larger rang

The anatomy and movements of the pyloric sphincteric cylinder.

Disagreement about various aspects of the structure and function of the pylorus persists. Morbid anatomical, living anatomical, manometric and radiographic studies were done in an attempt to clarify some problems. It is shown that the pyloric ring (sphincteric ring) is not a separate anatomical structure, but that it constitutes the aboral end of the pyloric sphincteric cylinder, a muscular tube several centimeters in length. The ring does not function independently and conventional peristaltic waves do not proceed as far as the ring. Consequently the ring does not relax reciprocally with an o

The inability to belch - A neglected disease in the era of high-resolution esophageal manometry.

Retrograde upper esophageal sphincter dysfunction (R-UESD) is characterized by the inability to belch. Evidence of using high-resolution manometry (HRM) in diagnosing R-UESD has emerged in recent years. We describe the clinical picture and HRM patterns of two patients with R-UESD. Case 1: A 23-year-old female presented with a two-year complaint of inability to belch. We performed HRM with a belch provocation test for which the patient drank 500 ml of carbonated water. The study revealed increased esophageal pressure, an absence of UES relaxation and secondary peristalsis once the patient menti…
