Three percent to 30% of the episodes of peritonitis occurring in patients undergoing chronic peritoneal dialysis are culture-negative or aseptic. The etiology of these episodes remains poorly defined, though endotoxin-contaminated dialysates and fastidious anaerobic organisms occasionally have been implicated. We treated a patient who had fever, epigastric pain, and peritoneal fluid neutrophilic leukocytosis while undergoing chronic peritoneal dialysis. Despite multiple negative pretherapy aerobic, anaerobic, fungal, and mycobacterial cultures, bacterial peritonitis was the presumptive diagnos…

Clinical features and morphology of circumscribed, general, and diffuse peritonitis were studied on the clinical material including 66 observations and in 320 small animals. In the reactive stage of the process because of rapidly increasing toxicity microcirculation was shown to be disturbed, immunoglobulins were fixed in basal membranes of the microvessels and peritoneum, endotheliocytes and mesotheliocytes decreased in number which facilitated marked plasmorrhagy but prevented migration of leukocytes from the vessels. The function of leukocytes and nonspecific resistance of the organism incr…

Five cases of primary peritonitis are presented, with a sixth related case of pneumococcal peritonitis secondary to a ruptured ovarian cyst. The patients comprised 4 young girls who recovered, and 2 elderly females who died. Pneumococci were isolated in 3 patients; no organism was found in the other 3. One case of primary measles peritonitis has no apparent precedent.

About 11 000 autopsies were done in Darmstadt from 1955 to 1977; 879 (8%) of the cases had had peritonitis. The incidence of acute peritonitis was highest (56,8%), followed by acute local peritonitis (26,6%), peritoneal abscess and covered perforation (11,4%) and strangulation ileus (3,8%). Peritonitis had been the cause of death in 56%; in 37% of the cases peritonitis was an essential finding, in 6,3% a minor finding. A comparison was made with statistics of peritonitis at postmortem in Berlin in 1908; total incidence of peritonitis was about the same, however peritonitis originating from app…

Previous reports have implicated the Dalkon shield intrauterine device (IUD) in septic second trimester abortion and maternal death from sepsis. In the case reported here, Fusobacterium necrophorum, a rarely pathogenic vaginal anaerobe, gained access to the uterus in a woman wearing a Dalkon shield and caused acute parametritis, overwhelming peritonitis, systemic sepsis, and death. It is well known that IUDs can contribute to the development of serious pelvic infections, and in this case it is possible that the Dalkon shield was the cause of the ascending infection into the uterus.

The high risk of peritonitis following a perforation of the gall bladder is discussed in a short review of the literature. In 15 years 9084 operations of the bile ducts were performed at Nord-West-Hospital, Franfurt/M., among these were 36 cases of free gall bladder perforation. Our experiences with abdominal drainage only, without perfusion of the abdomen following cholecystectomy and revision of the bile ducts with simultaneous systemic appliance of antibiotics are reported.
