Development and validation of AI-assisted transcriptomic signatures to personalize adjuvant chemotherapy in patients with pancreatic ductal adenocarcinoma.

BACKGROUND: After surgical resection of pancreatic ductal adenocarcinoma (PDAC), patients are predominantly treated with adjuvant chemotherapy, commonly consisting of gemcitabine (GEM)-based regimens or the modified FOLFIRINOX (mFFX) regimen. While mFFX regimen has been shown to be more effective than GEM-based regimens, it is also associated with higher toxicity. Current treatment decisions are based on patient performance status rather than on the molecular characteristics of the tumor. To address this gap, the goal of this study was to develop drug-specific transcriptomic signatures for per

Development of a machine learning algorithm to identify surgical candidates for hip and knee arthroplasty without in-person evaluation.

INTRODUCTION: Arthroplasty care delivery is facing a growing supply-demand mismatch. To meet future demand for joint arthroplasty, systems will need to identify potential surgical candidates prior to evaluation by orthopaedic surgeons. MATERIALS AND METHODS: Retrospective review was conducted at two academic medical centers and three community hospitals from March 1 to July 31, 2020 to identify new patient telemedicine encounters (without prior in-person evaluation) for consideration of hip or knee arthroplasty. The primary outcome was surgical indication for joint replacement. Five machine l

Development of Moral Judgments in Impersonal and Personal Dilemmas in Autistic Spectrum Disorders from Childhood to Late Adolescence.

A potential underlying mechanism associated with the difficulties in social interactions in Autistic Spectrum Disorders (ASD) concerns the abnormal development of moral reasoning. The present study examined utilitarian and deontological judgments in impersonal and personal moral dilemmas, comparing 66 individuals with ASD and 61 typically developing (TD) individuals between 6 and 18 years. Utilitarian judgments decreased with age. This decline was much more gradual for personal dilemmas in the ASD than in the TD group. ASD individuals rated utilitarian judgments as more appropriate but felt le

Developing and Validating the Narcissistic Personality Scale (NPS) among Older Thai Adults Using Rasch Analysis.

BACKGROUND: Being older could be stressful, especially among people with narcissistic personality disorders. Nevertheless, the tool is yet to be available among older Thai individuals. The study aimed to develop a tool to detect symptoms of narcissistic personality, and to validate its psychometric properties among older Thai adults. METHODS: The Narcissistic Personality Scale (NPS) was developed based on nine domain symptoms of narcissistic personality disorder from the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5), consisting of 80 items. The original scale wa

Personalized Medicine and Cervical Screening: Development of Individualized Quantitative Risk Assessments for Cervical Adenocarcinoma and Adenocarcinoma in situ.

INTRODUCTION: Cervical screening has decreased the incidence of cervical carcinoma around the world primarily by preventing cervical squamous carcinoma, with significantly less measurable protective benefits in prevention of cervical adenocarcinoma. In this study, we apply Bayesian modeling of cervical clinical, screening, and biopsy data from a large integrated health system to explore the feasibility of calculating personalized risk assessments on screened system patients for subsequent histopathologic diagnoses of invasive cervical adenocarcinoma (AdCa) or cervical adenocarcinoma in situ (A

Tailored modern GERD therapy - steps towards the development of an aid to guide personalized anti-reflux surgery.

As the incidence of gastroesophageal reflux disease (GERD) is rising, surgical treatment is continuously advancing in an effort to minimize side effects, whilst maintaining efficacy. From a database of patients that underwent anti-reflux surgery at our institution between 2015 and 2018, the last 25 consecutive patients that underwent electrical stimulation (ES), magnetic sphincter augmentation (MSA) and Nissen fundoplication (NF), following a personalized treatment decision aid, were included in a comparative analysis. After preoperative evaluation each patient was referred for an ES, MSA or N

Development of a thoracic personal sampler system for co-sampling of sulfuric acid mist and sulfur dioxide gas.

A novel personal sampler was designed to measure inorganic acid mists and gases for determining human exposure levels to these acids in workplaces. This sampler consists of (1) a parallel impactor for classifying aerosol by size following the ISO/CEN/ACGIH defined human thoracic fraction, (2) a cellulose filter to collect the residual acid mist but allowing penetration of sulfur dioxide gas, and (3) an accordion-shaped porous membrane denuder (aPMD) for adsorbing the penetrating sulfur dioxide gas. Acid-resistant PTFE was chosen as the housing material to minimize sampling interference. To tes

Genetically reduced FAAH activity may be a risk for the development of anxiety and depression in persons with repetitive childhood trauma.

Fatty acid amide hydrolase (FAAH) inhibitors are addressed for promising anxiolytics, but human studies on genetically reduced FAAH activity, stress and affective phenotypes are scarce. We investigated the effect of a functional polymorphism of FAAH (FAAH C385A or rs324420; low FAAH activity and high anandamide concentration are associated with the A allele) together with childhood adversity on the anxious and depressive phenotypes in 858 subjects from the general population. Phenotypes were measured by the Zung Self-Rating Depression Scale (ZSDS), the depression and anxiety subscales of the B

From idealistic helper to enterprising learner: critical reflections on personal development through experiences from Afghanistan.

There is little written about the cultural, social, and ethical challenges encountered by physiotherapists engaging in development work. This article takes a critical perspective on what it means to engage in development work as an expatriate physiotherapist, through a self-critical reflection on experiences from Afghanistan. The field notes from an ethnographic study of a development project conducted in Afghanistan were analysed to explore the transformative process of personal and professional development of the development worker. The critical reflective process entailed a change in meanin

Development of a job evaluation system to predict job placements for persons with mild mental retardation: a pilot study.

An attempt to investigate the types of jobs held by people with mild mental retardation in Hong Kong has been made by this pilot study, to evaluate the job-related skills required and to build the foundation for the development of a computerized job evaluation system. A local market survey was conducted, and 14 supported employment staff, five employers and 38 people with mild mental retardation were interviewed. Eleven job analyses were also conducted. All of the interviews were carried out with an interview guide sheet developed from O*NET. The majority of the jobs were unskilled (70.4%) and

The development of personal computer-based medical consultation system for diagnosis of congenital malformation syndromes using MUMPS.

This paper describes a medical consultation system for diagnosis of congenital malformation syndromes. The system consists of a personal computer, NEC PC-98XL (MS-DOS), using SP-MUMPS for the retrieval of knowledge data, and BASIC for the input/output of image data. This system displays pictorial knowledge and is designed so users can build up the knowledge base, as needed, by adding knowledge from medical literature and records of patients actually treated.

The mediation effect of personality functioning between different types of child maltreatment and the development of depression/anxiety symptoms - A German representative study.

BACKGROUND: Child maltreatment (CM) is associated with an increased risk to develop symptoms of depression/anxiety across an individual's lifespan. Recent studies indicated that impairments in personality functioning might mediate this association. The purpose of this study is to add evidence of this mediating effect by regarding different types of CM (emotional, physical and sexual abuse as well as emotional and physical neglect) in the general population. METHODS: A representative sample of the German population (N = 2,354) completed a set of standardized measures (OPD-SQS: Operationalized

[Follow-up study of school age children having received inpatient treatment for infant atrophy, with regard to their personality development and social adjustment].

In the personality development controlled by complicated and numerous influences we searched for those characteristics which can be made responsible for infantile atrophy. In comparison with children of average development and of the same age we found that in addition to a somatic retardation the intellectual efficiency is impaired in 60 percent of the cases; we were also able to find disorders of the emotional circle. All of these result in the disharmony of personality. In the capacities for adaptation the narrow, familial, environmental influences appear to be of importance in that in contr…

Recovery in personality disorders: the development and preliminary testing of a novel natural language processing model to identify recovery in mental health electronic records.

INTRODUCTION: The concept of recovery is of great importance in mental health as it emphasizes improvements in quality of life and functioning alongside the traditional focus on symptomatic remission. Yet, investigating non-symptomatic recovery in the field of personality disorders has been particularly challenging due to complexities in capturing the occurrence of recovery. Electronic health records (EHRs) provide a robust platform from which episodes of recovery can be detected. However, much of the relevant information may be embedded in free-text clinical notes, requiring the development o

Developing culturally appropriate physical activity promotion strategies for older Chinese adults in the UK: an evidence-based, theory-based and person-based intervention development study.

BACKGROUND: Older Chinese adults in the UK face unique needs when engaging in physical activity (PA), yet culturally appropriate strategies to promote PA remain limited. OBJECTIVE: To develop culturally appropriate PA promotion strategies for older Chinese adults living in the UK. METHODS: The intervention development comprised two stages: design and optimisation. It was informed by previous research findings and input from public and community involvement and engagement. Stage 1 combined the person-based approach (PBA), which involved developing guiding principles, an intervention planning
