Percutaneous Peripheral Nerve Stimulation of the Brachial Plexus for Intractable Phantom Pain of the Upper Extremity: A Case Report.

Phantom limb pain is a common condition occurring after amputations. Percutaneous peripheral nerve stimulation (PNS) has been reported to provide analgesia for established lower extremity phantom pain. However, this modality has never been applied to upper extremity phantom pain. A patient presented with acute exacerbation of established upper extremity phantom pain 2 years following forequarter amputation. A percutaneous PNS lead placed adjacent to the patient's brachial plexus under ultrasound guidance provided analgesia of the phantom pain for several weeks. PNS of the brachial plexus may p…

Postamputation pain: studies on mechanisms.

Amputation is followed by both painful and non-painful phantom phenomena in a large number of amputees. Non-painful phantom sensations rarely pose any clinical problem, but 60-80% of all amputees also experience painful sensations (i.e. phantom pain) located to the missing limb. The severity of phantom pain usually decreases with time, but severe pain persists in 5-10% of patients. Pain in the residual limb (i.e. stump pain) is another consequence of amputation. Both stump and phantom pain can be very difficult to treat. Treatment guidelines used for other neuropathic pain conditions are proba

Identification of phantom movements with an ensemble learning approach.

Phantom limb pain after amputation is a debilitating condition that negatively affects activities of daily life and the quality of life of amputees. Most amputees are able to control the movement of the missing limb, which is called the phantom limb movement. Recognition of these movements is crucial for both technology-based amputee rehabilitation and prosthetic control. The aim of the current study is to classify and recognize the phantom movements in four different amputation levels of the upper and lower extremities. In the current study, we utilized ensemble learning algorithms for the re…

And Yet It Moves: What We Currently Know about Phantom Arm Movements.

What is left over if I subtract the fact that my arm goes up from the fact that I raise my arm? Neurological evidence invites the provocative hypothesis that what is left over is a phantom arm movement-a movement of an arm that has been amputated. After arm/hand amputation, many amputees report that they can generate voluntary movements of the phantom limb; that is, they can move the arm that was amputated. But what is it like to move an arm/hand that is not there? Here, we review what is currently known about phantom limb movements at three descriptive levels: the kinematic level, the muscle …

Development of a Multimodal Analgesia Protocol for Perioperative Acute Pain Management for Lower Limb Amputation.

Multimodal analgesia may include pharmacological components such as regional anesthesia, opioid and nonopioid systemic analgesics, nonsteroi … h as chronic ischemic limb pain, postoperative residual limb pain, coexisting musculoskeletal pain, phantom limb sensations, and chronic phantom limb pain. These pain mechanisms are explored and a proposed protocol for multimodal analgesia is outlined taking into account the common patient comorbidities found in this patient population.

Phantom radiculopathy: a rare postoperative phenomenon.

Limb amputations are carried out for a number of reasons, which include trauma, vascular disorders, infection, oncology and congenital abnormalities. These patients can develop multiple complications postoperatively with phantom limb pain being a well-recognised issue. That being said, phantom radiculopathy is far less encountered and can therefore be easily overlooked. There are limited cases described in literature and as a result pathophysiology is poorly understood. In this report, we present a patient who had developed phantom radiculopathy decades after his left above knee amputation sur…

Successful treatment of phantom radiculopathy with fluoroscopic epidural steroid injections.

Phantom limb pain has been well described in the literature. However, new-onset lumbar radicular pain superimposed on baseline lower extremity phantom pain is a clinical scenario that can be challenging to recognize. Furthermore, literature on recognition and treatment of phantom radiculopathy is all but lacking. We present a patient who experienced new-onset lumbar radiculopathy superimposed on her phantom pain that was successfully treated with fluoroscopic interlaminar and transforaminal epidural steroid injections.

[Phantom pain and posttraumatic pain conditions].

After limb or body part amputation, three different types of perceptual sensitive phenomena can be recognized. They can be all named posttraumatic neuropathies: painless sensations in phantom limb, painful phantom limb and painful posttraumatic stump. Painless sensations in phantom limb can be seen in 90% of cases in resected body parts as soon as first postoperative day, less often during the first week, and its clinical characteristics are usually stabilized during the first year. Painful posttraumatic stump appears because of pain neuroma existing, that forms at the proximal end of amputati…

Individualized Augmented Reality Training Reduces Phantom Pain and Cortical Reorganization in Amputees: A Proof of Concept Study.

Phantom limb pain (PLP) may be relieved using a visual representation of an intact limb. However, patients with distorted (telescoped) phantoms seem unable to associate with visualizations of intact limbs. A virtual arm visualization was matched to the individual's phantom perception and controlled in an augmented reality (AR) intervention. Seven PLP participants with telescoped phantoms performed 8 supervised home-based AR-training sessions (45 minutes each) within 2 weeks. The virtual arm was superimposed in AR onto their residual limb and controlled using electromyography from the residual …

Optimizing the Transradial Amputation.

Meticulous bony and soft tissue handling is paramount in optimizing prosthesis wear and minimizing neuropathic and phantom limb pain in patients with transradial amputations. Resection of 7 to 8 cm of radius and ulna proximal to the wrist joint preserves the majority of native forearm rotation. A pedicled pronator quadratus is a useful target for targeted muscle reinnervation and can provide additional distal soft tissue padding over the bone ends. Targeted muscle reinnervation or regenerative peripheral nerve interface techniques that place neurorrhaphy sites deep and distal within the forear…

On the syndrome of the "spare limb": one case.

We describe a case of a brain-damaged patient who had a peculiar bodily illusion which could not be labelled an hallucination but seemed somatognosically and phenomenologically similar to the phantom limb without amputation. The patient, who showed left hemiplegia, felt a third upper limb (without seeing it) which he himself defined as "spare." The spare limb was not deformed; it could be moved and controlled by the patient, and there was no sensation of pain. The patient did not show psychopathological or cognitive disorders. A possible interpretation of the phenomenon is as a "phantom moveme…

A sensory-motor hand prosthesis with integrated thermal feedback.

BACKGROUND: Recently, we reported the presence of phantom thermal sensations in amputees: thermal stimulation of specific spots on the residual arm elicited thermal sensations in their missing hands. Here, we exploit phantom thermal sensations via a standalone system integrated into a robotic prosthetic hand to provide real-time and natural temperature feedback. METHODS: The subject (a male adult with unilateral transradial amputation) used the sensorized prosthesis to manipulate objects and distinguish their thermal properties. We tested his ability to discriminate between (1) hot, cold, and

Pain and its absence in an unfortunate family of amputees.

A family is described in which 5 male members sustained major traumatic injuries of their limbs. Two of these men had amputations of two of their limbs. The one surviving amputee is left handed. The development of phantom sensations, phantom pain and stump pain was unpredictable, despite their being first-degree relatives, and was independent of handedness.

Best Practices for Anesthetic Management and Analgesia for the Patient Undergoing Amputation.

This review article summarizes best practices for the anesthetic management and analgesia of patients undergoing limb amputation, populations in which acute stump pain and chronic phantom limb pain are highly prevalent. Organized across the perioperative continuum, it examines preoperative pain optimization and preemptive analgesia, intraoperative anesthetic technique selection and pharmacologic adjuncts such as ketamine and alpha-2 agonists, and postoperative regional and multimodal systemic analgesia. The authors also address long-term phantom limb pain through neuroplasticity-based retraini…
